Summary of Role The role of Staff Specialist Palliative Care works within the Mission and Mission in Action of HammondCare to ensure that palliative care medical services operate in alignment with organisational values and service requirements to ensure that the medical and holistic care provided to patients is of a high quality. The role provides medial governance and leadership to HammondCare s palliative care services which includes oversight of patient care, clinical assessment and patient review. The essence of this role: Provide high quality specialist Palliative Care medical care and services for inpatients, outpatients and community patients associated with HammondCare To work with other senior medical staff and within multidisciplinary/ healthcare teams in a cooperative and collegiate way, demonstrating the Mission, Motivation and Mission in Action of HammondCare Support the development of contemporary and evidence-based Palliative Care specialist services To proactively teach medical students and trainee doctors in areas relevant to palliative care medical services Participation in Palliative Care research opportunities. Key Responsibilities 1.0 Clinical and related activities To provide medical care, in co-operation with other members of the clinical team, and ensure that the clinical care of patients admitted under, or allocated to, the Specialist s care accords with Service Mission in Action Providing Leadership with others Delivers results Models and embodies the Mission, Motivation and Mission-in-Action of HammondCare Sets high standards of performance for own and other s work, seeking
policy in force from time to time and within the clinical privileges granted; To ensure best-practice clinical care is provided to patients admitted to the Palliative Care Service, in both hospitaladmitted and ambulatory care environments, specifically in provision of regular consultant services to external units as appropriate; To support the development of contemporary and evidence-based Palliative Care specialist services that conform with and meet the KPIs and terms and conditions (District) of HammondCare s Service Level Agreements with NSLHD. To attend meetings relevant to Palliative Care services, as requested for the NSLHD services network and provide liaison Consultant Palliative Care services in NSLHD facilities at appropriate hospitals; To mentor the professional development of junior colleagues in Palliative Care Medicine To participate in the teaching of undergraduate medical students and support the training of postgraduate staff in areas relevant to Palliative Care. To be included in the relevant on-call NSLHD Palliative Care Service rosters; To support planning, priority setting and implementation of service delivery. To orientate and supervise junior medical staff engaged in Palliative Care Services; To serve on relevant Hospital, Service or District committees; To maintain the required level of personal professional development; Adapting Approaches constant improvement of quality care Provides and responds to feedback and has positive working relationships Empowers others to develop their skills. Participates in College and District committees Builds collaborative relationships within HammondCare Health and Hospitals (HCHH) and NSLHD Follows through on tasks to completion Manages competing demands and implements plans to ensure work is completed Enhances the services reputation with key service providers and local health district palliative care services Introduces evidence based treatment regimens and enhances quality of life for palliative care patients Participates in discussions at each clinical services meeting and develops an action plan with team members to ensure KPI S are met. 2.0 Clinical Privileges Consistent with the usual practice within the defined scope of Palliative Care in a Public Hospital in NSW. 3.0 Admission and Visiting Rights The Specialist may admit under his/her care, or have allocated for such care, patients who have been accepted within the admission policy of the Service and within the clinical privileges of the Specialist. with Others Demonstrates Excellence in Work with others Demonstrates Excellence in Work Provides quality specialist medical practice with positive consumer feedback, from patients and families/carers Minimal errors or complaints Patients admitted to the service are within described scope of practice of case types for admission Regular home visits, clinics and on-call duties are undertaken 4.0 Medical Staff Council The Specialist is a member of the Medical Staff Council of HCHH, and HCHH Clinical Council. 5.0 Patient Records The Specialist will ensure that records for his/her patients are maintained, in terms of quality, time-lines and content, consistent with all HammondCare/Service policies and NSW Ministry of Health Policy Directives, especially in relation to clinical record keeping and specialist outcome measures. with others Delivers results Actively supports Medical Staff Council to progress the development of quality medical care in HCHH. Works effectively with other members of the senior medical staff Timely & accurate medical record entries are demonstrated upon medical record audit Supervises JMO staff in reviewing patients and completing their records; Records of patients under his/her care are accurate Works with team members and promotes accurate documentation in medical record Ensures 100% legal documentation is completed within prescribed time frame.
Develops with team members the inclusion of appropriate Clinical Indicators in medical record for inpatient services and community team Reviews documentation audits, develops action plan to address deficits, implements learning initiatives for team members 6.0 Quality Management To participate in development and maintenance of quality plans for the development of palliative care services, with development, measurement and refinement of appropriate KPIs; To participate in departmental and interdepartmental Quality Management projects and reviews; To participate in development of policies and procedures as they relate to areas of services. 7.0 Professional Development To attend and promote professional seminars, conferences and educational programmes; To maintain satisfactory level of personal professional development to ensure continuing medical specialist registration. Demonstrates Excellence in Work Providing leadership Ensures clinical benchmarking data for their patients is collected on a regular basis; Clinical data is benchmarked and reviewed with respect to Palliative Care in similarsized services Assists review of QM data for the Palliative Care service, including data for falls, errors and readmissions, transfers to acute care that are not planned, etc and discusses QM results at Clinical Services meeting QM data for incidents is maintained at or below agreed threshold levels Annual certificate of CPD completion Maintenance of specialist AHPRA registration Manages the task ensuring continuing personal development program both within HammondCare and with the respective Learned College.
