Position Objective The role of the is to provide comprehensive, evidence based, holistic clinical care for clients from 0-18 years residing in the Lower Gulf Communities. The will work in collaboration with existing service providers to provide child health checks, immunisations and health education/health promotion in the clinic and community to provide an integrated child health service. They will use a family centred and culturally appropriate approach alongside Aboriginal Health Workers, our broader multidisciplinary team and other service providers, utilising a case management model of care. Primary Roles and Responsibilities with Key Performance Indicators Primary Roles and Responsibility Areas To achieve optimal health outcomes for all clients Key Performance Indicator (KPI) Evidence of high quality, discipline specific service provision including: 1. Providision of advanced clinical and community nursing services that include comprehensive needs assessment and screening for both health and wellbeing issues for babies, children and young people to 18 years of age and their families. Also providision of monitoring, on-going clinical care, immunisation, care planning, counselling, education and appropriate referral for client group. Services will be based in the Communities, in Clinics, in the Family Centre and Well- Being Centres. 2. Participating in service planning, implementation and evaluation in line with service development initiative and strategic direction. 3. Providing culturally appropriate and community inclusive child, youth and family health care within a culturally and contextually safe and sensitive framework that enhances empowerment and is supportive of individuals taking greater control of health issues that affect themselves and their families. 4. Actively contributing to the coordination, Measure Meeting contracted service targets 1
planning and implementation of child, youth and family health clinics and specialist health programs in liaison with the General Practitioners, Community based health care providers and visiting health services. 5. Initiating and maintaining linkages with other child and youth related services (eg early learning, family support etc) in the local area, as well as other health service providers. 6. Provision of integrated care through working in a collegial and collaborative partnership with relevant Community and Regional agencies, local service providers and organisations. Location: Travel Requirements: This position is based in Normanton and provides outreach services to Gulf Communities. Travelling by road and/or in small planes to communities within the place catchment area Criminal History Check: All final applicants for this position will be asked to consent to a criminal record check. Registration as a general nurse with post graduate qualifications in Child and Family Health and a current annual practising certificate. Appropriate registration with APHRA. 2. A minimum of five years of broad-based child health experience demonstrating a sound professional background and ability to self manage within an autonomous setting 3. Immunisation endorsement. 4. Demonstrated ability to provide quality, contemporary and safe child health and car to a high professional standard. 5. An understanding of issues affecting Aboriginal Health, principles of Primary Health Care and issues affecting work practice in rural and remote areas. 6. Demonstrated knowledge of the contemporary picture of Aboriginal health in Queensland and an understanding and sensitivity towards, Aboriginal and Torres Strait Islander people, including an understanding of the socioeconomic determinants of health, social and emotional wellbeing and cultural issues which impact upon health status. Ability to work in a culturally sensitive and community inclusive manner and a willingness to learn about local issues 7. Demonstrated commitment to quality improvement, education and professional development. 8. Demonstrated ability to operate independently as well as participate in a multidisciplinary team. 9. Awareness of health issues in the Gulf of Carpentaria and a demonstrated ability to work with people of diverse cultural backgrounds. 10. Current C class driver s licence and the ability to drive a 4WD. (Manual experience viewed favourably) 4
Desirable 11. Willingness to regularly undertake work in communities, travelling by road or light aircraft, up to several hours from the base location, often resulting in overnight stays (up to 3 nights) and possibly after hours work. 12. Computer literacy in Microsoft Office Suite, or willingness and capacity to learn. Familiarity with Medical Director or willingness and capacity to learn. 13.
