The following definitions shall apply to terms used in the Medical Staff Bylaws and Manuals: 1. Active Staff or Active Medical Staff or "Active Staff Member" means those Members who are appointed to the Active Staff of the Hospital in accordance with the Bylaws. 2. Advanced Dependent Practitioner means an allied health practitioner who provides a medical level of care or performs surgical tasks (i.e., advanced practice registered nurse, physician assistant), consistent with the clinical privileges granted, but is permitted by State law and the Hospital to exercise those clinical privileges only under the direction of, or in collaboration with, a supervising Practitioner pursuant to a written supervision agreement. 3. Affiliate Staff or Affiliate Medical Staff or "Affiliate Staff Member" means those Members who are appointed to the Affiliate Staff of the Hospital in accordance with the Bylaws. 4. Allied Health Practitioner(s) means those allied health professionals who are approved by the Board to provide patient care services under the direction and/or supervision of members of the Medical Staff. Allied health professionals include, but are not necessarily limited to, physicians' assistants, nurse practitioners, and certified registered nurse anesthetists. Allied health professionals are not members of the Medical Staff and do not have the rights of Medical Staff members. 5. Applicant means a Practitioner who has applied for initial appointment or a Staff Member who has applied for reappointment. 6. Associate Staff or Associate Medical Staff or "Associate Staff Member" means those Practitioners who are appointed to the Associated Staff in accordance with the Medical Staff Bylaws and Manuals. 7. "At-Large MEC Members" or "At-Large Members" means the non-hospital based, Active Staff Members of the MEC, as discussed in the Medical Staff Bylaws and Manuals. 8. Board of Directors or Board means the Board of Directors of Doctors Hospital. 9. Bylaws or Medical Staff Bylaws means the Medical Staff Bylaws of the Medical Staff of Doctors Hospital, but does not mean the associated manuals, policies, procedures, rules or regulations. 10. Chief Executive Officer or CEO means the Chief Executive Officer of Doctors Hospital and his or her designees. 11. Clinical Privileges or Privileges means the authorization granted by the Board to an Applicant, Staff Member or Allied Health Professional to render patient care services in the Hospital within defined limits.

12. "Clinical Service" means any group of Practitioners who organize themselves in accordance with Article III, Section 1.2 of the Medical Staff Bylaws and Manuals. 13. Clinical Service Chief means an Active Staff Member who is duly appointed or elected in accordance with these Bylaws to serve as head or director of a Clinical Service. 14. "Credentials Committee" means the committee as described in Section I of the Credentials Procedure Manual. 15. Dentist means a doctor of dental surgery or doctor of dental medicine licensed under Chapter 466 of the Florida Statutes, as amended. 16. Department means the Hospital s departments as specified in Section 1.1 of the Organization and Functions Manual. 17. Department Chair means an Active Staff Member who is duly appointed or elected in accordance with these Bylaws to serve as head or director of a Department. 18. Dependent Practitioner means an allied health practitioner who is permitted by State law or the Hospital to function only under the direction or supervision of a supervising Practitioner pursuant to a written supervision agreement and consistent with the scope of practice granted. 19. "Emeritus Staff" or "Emeritus Medical Staff" means those Staff Members appointed to the Emeritus Staff in accordance with these Bylaws. 20. Good standing means a Staff Member who is not under suspension or restriction regarding Medical Staff appointment, admitting privileges or Clinical Privileges at the Hospital. 21. Hospital means Doctors Hospital. 22. "Hospital Based MEC Member" means those members of the Active Staff described in the Bylaws. 23. Hospital Based Medical Director means a member of the Medical Staff who is duly appointed or elected in accordance with these Bylaws to serve as head or director of a hospital based department. 24. Hospital Governing Documents means the articles of incorporation, bylaws, policies and procedures and rules and regulations of the Hospital and the Bylaws, Manuals, Policies and procedures, Rules and Regulations of the Medical Staff. 25. Immediate Past President means the Immediate Past President of the Medical Staff. 26. Initial Appointment means the first time the Practitioner becomes a Member and maintains privileges at the Hospital. Any subsequent application, regardless of time-frame or reason for reapplying, shall be treated as a Reappointment.

27. TJC means The Joint Commission (formerly known as the Joint Commission on Accreditation of Healthcare Organizations or JCAHO.) 28. Joint Conference Committee means the Joint Conference Committee of Doctors Hospital as set forth in the Articles of Incorporation and Bylaws of the Hospital. 29. Licensed Independent Practitioner means an allied health practitioner who is permitted by State law and by the Hospital to provide patient care services without direction or supervision, within the scope of his or her license and consistent with the clinical privileges granted. 30. Manuals means the Investigation, Corrective Action, Hearing and Appeal Manual; Credentials Procedure Manual; Organization and Functions Manual; Definition Manual; and Rules and Regulations adopted pursuant to the Medical Staff Bylaws. 31. Medical Executive Committee or MEC means the Executive Committee of the Medical Staff. 32. Medical Staff or Staff means all Practitioners who are granted Clinical Privileges to treat patients at the Hospital. 33. Medical Staff Office means the Medical Staff Office of Doctors Hospital. 34. Medical Staff Year means a calendar year. 35. Medical Staff Member or Staff Member or Member means any Practitioner who has been granted Clinical Privileges and Medical Staff appointment by the Board. 36. "Nominating Committee" means the Nominating Committee as set forth in the Medical Staff Bylaws. 37. Notification means notice in writing requiring evidence of receipt including certified mail, return receipt requested, express mail, or hand-delivery. 38. "Officers" or "Officers of the Medical Staff" means the President, Vice President, Secretary and Immediate Past President of the Medical Staff. 39. Patient Contacts means the contacts required of Members in connection with Reappointment. The Rules & Regulations set forth how Patient Contacts are earned. 40. Physician(s) means doctors of medicine licensed under Chapter 458 of the Florida Statutes, as amended or doctors of osteopathy licensed under Chapter 459 of the Florida Statutes, as amended. 41. Podiatrist means a podiatrist licensed under Chapter 461 of the Florida Statutes, as amended. 42. Policy or Policies means the policies of the Medical Staff adopted pursuant to the Medical Staff Bylaws or associated Manuals. 43. Practitioner means a Physician, Podiatrist, Psychologist or Dentist.

44. President or "Medical Staff President" means the President of the Medical Staff. 45. Professional Review Action means an action or recommendation of a Professional Review Body in the conduct of Professional Review Activity which is based on the competence or professional conduct of a Staff Member and which adversely affects or may affect the Clinical Privileges or appointment of the Staff Member. 46. Professional Review Activity means peer review activity of the Hospital regarding an Applicant or Staff Member: (i) to determine whether the Applicant or Staff Member may hold appointment and Clinical Privileges; (ii) to determine the scope or conditions of those Clinical Privileges and appointment; and (iii) to change or modify such Clinical Privileges and/or appointment when deemed appropriate. 47. Professional Review Body means the Board of the Hospital or any Board committee which conducts Professional Review Activity and includes any committee of the Medical Staff when assisting the Board in a Professional Review Activity. 48. Psychologist means a psychologist licensed under Chapter 490 of the Florida Statutes, as amended. 49. Reappointment means the application process for Practitioners who have previously been granted Membership and clinical privileges through Initial Appointment. 50. Rules and Regulations means the rules and regulations of the Medical Staff. 51. Secretary means the secretary of the Medical Staff. 52. State means the state of Florida. 53. Vice President means the vice president of the Medical Staff.
