Overview Structural heart intervention by trans-catheter technology has rapidly grown into a distinct subspecialty of cardiology over the last decade. This unprecedented ascent has been driven not only by advances in device technology, but largely due to advances in cardiac imaging, which now offers the spatial resolution and real-time anatomical detail to permit structural intervention without direct surgical visualization. Structural heart imaging, primarily employing CT and TEE, require additional time, training and knowledge, which are not conventionally taught in general cardiology fellowship and are often not part of routine clinical cardiovascular practice. It is not possible even for an experienced clinical imager to participate in structural heart procedures without additional training. This presents both the need and opportunity to share our experience with our colleagues, both to stream line patient diagnostic evaluation and facilitate appropriate referral when necessary. Target Audience Physicians, Cardiologists, imaging-lab staff, and cardiovascular disease fellows Learning Objectives After participating in this activity, the learner should be able to. Describe the advanced imaging techniques utilized to inform use of approved devices for structural heart intervention in the United States. Identify the pre-, intra-, and post- procedural imaging required for structural heart intervention. Discuss potential challenges to optimal imaging, as well as counter-measures for successful completion. Explain the diagnostic imaging, documentation and logistical steps necessary to support referral of a patient for structural heart care and avoid redundant testing.
