Community Grant Making Program Priority Areas for Funding Access to Care. Programs that increase access to primary care, specialty care and mental health care services. Overweight & Obesity. Programs that reduce and treat overweight and obesity. Also programs that focus on prevention through healthy eating and physical activity. Chronic Conditions. Submit Complicated Proposals Forget to submit required attachments Create unattainable objectives Use acronyms Include your accounting department as FA Forget to click submit on the application YOU WILL RECEIVE A CONFIRMATION OF SUBMISSION IF YOU DON T, THAT MEANS YOU HAVE NOT PROPERLY SUBMITTED CALL ME IF THIS HAPPENS IMMEDIATELY! 17

Sponsorship Program Application Requirements Completed (online) application & attestation sheet A copy of your organization s tax exempt status documentation A list of your organization s board members or governing body and their affiliations (ie Kristin Weber, Kaiser Permanente) If using a fiscal agent, a letter from fiscal agent on their letterhead and signed by their executive director or president, explaining the relationship between the two organizations.
