About Meeker Memorial Hospital We re the hospital your father s father trusted. 1910. A long time ago to be sure. In medical terms, it s an eternity. But that s how far back Meeker Memorial Hospital s proud heritage of care reaches. What began as the Litchfield Hospital nearly a century ago has grown, adapted and advanced to offer the best care for the health needs of area residents. Our 35-bed facility in Litchfield, Minnesota, is at the forefront of medical technology, service and care. We are a county-owned, not-for-profit community hospital offering sophisticated diagnostic, surgical, rehabilitation, behavioral health and specialty services with the personal attention and respect that patients deserve. No tax dollars goes to support the hospital. Our outpatient services bring specialty care close to home by offering physician and program specialists for consultation, diagnostic, surgical and rehabilitation services. In order to provide Dassel-Cokato area residents with access to the best possible healthcare, we realized a local clinic with a highly skilled, full-time staff was the answer. Meeker Memorial Clinic was founded to provide each patient with involved and informed care on issues ranging from pregnancy, birth and health education, to chronic conditions, acute illnesses and aging concerns. At Meeker Memorial Clinic we offer fulltime physician coverage and expanded services to offer physical therapy on-site M, W, F and an OB/GYN physician in clinic on a weekly basis. From same day appointments to a medical home with specialized care, you have a staff at Meeker Memorial that cares about you and the community. 10

There s no such thing as a little thing in medical care. Pertaining to your health, everything is a big thing. Every aspect of the care experience matters. Your surroundings and your comfort within them affect you as much as any ailment. Our ability to attend to your needs is paramount to effective care. At MMH, our comprehensive care strategy reaches beyond the treatment itself. Complementing our diagnostic testing, surgery, and specialized and skilled nursing care services will always be an up-to-date and well-managed hospital setting with tailored services that meet your individual needs. To accommodate your family and visitors, we offer: TDD/handicapped-accessible facility Comfortable lounges Dining services Vending services Overnight accommodations (upon request) Open Door Gift Shop (Monday through Friday from 8:30 a.m. to 5 p.m., Saturdays from 9 a.m. to 1 p.m. and most Monday evenings.) Multi-denominational chapel Medical/Surgical Care/Critical Care Patients receiving treatment from MMH will be cared for in private rooms. Our critical care rooms are equipped with the most up-to-date technology. Swing bed and transitional care options are also available in the event you need nursing care after acute care needs have ended. Our insistence on a low nurse-to-patient ratio provides you with the individualized and intensive treatment you need. Our team of more than 40 nurses provide family-centered care for newborn to geriatric patients. 11

12 MMH s OB department is designed around making birth safe, easy and comfortable, with newly redesigned suites featuring pullout beds and whirlpool baths. MMH recently received the Women s Choice Award for outstanding obstetrical care. 2013 Women s Choice Award America s Best Hospitals Obstetrics Inpatient Services Family Birthing Experience Our goal is to make the birthing experience as comfortable as possible for everyone involved, but especially for mom and baby. We utilize a variety of advanced pain management techniques, ranging from epidural anesthesia to natural alternatives. The staff is on hand 24 hours a day to provide one-on-one, personalized attention. The rooms are decorated to be a home away from home and put you at ease. Also, our certified lactation educator is available to visit with each new mother while she is an inpatient. Our physicians who help deliver babies are highly trained and partner with our nursing staff to deliver really personalized care. Meeker Memorial also now has a dedicated OB/ GYN as an active medical staff member. Surgery Services From general to specialized surgery, our 24-hour surgical team of board-certified general surgeons, specialists and surgery staff will take excellent care of you. The size and skill set of our staff ensures that the care you need is always available close to home. Patient Registration and Billing At MMH, the care experience begins the moment you enter the lobby. The admitting assistants will make the process smooth and efficient for you. We begin the documentation

process here to provide you with accurate billing services. MMH also has a Charity Care Program for area residents who are in need, helping them reduce the burden of medical expenses. Laboratory Services The laboratory is an integral part of your total healthcare. Using advanced instrumentation and quality-proven methods, our laboratory staff provides accurate and timely testing results for you and your physician. MMH offers a full spectrum of laboratory services, including microbiology and transfusion services. Radiology/Diagnostic Imaging Services Diagnostic imaging is an increasingly important tool in your medical treatment. Our highly trained radiologic technologists will assist you with CT scans, MRI, X-rays, ultrasounds, bone densitometry tests, digital mammography and onsite nuclear medicine PET/CT Imaging and Electrocardiography are mobile services that come to MMH regularly. We are backed by leading radiology specialists and sub-specialists through Suburban Radiologic Consultants, LTD, to offer high quality diagnostic consultation. Pharmacy Services Our registered pharmacists and pharmacy technicians provide medication management services for our patients and healthcare providers from 6:30 a.m. to 5:30 p.m. weekdays and 7:00 a.m. to 3:30 p.m. on weekends and holidays. Pharmacists work within a multidisciplinary healthcare team to monitor safety and efficacy of your medication treatment. The radiology department is committed to providing patients with the most up-to-date imaging capabilities available. 13

