Hope VI, Mixed Finance Modernization or Development, Demolition and/or Disposition, Conversion of Public Housing, Homeownership Programs, and Project-based Vouchers. Include statements related to these programs as applicable. (a). Hope VI or Mixed Finance Modernization or Development. The Albany Housing Authority received funding for a planning grant under the Choice Neighborhoods Initiative Program to improve properties of GA23000001 AMP 1, and the surrounding community. The AHA intends to apply for a Choice Neighborhoods Implementation Grant in 201 or 2014.. (b). Demolition and/or Disposition As a part of the revitalization strategy, our intention is to apply in 2013 or 2014 for the Choice Neighborhoods Initiative Implementation Grant. Part of the revitalization strategy is the demolition of two developments: McIntosh 7.0 Homes constructed in 1952, consisting of 125 public housing units previously referred to as GA 23-3, also located in GA023000001, AMP 1; and, Golden Age Apartments constructed in 1964, consisting of 66 public housing units and previously referred to as 23-7 located in GA023000003, AMP 3. The Albany Housing Authority has begun the process that s required to demolish these properties. This is a high density area that has been plagued with drugs, violence and poverty. If awarded, the Choice Neighborhood Implementation grant will also be used to establish new mixed income residential housing on these sites that will enhance and improve the neighborhood and improve the quality of life. This transformation will include potential redevelopment of existing blighted neighborhoods, as well.. Conversion of Public Housing. The Albany Housing Authority has no plans in this area. (d). Homeownership. The Albany Housing Authority has no plans in this area. (e). Project-based vouchers. In conjunction with the Choice Neighborhood Revitalization Plan and as part of the relocation requirements for current tenants, project-based vouchers will be issued. 8.0 Capital Improvements. Please complete Parts 8.1 through 8.3, as applicable. 8.1 8.2 8.3 Capital Fund Program Annual Statement/Performance and Evaluation Report. As part of the PHA 5-Year and Annual Plan, annually complete and submit the Capital Fund Program Annual Statement/Performance and Evaluation Report, form HUD-50075.1, for each current and open CFP grant and CFFP financing. Capital Fund Program Five-Year Action Plan. As part of the submission of the Annual Plan, PHAs must complete and submit the Capital Fund Program Five-Year Action Plan, form HUD-50075.2, and subsequent annual updates (on a rolling basis, e.g., drop current year, and add latest year for a five year period). Large capital items must be included in the Five-Year Action Plan. See Attached HUD-50075.2 Capital Fund Financing Program (CFFP). Check if the PHA proposes to use any portion of its Capital Fund Program (CFP)/Replacement Housing Factor (RHF) to repay debt incurred to finance capital improvements. Housing Needs. Based on information provided by the applicable Consolidated Plan, information provided by HUD, and other generally available data, make a reasonable effort to identify the housing needs of the low-income, very low-income, and extremely low-income families who reside in the jurisdiction served by the PHA, including elderly families, families with disabilities, and households of various races and ethnic groups, and other families who are on the public housing and Section 8 tenant-based assistance waiting lists. The identification of housing needs must address issues of affordability, supply, quality, accessibility, size of units, and location. 9.0 The Albany-Dougherty County 2005-2025 Consolidated plan indicated that 26% of families with housing needs are below 30% of the average median income for Dougherty county; 26% of families are more than 30% of median income but less than 50% of the average median income; 33% of families needing affordable housing are more than 50% of the average median income but less than 80% of median income with 15% of the elderly needing affordable housing. For all family types, affordability, and quality present the major problems to acquiring housing; size is second followed by supply, accessibility and location. The greatest obstacles indentified by families with disabilities are affordability, supply and accessibility. 9.1 Of the 2,278 applicants on the waiting list of the Albany Housing Authority, 78.43% are Extremely Low Income; 17.39% are Very Low Income and 4.18% are Low Income. These percentages are virtually unchanged from 2012. Strategy for Addressing Housing Needs. Provide a brief description of the PHA s strategy for addressing the housing needs of families in the jurisdiction and on the waiting list in the upcoming year. Note: Small, Section 8 only, and High Performing PHAs complete only for Annual Plan submission with the 5-Year Plan. The AHA will continue to provide quality housing to our residents; will continue to keep units leased; will continue to keep turnover rates low and will continue to keep our properties in good repair. The AHA is currently managing 230 units of city owned property with an offer to manage an additional 77 units for the City. The AHA is housing families in the jurisdiction as well as families on the AHA s waiting list. AHA also manages 76 elderly units.

