PENNSYLVANIA 4-H HORSE PROGRAM OUTSTANDING MEMBER AWARD 2013 The winner of this award will be announced during Recognition Ceremonies at the State 4-H Horse Show on Saturday, October 26, 2013. Finalists will also be recognized. Applications due by August 27, 2013. Application Criteria Applicants for the Outstanding Member Award must be at least 15 years old as of January 1 of the current year and must be in at least their third year of membership in the 4-H horse program. Members currently enrolled in any 4-H horse project are eligible. Ownership of a horse is not mandatory for completing an application. This award is to recognize an outstanding 4-H member active in the 4-H horse/horse science program. Emphasis is on the member and his/her participation in the horse program, plus his/her leadership and impact on club, county, and community in general. Finalists will be recognized during the State 4-H Horse Show. The member with the highest-scoring entry will receive a tack trunk (or similar item). Application Procedure Each county may nominate one outstanding 4-H horse program member for this award. The selection procedure should be determined by the individual county. Selection of the county nominee should be made by an appropriate process determined by the extension educator and local horse program leaders. Former finalists may apply if selected by their county. Former saddle/tack truck recipients may not apply. Checklist for completed application packet (forward all items to the state level for judging): 1. Completed Application Form. Use our form or format. Do not include news clippings, photographs, or more than two letters of reference. 2. Two completed recommendation forms, at least one which must be from a non-4-h related person (i.e., a teacher, employer, counselor, coach). Note: Do not ask a member of your immediate family to complete a recommendation form. Application Deadlines A completed application for ONE nominee per county should be sent to: Donna Grey, Luzerne County Extension, 16 Luzerne Ave., Suite 200, West Pittston, PA 18643. Send the original and two copies of the application and letters (one for each judge). Do not send materials in a notebook or plastic sleeves. Applications must be postmarked by August 23, 2013 and should be received in the Luzerne County office no later than August 27. There are absolutely no exceptions to this deadline! No faxes will be accepted! Send the entire packet as a unit; there is no guarantee that additional "pieces" sent under separate cover or via e-mail will be included with the final application at the time of judging. Recommendations should include one from the applicant's 4-H club leader and another from a community-based individual (such as a teacher or other adult in the community who can speak for the community involvement). Extension educators may also include comments on a separate sheet attached to the application. Please do not have a member of your immediate family give you a recommendation. If your parent is your only club leader, have an extension educator or a leader from another club complete a recommendation form. Remember to send three total copies of the application/letters (and double check that both sides of pages are copied where applicable). 3

Tips: Be concise, truthful and highlight key accomplishments. Be sure your application is neat and easy to read. Don't assume the selection committee will always know what a local event is by just listing a name. You may wish to describe (briefly) that event. Use our form or format. Do not include news clippings, photographs, or more than two references! I. Project Summary (5 points) List the 4-H projects that you have completed in each of the previous two years. Be specific as to the project level or unit. List only 4-H projects for which you have kept project books and record books. Note: Capital Days, Ambassador Program, etc. are state activities listed elsewhere in this application. They are not projects. II. 4-H Story (30 points) Attach a 1-3 page description of your accomplishments and experiences in the 4-H horse program. Focus on your current year's accomplishments and those of the two previous years. Include the goals you have achieved, knowledge and skills you have acquired, and/or special 4-H events, activities, and programs that were important to you. You need not list your projects again. Type or word-process your response. Stories should be double-spaced using a highly legible font (Times, 12 point is good). The emphasis should be on you, not just on your horses or show ring accomplishments.

III. Communications (20 points) Describe your two most important 4-H related communications experiences during the current year and/or two previous 4-H years (such as demonstrations/presentations, public speaking, marketing 4-H, etc.). Limit your response to the space provided. IV. Leadership (20 points) Describe your two most important 4-H related leadership experiences during the current year and/or two previous 4-H years (such as club or county offices, county or regional committees, teen council, horse camp counselor, Team Horse Power, show chairperson, etc.). Limit your response to the space provided.

V. Community Service (20 points) Describe your two most important community service experiences from the current or two previous years. Limit your response to the space provided.
