When using a data-driven process to select KOLs, particularly at the local level, knowing the size of a physician s clinical practice may also be an important determinant of your selection. Knowing which clinicians have high volume practices could be an indicator of thought leadership in the local provider community. For selecting KOLs who are clinicians working in inpatient or outpatient hospital facilities, adding available clinical volume data, such as the recorded number of procedures a physician performs or the number of patients seen with a specific diagnosis, may be valuable in your selection criteria. With insights into clinical volume at key facilities, you can uncover KOLs important to a hospital ecosystem who might not otherwise be identified through other data traditionally used for KOL identification. Clinical volume from Big Data For over a decade, Truven Health Analytics, part of the IBM Watson Health business, has been providing life sciences clients with Facility Targeting Reports generated from Big Data assets. The sample report below uses fictitious providers in Austin, Texas to illustrate top-volume physicians performing cholecystectomies. Total discharges for this procedure are calculated for each physician at each hospital and summed to all hospitals through physician decile ratings. Data used for these reports are sourced from the Truven Health Projected Inpatient Database and other large healthcare data sets. Our projection methodology uses the actual number of discharges from 73 percent of all US hospitals to project up to the total discharges from all US hospitals This report is for physicians performing cholecystectomies Figure 1. Using clinical volume in KOL selection Turning to KOL selection, we have incorporated clinical volume data from the same healthcare data sources we use for Facility Targeting Reports into our platform for selected US KOLs. So, in addition to traditional KOL selection criteria, you can now use available clinical volume to select KOLs. Below is an example. Bill Roche, MD, a fictitious physician who performs orthopedic surgery at Central Florida Memorial Hospital, a fictitious hospital in Tampa, Florida, is being considered for a specific KOL engagement. Figure 2. Sample Heartbeat Profiler physician profile page In addition to knowing his education, clinical trial experience, publications, social media experience and work with competitors, you d like to evaluate the size of his practice for the KOL engagement you have in mind. Is he the highest volume practitioner in the area? Looking at Dr. Roche s profile in Heartbeat Profiler, the Truven Health stakeholder management platform, you can identify the top procedures or diagnoses associated with his patients for the most recent three years of available data. You determine that Dr. Roche performed 137 hip replacement surgeries in the past year at Central Florida Memorial Hospital, which is the highest volume for that procedure compared to other colleagues in the Tampa Bay catchment area. If Dr. Roche also has privileges in other local hospitals, you can easily identify the facilities where he performs surgery. In this example, he performs the highest volume of surgeries at Central Florida Memorial Hospital, which is the leading hospital provider in the area for hip replacements in our example. The volume of procedures Dr. Roche performs can be selected along with other criteria such as clinical trial work, publications and social media experience in the Profiler segmentation analysis. This allows you to compare and rank Dr. Roche with others you are considering to select the top KOL for the engagement activities you are planning. Page 3

Summary Data from very large healthcare databases may add another important parameter to your KOL selection process, particularly at the local level, by allowing you to understand the size of a KOL s clinical practice. In the past, clinically based physicians may have been overlooked when using conventional identification approaches and they could be just the right KOLs for your business needs.