Additional Information. Describe the following, as well as any additional information HUD has requested. (a) Progress in Meeting Mission and Goals. Provide a brief statement of the PHA s progress in meeting the mission and goals described in the 5- Year Plan. Goal: Expand the supply of assisted housing. Because HUD has not provided funding for new public housing units or for additional vouchers, we have not been able to utilize these traditional sources to expand the supply of assisted housing. We have worked with a local non profit entity to construct a total of 52 units of assisted housing for the elderly, which are currently fully leased, and we continue to manage 230 city owned properties, with an option to manage an additional 74 units for income eligible residents. We have also been diligent in reducing vacancies of our existing public housing and have consistently maintained a vacancy rate during 2012 well below 3%. Goal: Improve the quality of assisted housing. Through the use of Capital Fund Grants, we are continually improving the quality of our housing stock. Significant improvements include: the installation of air conditioning; the installation of energy efficient windows in several developments, installing new roofs, replacing water heaters and heat pumps, replacing interior doors, installing new toilets and site improvement to include professional landscaping, installation of sprinkler systems and security cameras. 10.0 Goal: Provide an improved living environment. With the demolition of two buildings in GA023000001 AMP1 McIntosh, density has been reduced, the appearance of the property has greatly improved and is aesthetically pleasing and security cameras have been installed to provide a safer environment. Only residents and their guests are permitted to enter the complex through controlled gates. Parking is available to residents within the complex. The buildings, constructed in 1942, have received a new facelift with new paint and the grounds have been professionally landscaped. In addition, another development in AMP 1, O.B. Hines has also undergone a transformation with new winding serpentine sidewalks, parking wells, exterior painting of all buildings, decorative fencing, new doors, professional landscaping and a paved alley. With the proposed demolition of two other developments located in this same area, McIntosh and Golden Age, this will also enhance the overall appearance of this neighborhood. Goal: Promote self-sufficiency and asset development of assisted households. Albany Housing Authority employs a Family Self Sufficiency Case Manager to provide case management services and to coordinate other services for participants in the Family Self-Sufficiency (FSS) program. We have partnered with the Council on Aging to bring a meals program to our elderly residents. Meals are served on the second level of our administrative offices and transportation is provided to our tenants to and from their residence. AHA engages the services of a consultant to provide life skills to our residents which will eventually lead to residents being employed. A sorority is making available financial workshops to our residents, Adult Literacy classes are being held at one of our sites and after the Albany Housing Authority was awarded the Choice Neighborhood Planning Grant, a Resident Planning Group was formed and residents serving on this committee have enhanced their skills in communication and activism. Goal: Ensure equal opportunity and affirmatively further fair housing. We continue to operate our housing program in a manner that promotes access to housing regardless of race, disability, color, religion, national origin, sex, or familial status. In an effort to maintain this goal, Agency staff is trained on fair housing practices, including Property Managers, Assistant Property Managers, Office Clerks and Maintenance staff. (b) Significant Amendment and Substantial Deviation/Modification. Provide the PHA s definition of significant amendment and substantial deviation/modification Definition for Substantial Deviation and Significant Amendment or Modification 1. A change to a development account number on the capital fund program in excess of the greater of 10% of the grant amount or $100,000. 2. A change in the selection preferences for admission.

