ZILLA SWASTHYA SAMITI, ANGUL WALK IN INTERVIEW No:4536 Dtd. 06/10/2016 A walk-in-interview shall be conducted on Dtd.19/10/2016 at 10 A.M in the Office Chamber of the Chief District Medical Officer, Angul for a single contractual post of Office Assistant for District Program Management Unit DPMU, O/o Chief District Medical Officer under National Health Mission, Angul with a monthly consolidated remuneration of Rs. 11,600/-(Rupees Eleven Thousand and Six hundred only ). The interested candidates are here by informed to log on to the District website i.e www.angul.nic.in for application form, eligibility criteria and other details to attend the walk-in-interview for the said post. The undersigned is not responsible for any transport delay and reserves all right to accept or reject any/all applications without assigning any reason thereof. No T.A/D.A is permissible to the candidates for attending the above walk-in-interview on exact Date. The said walk-in-interview may be cancelled/postponed by publishing a notice even on the date of interview in case of any emergency or exigency. Typographical errors (if any) may be exempted.

ELIGIBILITY CRITERIA FOR THE POST OF OFFICE ASSISTANT FOR DPMU UNDER NHM, ANGUL Nature of Contract: This post is purely contractual / temporary in nature and also Co-terminus with the project period. The continuation of the job is subject to contract renewal of the candidate upon annual/quarterly performance appraisal / assessment. Eligibility Criteria: The candidate must be any graduate with computer qualification of 01 year Diploma in Computer Application / PGDCA or BCA, MCA or equivalent. Age Limit : The candidate must not be below 21 years of age or above 35 years of age as on 1 st Date of advertisement i.e Dtd. 01/10/2016. Marital Status: If married the candidate must not have more than one spouse living, provided that Govt. may, If satisfied that such marriage is permissible under the personal law applicable to such person or there are other specific grounds for doing so, exempt any person from the operation of this rule. Physical Fitness: The candidates must be in a good physical and mental health and free from any physical defects likely to make his/her incapable for discharging his/her duties in the service. A candidate who after such medical examination as the Government may prescribe is not found to satisfy the requirements shall not be appointed in the service. Selection Procedure : The candidates shall be selected on the basis of career weight age, Computer Practical Test and Interview as mentioned below : Matriculation : - 20 Marks 10+2 (Intermediate) 20 Marks Career Graduation : - 20 Marks Computer Practical Test 30 Marks Interview :- 10 Marks. Total : - 100 Marks. Candidates in the ratio of 03 times the vacancy shall be called for interview from the merit list. The assessment of the career marks shall be done upto 03 digits for example 40.123 and the Candidates securing the highest marks shall be provisionally shortlisted by compiling the total weight age of the all above three stages for the post and final selection shall be done on and after verification of the Original documents of the shortlisted candidate. If in case there shall be a tie of weight age / marks then the selection of candidate shall be done as per the decision of the selection committee in pursuance to the prescribed guideline towards selection of other posts having equal monthly remuneration as like the monthly remuneration of the Office Assistant. After final selection the candidate shall be issued with an engagement order/offer letter and his/her date of joining shall be calculated from the date of his/her contract agreement with the Chief District Medical Officer cum District Mission Director, Angul.

Completely filled up application form as available in the District website i.e www.angul.nic.in. 2. Two recent attested passport size color photographs and 01 to be affixed on the application form at the desired space earmarked in the application form. 3. Attested copies of mark sheet and passed certificate of Matriculation, 10+2 Level, Graduation (mandatory) and other higher qualification (if any) 4. Attested copies of Mark sheet and certificate pertaining to the 01 year completion course in PGDCA, DCA or BCA, MCA, Equivallent. 5. Attested copy of valid residence certificate (The residence certificate should not be older than 06 months from the date of advertisement.) 6. Attested copy of valid Employment Exchange registration certificate. 7. Attested copies of self identity proof documents like Voter ID Card, ADHAR Card, PAN Card, Driving License or Passport. 8. Attested copy of Post qualification experience certificate along with a performance certificate from your current employer. 9. No objection and a performance certificate from the concerned employer / appointing authority those who are working in the Health Department in any capacity either regular or contractual. NOC must be specific for the post S/He applying and must be issued subsequent to the issue of advertisement. 10. One self addressed envelope (size 24 x 10 ) with postage stamp of Rs 25/- must be affixed on the self addressed envelope. GENERAL INSTRUCTION 1. Undertaking or affidavit for non submission of any of the requisite documents as mentioned above and the prescribed application form as available in the District website i.e www.angul.nic.in is subject to rejection of the candidature of the candidate. 2. Incomplete application form / canvassing in the application form is subject to rejection of the candidature for the said post. 3. The merit list shall be remaining valid for 12 months from the date of approval for future reference. 4. Under/Over age and under qualification candidates as mentioned in the advertisement are subject to rejection. 5. Candidates who are disengaged earlier from the Odisha State Health & Family welfare Society, NHM Office, ZSS under administrative ground such as disobedience / poor performance / misconduct or misbehavior or any criminal activity etc. are not eligible to apply for the said post. 6. ORV Act/Rule in any form shall not be applicable for the above recruitment/selection process. 7. If any important documents, information of any candidate found to be suppressed or forged during the selection process or in future then the candidature of the said candidate shall be automatically rejected. 8. No personal communication shall be entertained regarding this selection till the completion of the entire recruitment process. 9. In case the marks obtained in CGPA, OGPA,DGPA,GPA,CPI Etc., a certificate for conversion as applicable to percentage of marks shall be submitted at the time of submission of application form. Certification towards AICTE, UGC recognition of institutions/universities shall also be submitted at the time of submission of application form, wherever applicable, without which the application shall not be considered & shall be rejected.
