The RN confrmed that Patent 1 complaned of pan n hs left chest (the nurse ponted to her left chest area) at 6 out of 10 (0-10 pan scale, 10 beng the worse pan). The. RN stated she dd not ask any more questons regardng the chest pan because Patent 1 dd not look sck and she dd not beleve the patent. She documented t because she "just followed the form." The RN checked the clncal record and confrmed that she dd not ntate the chest pan as a treatment plan ssue. She was not sure of the reason. The RN confrmed she dd not notfy the physcan of the chest pan because the patent dd not look sck to her. A 3951 RN A stated that Patent A told her that he needed PAP every nght for hs sleep apnea (condton wth perods of stopped breathng durng hours of sleep). The patent told the RN the PAP was at -home and he would tell hs wfe to brng t n the next day. When asked what the plan was for the upcomng nght, the RN stated that she developed a care plan ndcatng the patent needed the PAP but she dd not ensure Patent 1 had the PAP.
