Shultz Ben Lastly 4-H Extension Specialist, Livestock Executive Secretary, Georgia FFA SUBJECT: JOHNATHON GABRIEL MEMORIAL SCHOLARSHIP We are extremely pleased to again offer a scholarship in honor and memory of Johnathon Gabriel, an Oglethorpe County 4-H and FFA member who was killed in an automobile accident in January of 1997. Funds were raised and a scholarship endowed in Johnathon s name. The recipient of this scholarship will be notified in July of 2018. We are looking for a student who embodies the standards of excellence and commitment that Johnathon showed in both 4-H and FFA projects. We need your help in identifying these young people and selecting a recipient for this year s scholarship. The scholarship is a cash award in the amount of $1,200. Details are explained in the attached Statement of Scholarship Agreement sheet. The applicant must be graduating in the spring of 2018. The recipient will be expected to pass along their grade report from the college they attend at the end of the Fall 2018 semester. The recipient will then be recognized at the Exhibitors Banquet of the Georgia Junior National Livestock Banquet in Perry, in February of 2019. Enclosed is an application form which must be filled out and signed by the student s County Extension Agent or Agriculture Education Teacher. The signing teacher or agent must also attach a letter of recommendation. When appropriate, an applicant may enclose letters from both an agent and agriculture education teacher. The completed application and attached letters of recommendation must be received by Heather K. Shultz, at the indicated address, no later than June 15, 2018. Johnathon Gabriel Memorial Scholarship Scholarship Guidelines 1) Recipient must attend the Georgia Junior National Livestock Banquet on February 20, 2019, in Perry, GA, to be recognized and to receive payment of this scholarship. 2) Applicants must be in the 12th grade at the time of application. 3) Scholarship request must be made within one year following graduation from high school. In the event the scholarship is not used within the year of limitation, the winner should request a time extension. Extensions should be requested in a letter to Mary Ann Parsons; Georgia 4-H Foundation; Hoke Smith Annex; Athens, GA 30606. Failure to do so will result in forfeiture of scholarship. 4) Recipient must satisfactorily complete 12 credit hours of college credit and maintain a grade point average of 2.0 or better before requesting scholarship payment. 5) The $1,200 scholarship may be applied toward: tuition at any two- or four-year college or university; room & board; books and supplies; or any other education-related expenses. 6) Recipient must send a written thank you note to the parents of Johnathon Gabriel at: Mr. & Mrs. Larry Gabriel 2279 Beaver Dam Road Colbert, GA 30628 Recipient must also send a copy of the note to the scholarship selection committee in care of Heather K. Shultz. 7) Recipient must attach a copy of semester grades to the Request for Payment, by sending either an official college transcript or a copy of the grade report sent to the student s home. Website printouts of the grades are not acceptable. 8) Request for payment of scholarship funds must be made through County Extension Agent or Agriculture Education Teacher. Scholarship Timeline: June 15, 2018 Applications are due. June 18-22, 2018 Applications will be reviewed. July Recipient notified. Jan. 2019 Submit Grade Report to Heather K.