Rehabilitation Services Rehabilitation needs are handled through our Rehabilitation Services Department. Physical therapy, occupational therapy and speech pathology help you work toward returning home as soon and as safely as possible. MMH s team of skilled therapists also help victims of back injury, stroke, heart attack, fractures and other ailments return to independence. For inpatients, physical therapy is available five days a week, from 7 a.m. to 5:30 p.m. and Sat., from 9 a.m. to 12 p.m. Occupational therapy and speech pathology are available weekdays from 8 a.m. to 5 p.m. and are scheduled as needed through a physician s request. Regardless of the rehabilitative service provided, our goal is always to return you to health as quickly and as safely as possible. We will be with you to achieve every milestone along the way. And because we are available in your local hospital, you can easily reach us anytime with questions or concerns. Dining & Nutrition Services MMH is pleased to offer you room-service dining for personally selected and individually prepared meals. With few exceptions, you have the freedom to order anytime between the hours of 7:00 a.m. and 6:00 p.m. from our room-service menu. A nurse or aide can assist you with menu choices to make sure that the diet ordered by your physician or dietitian, along with any diet restrictions, are followed. Your family and visitors are welcome to come to the Prairie Winds Café (cafeteria) anytime between the hours of 8:00 a.m. and 6:00 p.m. The daily lunch special is served 11:45 a.m. to 1:00 p.m. Daily specials are posted in the elevators. We also feature a full salad bar from 11:45 a.m. to 1:00 p.m. Monday through Friday. Cold choices such as sandwiches, desserts and cookies are available throughout the day. Coffee is available 24 hours a day. During your stay, the dietitian may meet with you regarding your nutrition requirements and concerns. You may request a visit from the dietitian at any time while you are here. Outpatient nutrition counseling is also available with a referral from your physician. 14

Expert caregivers look after patients in state-of-the-art private patient rooms that allow you to relax in a quiet atmosphere during your stay. 16

Pain Management We believe everyone has the right to have their pain assessed and treated, and we make managing pain an essential part of patient care. For this reason, our doctors, nurses and therapists are trained to work with you to develop strategies to prevent and relieve pain. Whether chronic or acute or related to surgery or labor and delivery, we employ a wide range of techniques to manage pain. Combining medical solutions with non-medical approaches like massage, heat/cold therapy, music and relaxation exercises, our goal is to minimize your pain to help you heal faster and return to the activities you enjoy. Emergency Room Services Ever vigilant. Always prepared. Emergencies don t adhere to a nine-to-five schedule. Our emergency department provides 24-hour adult and pediatric emergency and trauma care. Telemedicine services are now available 24/7 for stroke, with a neurologist dedicated to provide this added service. It is staffed by a skilled medical and nursing staff proficient in Emergency and Advanced Cardiac Life Support. Our facility is fitted with the latest emergency and trauma care technology and is capable of accommodating ambulance and helicopter transportation. Be sure, when the unexpected occurs, the MMH emergency department is ready. 17

