Learn healthy habits When you take steps to care for your health, you re making important lifestyle choices that can reduce your risk for serious health conditions. It s not always easy living with a health condition. Sometimes, the medications, tests, and doctor appointments can feel overwhelming. You don t have to face this alone. We re here to help you achieve your best health and well-being. HMSA has a program with well-being resources that offer support, information, and guidance to help you manage your condition. Our program doesn t replace your doctor s care. Instead, it encourages you to work closely with your doctor to make sure that your condition doesn t get worse or cause complications. These resources are available to Blue Cross Blue Shield Service Benefit Plan members who have: - Heart disease. - Hypertension (high blood pressure). How the Program Works If you have any of the health conditions mentioned above, we ll enroll you in the program and send you a letter about it. Then a care coordinator may call you to: Answer your questions about medications, diet, exercise, or any other health concerns. Discuss how you can better manage your health. Remind you about preventive screenings and exams. Help you find a doctor. Give you informational materials. Refer you to HMSA s health education workshops. Frequently Asked Questions Q: How much do I have to pay for this program? A: There s no cost for this program or any of its services. Q: How do I join the program? A: HMSA members who have any of the health conditions mentioned earlier are automatically enrolled. If you haven t received a letter or phone call about the program, call one of the phone numbers on the back of this brochure. We ll be happy to enroll you. Q: Should I tell my doctor that I m in the program? A: Yes. Your doctor will receive regular reports on how you re doing. Q: What can I expect from this program? A: You ll have a better understanding of your condition and how to manage it. The goal is to take care of your health and well-being so that you can have the best possible quality of life. Q: I don t want to participate in this program. How do I opt out? A: Call us at a phone number on the back of this brochure and we ll remove you from the program. Q: Are FEP-specific programs available only to FEP members? A: Yes, FEP members benefit from specific programs only available from Blue Cross Blue Shield and HMSA.

HMSA s Well-being Resources The most important step to managing your condition is to work with your doctor on a treatment plan. In addition, these well-being resources and health education programs are here to help you. Take advantage of these resources for Service Benefit Plan members. Blue Health Assessment* The Blue Health Assessment, or BHA, is a guided survey that gives you a snapshot of your overall health based on the information you provide. It s the first step in the Wellness Incentive Program that will earn you $50 on your MyBlue Wellness Card. Once you complete the assessment, you may be eligible to earn an additional $120 for completing up to three Online Health Coach goals related to nutrition, weight management, exercise, stress management, emotional health, and managing certain chronic illnesses. * You must be the contract holder or spouse on your plan, age 18 or older, to earn incentive rewards. Health Coaching A health coach can provide guidance and support to help you improve your well-being and manage your health condition. For more information or to discuss your specific needs, call 1 (855) 329-5461 toll-free. Online Health Coach* From reducing stress to eating better and exercising regularly, the Online Health Coach can help you reach your health goals. As part of your Wellness Incentive Program, you can earn rewards after you complete the BHA and achieve Online Health Coach goals. You ll receive $40 for each eligible goal you complete, up to three, for $120 total. * You must be the contract holder or spouse on your plan, age 18 or older, to earn incentive rewards. Hypertension Management Program** You may be eligible to receive a free blood pressure monitor with the Hypertension (high blood pressure) Management Program. If your doctor says you have high blood pressure, complete the BHA and answer yes to the question about a high blood pressure diagnosis. After we receive a claim from your provider noting that you have high blood pressure, we ll automatically enroll you in the Hypertension Management Program. This program can help you monitor your blood pressure at home. ** You must be the contract holder or spouse on your plan, age 18 or older, to be eligible for the Hypertension Management Program. You re eligible to participate in this program every two years. Diabetes Management Incentive Program Our Diabetes Management Incentive Program can educate and support you as you take an active role in managing your diabetes. The contract holder and spouse, 18 and over, can participate in the program and earn up to $100 on their MyBlue Wellness Card. If eligible, you can use these rewards to pay for qualified medical expenses, such as copayments and prescriptions. To learn more and start earning your reward, go to fepblue.org/diabetes. Asthma Management Resources* To live better with asthma, take steps to protect your health. These tools and resources can help: Refill your prescription online: One of the most important steps in managing asthma is to use the correct medication as directed and get refills as needed. Asthma tracker: Track your medication and inhaler use and monitor how well you re breathing with measurements like peak expiratory flow and forced expiratory volume. Asthma assessment: Easily assess your symptoms and learn about options for care. You ll get tips on setting goals based on the information you provide to help create a care plan to manage your asthma.

Blue Cross Blue Shield s Well-being Resources (continued) Personal support: We offer patient education and clinical support through our Care Management Programs. Wellness Incentive Program: Complete the BHA or set asthma-related goals with your Online Health Coach to earn up to $120.* Contact Blue Cross Blue Shield for more information on the asthma management program near you. * You must be the contract holder or spouse on your plan, age 18 or older, to earn incentive rewards. HMSA Pregnancy Support Program To help you have a safe and healthy pregnancy, this program pairs you with your own maternity nurse who ll call you to provide personalized education and counseling. Nurse support doesn t replace the care you receive from your doctor, but provides additional services to complement your regular prenatal care. This is a voluntary program that s available to HMSA Service Benefit Plan members in Hawaii at no cost. For more information, call 948-6079 on Oahu. Pregnancy Care Incentive Program Our Pregnancy Care Incentive Program offers expectant mothers support and incentives throughout their pregnancy. Go to fepblue.org/ maternity to learn if you re eligible to participate. Healthy Families The Healthy Families programs provide support, advice, and guidance for families with children and teens. Healthy Kids offers games, activities, and tools to teach children about weight management and healthy lifestyle habits. The Healthy Teens program offers a special body mass index (BMI) calculator and information to help you respond to your teen. Behavioral Health Services Beacon Health Options can recommend behavioral health providers, coordinate care, and provide care management for depression and substance abuse issues. Telehealth Services See a doctor or other health care provider from the comfort and privacy of your home, office, or anywhere you can go online. Get real-time answers to your health questions and concerns. Go to fepblue.org/telehealth, call Teladoc at 1 (855) 636-1579 toll-free, or download the Teladoc app. 24/7 Nurse Line Need health advice? Call the Nurse Line at 1 (888) 258-3432 toll-free for well-trained and caring professionals ready to help you 24/7. If there s an emergency, call 911. Tobacco Cessation Incentive Program With the Tobacco Cessation Incentive Program, you ll get tips, support, and a range of smoking and tobacco cessation drugs at no charge. To get started, take the BHA and indicate that you use tobacco. Learn more at fepblue.org/tobacco. Stop Smoking Quitting smoking is one of the hardest things to do. The Hawai i Tobacco QuitLine gives you the support you need to quit and improve your health and well-being.