11.0 Required Submission for HUD Field Office Review. In addition to the PHA Plan template (HUD-50075), PHAs must submit the following documents. Items (a) through (g) may be submitted with signature by mail or electronically with scanned signatures, but electronic submission is encouraged. Items (h) through (i) must be attached electronically with the PHA Plan. Note: Faxed copies of these documents will not be accepted by the Field Office. 6.0 PHA Plan Update DRAFT 2013 (a) Identify specifically which plan elements have been revised since the PHA's prior plan submission There have been no new revision to any plan elements since last submission. Eligibility, Selection and Admissions Policies, including Deconcentration and Wait List Procedures In the ACOP policy, section C6.0 Applicant Selection and Assignment Plan, (b) Second priority will include an additional community program designed to assist clients in living independently of nurising homes and personal care homes. The program is called Money Follows The Person. Recommendation for inclusion in this program will be determined by a local community social services agency, Southwest Georgia Council on Aging. Based on this preference, no more than 10 families, authority wide, will be selected per year. See attached Admission and Continued Occupancy Policy (ACOP) 2. Designated Housing for Elderly and Disabled Families The Albany Housing Authority does not intend to apply, in the upcoming fiscal year, to designate any property as elderly or for disabled families. 7. Community Service and Self Sufficiency The Albany Housing Authority will require residents to meet the requirements of Section 12 of the Act (the United States Housing Act of 1937, as amended). Specifically, all adult residents, except those exempted by virtue of age disability, or involvement with work activities, will be required to contribute eight hours of community services each month or participate in an economic self sufficiency program for eight hours each month as a condition of their lease. This requirement will lbe enforced through the provisions of the dwelling lease and in accordance with 24 CFR Subpart F 960.600-609. AHA, in colloboration with other agencies, provide the following services for families enrolled in the FSS program: Adult literacy classes, individual credit counseling and career counseling. 8. Safety and Crime Prevention Because of the high incidence of violent and/or drug related crimes in some of the developments and areas adjacent, repeated vandalism and graffiti on PHA property, and the presence of gangs, the Albany Housing Authority properties will be the target area proposed in the Bryne Criminal Justice grant that the District Health Dept is applying for and. hope to receive. The AHA has arranged to meet on a regular basis with the gang task force and other law enforcement agencies in the city to discuss issues that the AHA is having in it's developments. or employees report any crime, the police is notified immediately. The AHA has access to reports from the police of calls made to AHA's development, so that assigned staff can address tenants involved in police calls. Albany Housing Authority Plan Elements continued 9. Pets. See attached Pet policy 10. Civil Rights Certification. See 2012 attached signed and dated Civil Rights Certification form. The AHA is incompliance with all Civil Rights Certifications. Staff has received training on fair housing to ensure that citizens have access to affordable, decent housing without discrimination. 11. Fiscal Year Audit. The 2012 audit has been completed with no findings. Review of audit available upon request. 12. Asset Management. The Albany Housing Authority implemented property based management for all of its properties, effective October 2007. All properties were assigned to a specific development office of no less than 188 units. All inventory that was a part of those units was shifted to each site based property, including trucks. All modernization, rehabilitation and dispostion projects are planned and managed by the Cost Center. The Property Manager, through the budgeting process, identifies the needs of the property being managed. Each AMP has its own staff. 13. Violence Against Woment Act (VAWA) See Attached The Albany Housing Authority has amended its Admission and Continued Occupancy Policy to comply with th Violence Against Women and Justice Department Reauthorization Act 2005, Public Law 109-162. Specifically, the policy prohibits the eviction of, and removal of assistance from, certain persons living in public housing or Section-8 assisted housing if the asserted grounds for such action is an instance of domestic violence, dating violence, sexual assault, or stalking. Tenants will be protected, as well as members of their immediate families, from losing their HUD- assisted housing as a consequence of the abuse of which they were the victims. Currently, the Agency does not have a written agreement with other service providers; however, we do receive referrals from the local agencies that provide services and/or temporary shelter for domestic violence victims and we have provided housing to some of these clients.
