Documenting Medicare Annual Wellness Visit Documenting Medicare Annual Wellness Visit for Senior Programs (AMV) in NextGen is available in one place. You can document all requirments in the Medicare Preventive Template. Select one of the two Visit Types: 1. Medicare Preventive- Will Generate G codes (G0402, G0438 or G0439) 2. Preventive Medicine- Will Generate a Well visit CPT Code (99387 or 99397) Reason for Visit/HPI: 1. Click on Additional/Manage 2. Documenting Medicare Annual Wellness Visit (Continued) To document the Medicare Preventative HPI, select the radio button to indicate the following options: Initial Medicare Visit (Welcome to Medicare (IPPE)) Annual Well Visit (First) Subsequent Annual Wellness Visit Expand the panels and document required information. To add additional Annual Wellness Codes: From the finalize tab, navigate to the evaluation and management coding panel a) Click on AWV active text link b) An additional AWV template pop-up will launch c) Click in the procedure field to enter additional codes e.g.,blood pressure, medication review, BMI,etc d) Select the CPT code e) Click in the diagnosis field and search for a diagnosis, or select one from the diagnosis billing history f) Click submit code 3
DID YOU KNOW??? The Medicare Access and CHIP Reauthorization Act (MACRA) of 2015, requires Centers for Medicare & Medicaid Services (CMS) to remove Social Security Numbers from all Medicare cards by April 2019. A new Medicare Beneficiary Identifier (MBI) will replace the SSN based Health Insurance Claim Number (HICN) on the new Medicare cards for Medicare transactions like: Billing Eligibility status Claim status. Beginning in April 2018, CMS will start mailing the new Medicare cards with the MBI to all people with Medicare. Distribution will be made in a randomized card mailing by geographic location. Clearly different than the HICN How will the MBI look? 11-characters in length Made up only of numbers and uppercase letters (no special characters) Transition Period There are plans for a transition period where you can use either the HICN or the MBI to exchange data. The transition period will begin no earlier than April 1, 2018 and run through December 31, 2019. After the transition period ends on January 1, 2020, you will need to use MBIs on your claims with a few exceptions. Medicare Plan Exceptions include: Appeals - You can use either the HICN or the MBI for claims appeals and related forms Adjustments - You can use the HICN indefinitely for some systems (Drug Data Processing, Risk Adjustment Processing, and Encounter Data) and for all records, not just adjustments. Fee-for-Service Claim Exceptions include: Appeals - You can use either the HICN or the MBI for claims appeals and related forms. Claim status query - You can use either the HICN or MBI to check the status of a claim (276 transactions): If the earliest date of service on the claim is before January 1, 2020. If you're checking the status of a claim with a date of service on or after January 1, 2020, you have to use the MBI.
