Cnemaugh Meyersdale Medical Center Implementatin Plan In June 2013, Cnemaugh Health System (CHS) initiated a Cmmunity Health Needs Assessment (CHNA) n behalf f its three member hspitals: Cnemaugh Memrial Medical Center (MMC), Cnemaugh Miners Medical Center (MIMC), and Cnemaugh Meyersdale Medical Center (MYMC). This implementatin plan was develped t address the pririty health needs identified within the CHNA with respect t Cnemaugh Meyersdale Medical Center (MYMC). This plan describes the actins that the hspital will take t address the need, the anticipated impact f these actins, the prgrams and resurces cmmitted t address each need, and any planned cllabratin between the hspital and ther rganizatins. The implementatin plan will als describe why an identified health need will nt be addressed. Pririty Health Needs 1. Increase the percentage f 40+ females that btain an annual mammgram CHS is engaging in a multi-media campaign t increase awareness. Cnemaugh Physician Grup (CPG) will encurage annual mammgrams in the target ppulatin. Free mammgrams are nw ffered at 6 sites in Cambria and Smerset Cunty every mnth. Vlumes will be tracked in the nfcus strategic planning system. Cnemaugh Health System is develping strnger relatinships with natinal rganizatins i.e., Pennsylvania Breast Cancer Calitin, American Cancer Sciety and the Susan G. Kmen Fundatin. CHS is wrking with the Susan G. Kmen Fundatin t prvide vuchers fr very lw cst mammgrams The number f mammgrams perfrmed will be mnitred within nfcus. A 6% increase in primary care physician rders abve F-13 by the end F-16. Tumr staging will be trended. Over several years as the number f mammgrams increases, the stage at which breast tumrs are identified shuld begin t decrease. It is t early t predict a metric. 2. Increase the rati f primary care physicians t ppulatin in Smerset Cunty Existing Cnemaugh Physician Grup practices have capacity fr new patients Cnemaugh Meyersdale Medical Center Family Health Center The lw rati wuld suggest that patients may nt seek rutine medical care. Cmmunicatin abut the need t select and fllw up with a PCP will be cmmunicated. Cnemaugh Physician Grup is actively recruiting an additinal primary care physician in Smerset Cunty. Recruitment and number f unique patient is mnitred within the nfcus strategic planning system. An additinal Family Medicine physician will be recruited subject t the availability f Family Medicine physicians within the natinal pl as primary care physicians are in limited supply. The number f unique patients within the Cnemaugh Meyersdale Medical Center Family Health Center will increase by 40 by the end f F-16. This increase f apprximately 1% tgether with a 0.8% decrease in the ppulatin will result in a psitive increase in the number f patients being seen by primary care physicians. 4. Increase the rati f mental health prviders t ppulatin Cnemaugh Physician Grup is actively recruiting an additinal psychiatrist wh will treat patients in the MYMC primary service area. Recruitment is mnitred within the nfcus strategic planning system. The United Way f the Laurel Highlands (UWLH) funds a number f mental health prgrams which supplement mental health prviders. An additinal Psychiatrist will be recruited subject t the availability f psychiatrists within the natinal pl as Psychiatrists are in limited supply. CHS will cntinue t wrk clsely with the UWLH t increase the quality f cmmunity prgrams and number f patients seen. 5. Increase access t dental care fr individuals wh are under r uninsured UWLH is taking the lead n this issue. UWLH is wrking with Salvatin Army t pen a dental clinic in Fiscal 2014 that will accept Medicaid patients. CHS will prvide assistance t start-up per the nging relatinship with UWLH 41

Imprve cmmunity educatin f hw t live a healthy lifestyle Tbacc Use Use f tbacc prducts by emplyees during wrk shifts is nw prhibited at CHS. CHS will prvide cmmunity messaging and easy t access infrmatin n the CHS web site abut the dangers f tbacc use. CHS will prvide prgramming t reginal businesses that fcus n initiatives t prevent and treat tbacc use. CHS will ffer N Butts Abut It grup cessatin classes at n r limited cst t the cmmunity t increase awareness, registratin and cmpletin rates The number f participants in the N Butts Abut It prgram will be mnitred within the nfcus strategic planning system. Gal f 10 individuals t cmplete the smking cessatin prgram in F-14, 15 and 16. Healthy Lifestyle Educatin Cnemaugh Health System will supprt and implement initiatives t prmte psitive health behavirs, health awareness and educatin, t reduce besity and lack f sufficient physical activity. Cllabrate with lcal businesses and schl districts t prmte a healthier wrkplace thrugh the nline Cmmunity Wellness platfrm. Begin t implement Cmmunity Wellness the CHS nline health tracking and gamificatin platfrm that inspires and engages the entire cmmunity t prmte a healthier lifestyle. Increase cmmunity educatin abut Drug/Alchl dependency and treatment Drug and alchl use was identified in the CHNA as a significant need and is being addressed by the UWLH which nw funds Btvin Life Skills Training, an evidence-based preventin prgram, at all Smerset Cunty High Schls and 4 f 10 high schls in Cambria cunty 42

CHS will wrk with the UWLH t identify pprtunities t develp r enhance cmmunity based prgrams fr the adult cmmunity N specific utcme metric has been identified at this time due t the pervasiveness and insidiusness f Drug and Alchl abuse and the inability f mst cmmunities t make significant headway in dealing with this matter. 8. Increase cmmunity educatin abut access t care fr un/underinsured CHS and UWLH prvide the majrity f annual funding t the Jhnstwn Free Clinic. CHS has updated the Charity Care Plicy and expanded access by implementing a presumptive charity care prgram. Accunts qualifying fr charity care have dubled since this was put int place in July 2011; and there are fewer accunts sent t cllectin agencies. CHS hspitals help eligible patients register with Medicaid, CHIP and ther prgrams by utilizing bth internal and external resurces. CHS hspitals extend a discunt f 60% t individuals withut health care insurance. The Affrdable Care Act is expected t increase the number f individuals with insurance cverage. Health Exchanges t be intrduced in the fall f 2013 are t increase the number f individuals wh will have insurance cverage. The Cmmnwealth f Pennsylvania may r may nt increase Medicaid eligibility t allw mre individuals t apply fr Medicaid cverage. Cnemaugh Health System prvides infrmatin n its website t guide patients thrugh insurance ptins and issues. The Hspital Cuncil f Western Pennsylvania, f which CHS is a member, will be a CMS certified prvider t educate and assist individuals with health exchange enrllment. Anticipated Impact N specific metrics have been identified at this time due t the significant changes nw being intrduced as a result the Affrdable Care Act and the lack f a decisin by the Cmmnwealth f Pennsylvania t expand Medicaid cverage. MYMC will mnitr the specific actins identified abve and will react where pssible t imprve perfrmance given the significant changes expected in insurance access. 43
