Phase III Comprehensive Schedule B Project Implementation Summary INTEGRATED DELIVERY SYSTEM Overview The Performing Provider System (PPS) Integrated Delivery System project promotes the creation of a coordinated health care network that will provide integrated, collaborative, and accountable health care services across the continuum of care based on population health needs. The objective of this transformation is to provide evidence-based, cost-efficient care, including a reduction in avoidable hospitalizations. The Integrated Delivery System will be foundational in supporting excellence and sustainability for all work done within the partner network. All OneCity Health PPS partners are expected to participate in the Integrated Delivery System project. The level of involvement in each of the key elements may vary depending on partner size and types of services provided. Key elements of an Integrated Delivery System include: Access to high-quality primary care Enhancement of community-based services, including behavioral health services Incorporation of community needs assessments into health services planning Quality improvement within health service delivery organizations Support for patient outreach and navigation Cultural competency Health information exchange Value-based financial incentives

N_006 Report on the completion of National Committee on Quality Assurance (NCQA) Patient Centered Medical Home (PCMH) Level III 2014, NCQA PCMH 2017, or NYS DOH Advanced Primary Care (APC) recognition. (for PPS Partners that provide primary care services) N_007 Demonstrate that Electronic Health Record (EHR) meets connectivity to a Regional Health Information Organization (RHIO)/ Health Information Exchange (HIE) and Statewide Health Information Network of New York (SHIN-NY) requirements. N_008 Demonstrate use of an approved social services referral platform to generate and/ or receive at least fifteen (15) social service referrals per quarter. Quarterly 4/30/2018 7/31/2018 10/31/2018 1/31/2019 N_011 Demonstrate high rating on the Centers for Medicare and Medicaid Services (CMS) Five-Star Quality Rating System Score for Nursing Homes. (for PPS Partners that are Nursing Homes) 12/31/2018 Outcome Measures Refer to Section F3 of Phase III Comprehensive Schedule B. Partners are not required to report on these measures. Partners are eligible to earn the Maximum Valuation associated with each Outcome Measure that appears in their contract if: Partner successfully completes at least 50% of its Process Metrics*; and successfully achieves the Outcome Measure, as determined by the New York State Department of Health (NYS DOH). Payments to partners will be calculated by NYS DOH for the PPS as a whole and thus may impact total payments to PPS which, in turn, may impact payments to the individual PPS partners. *For the purposes of calculating the 50% threshold, the following metrics (if they appear in the Partner s Phase III Schedule B) are excluded: N_007; PS_007; PS_008; PS_009; and PS_010.

Complete Learning Management System (LMS) Pre-launch Survey and coordinate with Workforce Development and Training Team to receive access to the LMS 2. Connect to Regional Health Information Organization (RHIO)/Health Information Exchange (HIE) (for partners that provide clinical services and/or have an electronic medical/health record) 3. Achieve National Committee on Quality Assurance (NCQA) Patient Centered Medical Home (PCMH) 2014, NCQA PCMH 2017, or NYS DOH Advanced Primary Care (APC) recognition (for partners that provide adult and/or pediatric primary care services) 4/30/2018 12/31/2018 3/31/2018 4. Train staff on the NowPow Social Services Directory and Referral Platform 5. Other (as described in Implementation Toolkit(s))
