A. General Information Welcome to UWSELA s 2016-2017 Prosperity Program Grant online grant submission manual. The UWSELA online portal, Andar e-community, is now open to accept proposal submission for the 2016-2017 Prosperity Program Grant at www.uwgnoa.org. Our ultimate goal is to partner with agencies that align with our new Prosperity Blueprint and are able to impact our vision of equitable communities where all individuals are healthy, educated and economically stable. Programs must focus on one or more of the four high-level outcomes: Stability Today; Prosperity Tomorrow; Personal Wellness; and Vibrant Communities. The Blueprint for Prosperity framework, grant guidelines, and grant outcomes menu can be found on the Attachments tab on www.uwgnoa.org. Questions? Login How to access United Way s online submission portal: Access to the Andar E-community site can be obtained from any computer with dial up or high speed internet capability. - Enter the site: http://www.uwgnoa.org - Enter your User ID and Password. You MUST contact Tap Bui at TapB@unitedwaysela.org to receive a user ID and password if you do not already have one. ** Your User ID WILL ALWAYS be the first initial of your first name and your last name. ** Image 1: Log-in Page 4 P a g e
Forgot Password? Should you forget your password: - Click on the Forgot My Password button on the Login page. - Enter either your User ID or email address when prompted. - Click e-mail My Password to receive your password. C. Welcome Page Once logged in, you will be brought to the Welcome Page. On the left side of this page, you will find 8 menu items from which you may choose: - Welcome Page: Answers the question, Who do I contact should I have a question? - My Profile: Allows you to update your information, as CB Administrator, including your password. - CB Packages: Contains the actual application/report components. - Attachments: Where to go to download necessary documents. - My Reports: Location to retrieve reports that have been run. - Signoff: Once clicked, you will be signed off the system. D. My Profile Select the My Profile menu item to see information about you, as a CB Administrator, for your Agency. - It is at this location that you can also CHANGE YOUR PASSWORD by clicking on the update icon next to your current password. E. 2016-2017 Prosperity Program Grant RFP To access the 2016-2017 Prosperity Program Grant RFP Application, please see CB Packages. - Double-click 2016-2017 Prosperity Program Grant RFP Application to begin your proposal submission. 5. PROPOSED PROGRAM OUTCOMES - This section will allow you to select the program outcomes you are proposing to accomplish in this RFP. Please refer to the Program Grant Outcomes Menu document in Attachments to assist you in this process. - Select Add to start the outcomes selection process. Select from the drop-down list of outcomes under Community Outcome. - Once selected, COPY and PASTE the chosen Community Outcome into the Program Outcome. - Click on Add to enter selected outcome. 6. PROPOSED PROGRAM INDICATORS - This section will allow you to select the program indicators you are proposing to accomplish in this RFP. Please refer to the Program Grant Outcomes Menu document in Attachments to assist you in this process. - Select Add to start the indicators selection process. Select from the drop-down list of outcomes you previously selected to link indicators to. - Then select from the drop-down list of indicators in Community Indicator that are linked to the selected outcome. - Once selected, COPY and PASTE the chosen Community Indicator into the Program Indicator. - Enter your target data for your program indicators in the Measurements section Base: this is the number of X your indicator is hoping to achieve (e.g. number of participating households or individuals) ; PLEASE NOTE: this number remains CONSTANT throughout the program Percent: leave alone Achieved: leave alone Data as of: leave alone Comment: enter any comments Instrument: please describe the data collection methods and tools the program will utilize to measure proposed outcomes and indicators Notes Target: enter any additional notes Click Add to enter selected indicator 14 P a g e

7. PROPOSED PROGRAM ACTIVITIES - This section will allow you to select the program activities you are proposing to accomplish in this RFP. Please refer to the Program Grant Outcomes Menu document in Attachments to assist you in this process. - Select Add to start the activities selection process. Image 18: Proposed Activities Form - You will be brought to the activities submission page. - Enter the proposed activity in the Description text box. - You may enter a proposed activity from the pre-defined list in the Program Grant Outcomes Menu document or type in your own. - Select Add to enter the proposed activity. - Under Output Type: Target: Click on Add to enter outputs for the proposed activity Enter the type of output and the target units of service Repeat process until all output types have been entered Click on Update Image 19: Add Activities and Outputs 17 P a g e

F. Download Submitted RFP: At any point in time, you have the option to print the RFP package components or download the information and retrieve at a later date. - To download in PDF, click the printer icon to automatically pull that report up in PDF format. - To download in Excel, click the excel icon to automatically pull that report up in an Excel format.
