Aim of Telehealth To support people with long term conditions to develop confidence to manage their own health, improve health outcomes and reduce incidences of exacerbations, GP attendances and hospital admissions. Improve Patients Quality of Life The department of health is encouraging the use of technology as a tool to support the more traditional approach to health care. It does not replace clinical visits when needed, but provides clinical teams with information to make informed decisions about their patients.
