Manteno Parent Teacher Organization (P.T.O.) Scholarship Program Application 2017-2018 Academic Year INTRODUCTION The Manteno P.T.O. annually awards academic scholarships to graduating seniors of the Manteno C.U.S.D. No. 5. The scholarship program helps graduating high school seniors continue their education at a university, college, trade or vocational school. The scholarships are based on positive leadership and volunteering in school and community activities. The P.T.O. Board administers the application process and selects the scholarship recipients. The name of each scholarship applicant is concealed from the scholarship selection committee to ensure that the recipients are selected entirely on their merits. SCHOLARSHIP AMOUNT Each recipient of the Manteno P.T.O. scholarship will receive a $500 scholarship. scholarship recipients will be determined by the P.T.O. Board. The number of ELIGIBILITY Anyone who is a Manteno C.U.S.D. No. 5 Senior graduating during the academic year shown on this application is eligible to apply for the scholarship. Applications will be accepted during the last semester of school. If the student is an early graduate, applications will be accepted during the first semester. Applications will not be accepted after April 13 of the current academic year. SELECTION CRITERIA Scholarship recipients will be selected on the basis of the following: 1. Positive leadership exhibited in both academic and extracurricular school activities. 2. Positive leadership exhibited in community and/or religious activities. 3. Volunteering service in school, community, and/or religious activities. 4. Part-time/full-time employment A transcript for seven semesters is also required. APPLICATION AVAILABILITY Scholarship applications will be available in the high school guidance counselor s office between January 1 and April 13 of the current academic year, and also online at www.mantenopto.weebly.com. The student must print and complete the application and submit a signed copy. The format of the application may not be altered. SELECTION PROCEDURE The evaluation meeting will be held during the month of April or at the call of the P.T.O Board President. Announcement of the winners will be made by the students high school graduation day.
