Subject material was chosen based on assessed needs, current research and future trends. This educational program was developed by family physicians for family physicians. AMA: The Georgia Academy of Family Physicians is accredited by the Medical Association of Georgia to provide continuing medical education for physicians. The Georgia Academy of Family Physicians designates this live activity for a maximum of 24 AMA PRA Category 1 Credits. Physicians should claim only the credit commensurate with the extent of their participation in the activity. AAFP: Application for CME credit has been filed with the American Academy of Family Physicians. Determination of credit is pending. AOA: This program has been reviewed and it is acceptable for up to 24 credits in Category 2-A by the American Osteopathic Association. Learning Action Network Workshop: Collaborative Care Training Primary Care Providers- Behavioral Health THURSDAY, OCTOBER 26 Integrated care programs, in which mental health care is delivered in primary care settings, exist as a promising solution to common, disabling and costly behavioral health problems, such as depression, anxiety and substance use disorders. Collaborative Care, a type of integrated care, has emerged as the strongest evidence-based approach. In this model, PCPs work with psychiatrists and Care Managers to deliver evidence-based care that is proven to improve outcomes, reduce costs, and improve the patient experience.
