Forsyth Technical Community College 2100 Silas Creek Parkway Winston-Salem, NC 27103-5197 Interventional Cardiac and Vascular Technology - Cardiac Certificate Intended for applicants who are currently employed in Interventional Cardiovascular Technology Fall 2019 Deadline: June 27, 2019 ***Admissions Information*** Interventional Cardiac and Vascular Technology (ICVT) - Cardiac is a selective enrollment program beginning each fall. Resources such as faculty, as well as approval standards, limit the number of applicants accepted to 15 students for the fall. Interventional Cardiac and Vascular Technology (ICVT) Cardiac is an advanced two-semester, online certificate program and all applicants must meet minimum requirements to be considered for admission. Please note, however, that meeting minimum requirements does not guarantee admission. Applicants for this program must be graduates from an approved accredited program who have earned an Associate Degree or higher in Nursing, Surgical Technology, Paramedic, Respiratory Therapy, or Radiography; or graduates who have earned Bachelor of Science in Exercise Physiology. Applicants must have a current licensure/registry in good standing and must meet the minimum admission requirements (MAR). Acceptable accreditation agencies include: Commission on Collegiate Nursing Education (CCNE) or National League of Nursing Accrediting Commission (NLNAC) Commission on Accreditation of Allied Health Education Programs (CAAHEP) Accrediting Bureau of Health Education Schools (ABHES) American Nurses Association (ANA) or American Association of Colleges of Nursing (AACN) Committee on Accreditation of Educational Programs for the EMS Professions (CoAEMSP) National Board for Respiratory Care (NCRC) Joint Review committee on Education in Radiologic Technology (JRCERT) Other accreditation agencies will need to be individually evaluated by the ICVT program coordinator. When the applicant has completed all of the minimum requirements, they must contact the Program Coordinator, Marti Feathers-Magee, at 336-734-7189 or mmagee@forsythtech.edu, for an Interventional Cardiac and Vascular Technology (ICVT) Cardiac program review. This must be completed by the deadline date stated above. Qualified applicants will be ranked based on criteria listed in this packet. Applicants not admitted to the program must reapply if they wish to be considered for the following year s class. pg. 1

PHASE I: APPLICATION PROCESS The following steps must be completed by June 27, 2019 for the fall 2019 program. Please note you are not required to participate in an Allied Health Information Session. 1. Complete the Residency Determination Service (RDS) and a Forsyth Tech application. If currently enrolled in classes at Forsyth Tech, complete the update form in the Admissions Office. 2. Complete the Forsyth Tech application. 3. Have an official high school transcript or GED scores mailed to the Admissions Office. 4. Have official transcripts from ALL colleges and schools attended since high school sent to the Admissions Office. Official transcripts must have the school seal or original signature and be in a sealed unopened envelope or submitted electronically from the school. All transcripts are required regardless of applicability to the program of study. Electronic transcripts should be sent to Admissions@forsythtech.edu. We strongly recommend that official college transcripts be received and evaluated before a MAR is conducted. Evaluation of transcripts typically takes 5-7 business days to complete once transcripts have been received. Please do NOT bring in transcripts the day you come to complete your MAR review 5. Submit official copies of any Advanced Placement (AP) scores, if applicable. Official scores are required to assign points for ranking. Please note that this can take up to four to six weeks to be received due to the College Board request process.. PHASE II: MINIMUM ADMISSIONS REQUIREMENTS 1. Grade Point Average (GPA) Requirement Applicants must have a final, cumulative GPA of 2.5 or higher to qualify for acceptance into the Interventional Cardiac and Vascular Technology Cardiac program. If the final, cumulative GPA is less than 2.5, the applicant will be required to submit a letter of recommendation from their Current Manager to be considered for admission to the program. This recommendation letter can be included with the letter of employment confirmation, which must be submitted to the ICVT Program Coordinator. A final, cumulative GPA of lower than 2.0 will not be considered. Radiologic Technologists who have graduated within 5 years of acceptance into the ICVT program will be issued transfer credit for ICV 112. Licensed Paramedics and Respiratory Therapists who have graduated within 5 years of acceptance into the ICVT program will be issued transfer credit for ICV 111. Licensed Nurses who have graduated within 5 years of acceptance into the ICVT program will be issued transfer credit for ICV 216. *Additional proficiency exams for ICV core courses may be offered. pg. 2

