Staff Name (print): Program: Date of Quiz: Medication administration is a key role of support staff, and must be performed correctly and in accordance with CARF accreditation standards, PDCRS policy, procedures and correct practice. As part of your training, the is designed and given to determine your medication administration competency. It is based only on current medication policies, procedures and correct practice. Open-book is permitted. You must obtain 100% upon correction (upon correction means you review any incorrect answer(s) until correct) to achieve a pass. Medication Control and Monitoring 1 T F PDCRS will ensure that programs have appropriate controls to ensure safe administration of medication. 2 T F Administered medications do not need to comply with federal laws. 3 T F You only need verbal, informed consent of the individual, or his/her legal guardian for monitoring medication administration. As part of risk management, the following will be considered to be (but not limited to) medication errors: 28 T F Unauthorized drug use 29 T F Failure to document an unusual occurrence on the Medication Administration Log (MAR sheet) 30 T F Medication errors will be reported to the participant s parent/guardian. 31 T F Staff must report each occurrence of a medication error on an PDCRS Incident Report Form J:/CCSS/CCSSSupervisors/Staff/Training/CIRQuiz2012 Page 3 of 5

Protocols and Controls 32 T F CCSS programs will keep all prescribed and over-the-counter medications and medical supplies (except for first aid kits) in an unlocked medication cabinet. 33 T F A Medication Administration Log (MAR sheet) will be kept in a different place away from the medication. Labeling and Packaging of Medication 34 T F An up-to-date Medication Administration Log (MAR sheet) of all medications, including prescription and non-prescription medications, used by the participant will be kept. The records and labeling of all medications, where possible, will include: 35 T F the prescribing doctor name 36 T F the doctor s orders, when given by the pharmacy 37 T F a number or code identifying the written order, when available 38 T F the time to call home 39 T F the expiration date of the medication Disposal of Medications, Supplies and Waste 40 T F Empty blister packs and other containers will be returned by authorized personnel to the parent/guardian for disposal. 41 T F Until they are returned, empty blister packs and other containers will be stored in an unlocked cabinet. Written Orders 42 T F Program staff will accept verbal orders for monitoring the administration of medication. 43 T F Participants, families and other care providers will be required to have written orders accompany any medication to be monitored and administered.