Center for Senior Behavioral Health When the body is willing but the mind unable. For people over 55 years old and facing acute symptoms related to mental or behavioral health, there is help. Our Center for Senior Behavioral Health (CSBH) offers psychiatric hospitalization services. Each patient receives comprehensive psychiatric and medical care services during their hospitalization. Our mission is to provide individualized, inpatient care aimed at improving daily function and quality of life for the older adult. We offer a multidisciplinary team approach that combines the expertise of a psychiatrist, medical physician, psychiatric nursing staff, social worker and activity coordinator. Together this team tailors individualized plans of care that include group therapy, scheduled activities, medication management and individualized interventions. Should a patient have other medical issues while in the care of the Center for Senior Behavioral Health, they will be treated by the Center s medical director or their local personal care provider. Families are a central part of our patients care. Regular hours of visitation are available each day for loved ones. Follow-up recommendation and discharge planning will help ensure a smooth transition to enhance the patient s quality of life when they leave the hospital. Referrals to the Center can be made by anyone. Our staff will help the referring agency or person get the needed information and/or treatment quickly and professionally. Outpatient Services Why leave home when the care s right here? Even serious care doesn t always require a long hospital stay. In fact, many ailments can be addressed without an overnight visit. That means you get the help you need without being away from the comfort of your own home. Even if you didn t receive your primary care or surgery from MMH, you can still schedule your follow-up treatments with us where it s close, convenient and, most important, professional. Cardiac Rehabilitation Phase II and III Cardiac Rehabilitation begins as you recover from a heart attack or heart surgery. At this point (we call it Phase II), a cardiac care registered nurse will monitor your exercise and provide education to you three times per week for six to 12 weeks close to home. Phase III is a continuation of this rehabilitation program and focuses on making positive lifestyle changes. EKG Interpretation/Stress Tests for Your Heart Our treadmill stress tests and diagnostic monitoring techniques assist physicians in the early diagnosis and treatment of heart disease. Specialized heart testing helps your physician better detect heart disease. You can have a stress echocardiogram or a cardiolite stress test that combine diagnostic ultrasound or nuclear scans of your heart with a monitored treadmill test. The adenosine cardiolite nuclear scan is another specialized test that does not require you to walk on a treadmill and gives your doctor a closer, more exact look at your heart, all done at MMH on a scheduled basis. During these procedures, you will be monitored by a physician and a cardiac care registered nurse. 19

Cancer Care You may not be aware of it, but chemotherapy treatment is right here. Our oncology team of specialits provide consultation at MMH in the Specialty Clinics. In a team approach with the laboratory and diagnostic services, working closely with your doctor, the oncologist can supervise chemotherapy, immunotherapy and targeted cancer therapy treatments. This cutting-edge approach accompanied by personalized care from our trained cancer care staff make the patient experience exceptional. Laboratory Services The laboratory is your link between technology and good health. Laboratory services are available for specialty services and presurgical patient needs. Our staff works closely with the chemotherapy/oncology patients. MMH provides laboratory services for local nursing homes and clinics, provides occupational health drug and alcohol collections for area businesses, and participates in community health events. In addition to our standard laboratory services, MMH is offering an innovative service called Direct Access Diagnostic Testing. With this service, patients will be able to proactively choose their own laboratory tests from a select menu and monitor their own health at a reduced cost. People may come to the hospital Monday through Friday from 7:30 a.m. to 5:30 p.m. for Direct Access testing; appointments are not required. Radiology/Diagnostic Imaging Services Diagnostic imaging is an increasingly important tool in your medical treatment. MMH is an accredited digital mammography site offering preventative outpatient screening and diagnostic breast imaging ordered by your physician. Our highly trained radiologic technologists will assist you with CT scans, MRIs, X-rays, ultrasounds, bone densitometry tests, digital mammograms and nuclear medicine. MMH offers echocardiography and PET/CT Imaging which are provided by mobile service companies on a regular basis. Nuclear Medicine Imaging is also provided in-house two days a week. 20 Direct Access Diagnostic Testing services allow patients to have Carotid Artery and Abdominal Aortic screening tests completed without a physician referral via ultrasound imaging. These tests are available by appointment Monday through Friday from 9 a.m. to 3:30 p.m. Call (320) 693-4519 to make an appointment.

Outpatient Rehabilitation Services After an injury or a surgery, rehabilitation can be a long road. The professionals at MMH will reach out with care and help you on your journey. Rehabilitation services at MMH offers an outpatient program or home service if the need arises. Skilled therapists, therapy assistants and technicians will guide you through exercise or special treatments. Therapies can bring specialty treatment to the community by offering services in concussion management, women s health, lymphedema, pressure mapping, pediatric care, sports rehabilitation and hand therapy. Whether you are recovering from a sports injury, a fracture, joint replacement surgery, speech and swallowing difficulties, a back injury or a stroke, our rehabilitation department will be with you every step of the way. Physical therapy, occupational therapy, and speech pathology services can be scheduled for you as an outpatient or at home. Our goal is to return you to health as quickly and safely as possible, and we will work directly with your physician on your road to recovery. In our occupational therapy department we offer special services related to lymphedema treatment provided by our certified lymphedema specialist, and hand therapy provided by our certified hand therapy specialist. Same-Day Surgery Our program offers specialized surgery in the areas of ear, nose and throat surgery; podiatry; orthopedics; eye surgery; and urology. Our general surgeon, select family practice physicians and our orthopedic surgeons will also perform surgery on an outpatient basis. Nutrition Education Services Learning to manage your diabetes or to make positive lifestyle changes in your diet can be difficult. Community Health Education Community education efforts are designed to meet the health needs of the community. Our programs are often collaborative efforts with other healthcare providers, community organizations or volunteer organizations.