2. Employment Requirement This program is intended for applicants who are already employed in the field of interventional cardiology and wish to obtain study guides/materials to pass their RCIS or CI registries. An applicant who is NOT currently employed in an interventional lab may enroll in the ICVT cardiovascular certificate program; however, they must secure their own clinical rotations. Forsyth Tech will not be responsible for obtaining a clinical affiliate agreement or assigning clinical rotations during this certificate program. Students who obtain personal permission to perform clinical rotations may be required to purchase private malpractice insurance by their clinical site. The agreement exists solely between the student and the clinical site. 3. Letter of Recommendation Requirement Applicants must present a letter of recommendation from a department manager verifying their employment (or personally secured rotation) in a cardiovascular interventional lab. The letter must include the number and types of exams performed per year in the department. The ANCC also offers Cardiac/Vascular Nurse Certification for registered nurses. PHASE III: RANKING Students with the highest cumulative GPA will be accepted. In the event of a tie, the completion date of Phase I will be the determining factor. An alternate list will also be developed. Alternates will only be accepted if a student originally accepted, declines their seat. TECHNICAL STANDARDS FOR PROGRAM COMPLETION A Degree in Interventional Cardiac and Vascular Technology (ICVT) signifies that the graduate is prepared for entry into the practice of interventional medicine. Therefore, the graduate must have both the knowledge and skills to function in a broad variety of situations and to render a wide spectrum of health related services. Candidates for the degree must have a variety of abilities and skills in the areas of: A. Observation B. Communications C. Motor and Function D. Intellect: Conceptual, Integrative, and Quantitative E. Behavior and Social Skills Technical compensation (i.e.: eyeglasses, hearing aides) can be made for some handicaps in certain areas but a candidate must be able to perform in a reasonably independent manner.z A. Observation: abilities required of a candidate include sight sufficient to: 1. Skillfully use precision instruments such as microscopes, oscilloscopes, gauges, control panels, and other electronic and digital equipment. 2. Observe and evaluate (i.e.: patient s body habitus, cassette sizes and radiographic density and contrasts). 3. Observe the results of technical factors (i.e.: film critique). 4. Observe the results of certain stimuli (i.e.: medication reaction). 5. Observe changes in equipment operation (i.e.: smell overheating). The senses of vision, smell and hearing are necessary for a candidate to effectively function in the Interventional Program. B. Communication Skills: required by candidate include the ability to speak, hear, & observe patients in order to: 1. Elicit information and cooperation (ie: questioning of patient, giving breathing instructions). 2. Describe changes in mood, activity, and posture. 3. Perceive nonverbal communication (ie: pain, lack of understanding). A candidate must be able to communicate effectively and sensitively with patients including not only speech and hearing but reading, writing, and electronic forms. A candidate must be able to communicate effectively and efficiently in oral and written form with members of the health care team. pg. 4

C. Motor and Function Abilities: required of a candidate include gross and fine muscular movements, equilibrium, strength, and functional use of the senses of touch and vision for the safe handling of patients, self and equipment in order to: 1. Elicit information from patients by palpation, auscultation, percussion, testing muscle strength and function, penetration of the skin and vessels, and other interventional maneuvers. 2. Safely perform therapeutic procedures and/or laboratory procedures. 3. Provide other patient services and patient associated services. 4. Safely lift, manipulate, and use equipment. A candidate must have these abilities to provide safe care for patients in various states of mobility and awareness. D. Intellectual: Conceptual, Integrated, and Quantitative Abilities: required for a candidate include measurement, calculation, reasoning, analysis, and synthesis to: 1. Solve problems (i.e.: mathematical computation; medication dosage calculation). 2. Comprehend three dimensional relationships (i.e.: anatomical relations). 3. Understand the spatial relationship of structures. A candidate must have these abilities to accurately perform angiographic procedures to produce quality images. E. Behavioral and Social Skills: required of a candidate: 1. Allow mature, sensitive and effective relationships with patients, clients, and fellow workers (interpersonal skills). 2. Tolerate physically taxing workloads. 3. Function effectively under stress. 4. Adapt to changing environments (i.e.: flexible schedules). 5. Display flexibility and ability to function during uncertainty inherent in health care (i.e.: emergency conditions). 6. Display: A. Compassion B. Empathy C. Integrity D. Concern for others E. Interest and motivation A candidate must have these abilities to provide empathetic support to patients where quality care is dependent on a team approach. All of the above abilities, skills, and knowledge in a candidate will be continually tested and evaluated during the educational process of this program. pg. 5
