CONSTRUCTION MANAGEMENT STATEMENT OF QUALIFICATIONS I. NOTICE FOR PROCUREMENT Pursuant to Florida Statutes, the State of Florida s Consultants Competitive Negotiations Act, and The State Requirements for Educational Facilities, the School Board of Alachua County, Florida (SBAC), will consider the contracting of a Construction Management- At-Risk firm for providing professional services for: Construction project known as SBAC Project A1801 New Elementary School I, Parcel # 04427-002-000 located in Alachua County, Florida. The project consists of: Pre-Construction services including: constructability review; value engineering; cost control and the development of a Guaranteed Maximum Price; and Construction Services including site work and new construction. Instructions for completion and submission of the Statement of Qualifications may be obtained at the following website: https://fl02219191.schoolwires.net/page/1287 A mandatory pre-submittal meeting will be held to review the requirements of the Statement of Qualifications at 10:00 a.m. local time, Wednesday, February 27, 2019, at the SBAC, Sivia Support Center, Facilities Department, 3700 NE 53 rd Avenue, Gainesville, Florida 32609. Business entities interested in providing Construction Management-At-Risk services to SBAC are hereby notified that eight sealed Statements of Qualifications for providing the required services must be received by 3:00 p.m. local time, Wednesday, March 6, 2019, at the Facilities Department located at the SBAC, Sivia Support Center, 3700 NE 53 rd Avenue, Gainesville, Florida 32609. In accordance with Florida Statutes, a person or affiliate who has been placed on the convicted vendor list following a conviction for a public entity crime may not submit a bid on a contract to provide any goods or services to a public entity, and may not be awarded or perform work as a contractor, supplier, subcontractor, or consultant under a contract with any public entity, for a period of 36 months from the date of being placed on the convicted vendor list. SBAC may reject all applicants, may stop the selection process at any time, may reject any or all Statements of Qualifications, and reserves the right to waive any informalities in the selection process. CONSTRUCTION MANAGEMENT STATEMENT OF QUALIFICATIONS II. INSTRUCTIONS: 1. Submit a Letter of Introduction Summary of the qualities that best qualify the applicant for this project. (Maximum 1 page) 2. Provide a Table of Contents. 3. Complete the Construction Management Statement of Qualifications in as much detail as possible and submit eight (8) bound copies per the legal advertisement. Proposals must be limited to eighty (80) pages. Number each side of each page consecutively, including letter of introduction, brochures, licenses, resumes, joint venture agreements, and any supplemental information. Covers, table of contents and divider tabs will not count as pages. Work product samples (estimates, schedules, reports, etc. provided in response to questions 16-18) will not be counted in the proposal page limit. Package these separately from the proposal, labeling each sample clearly. Submit only one (1) copy of the work product sample package. Any proposals exceeding the 80 page limit will be disqualified. 4. Submit copies of applicant s current State of Florida Contractors License. Provide copies of current State of Florida Department of Professional Regulation Construction Industry Licensing Board Certificate of Corporate Authorization showing: (1) License Number; (2) Certificate of Authorization date; and (3) designation of professional(s) qualifying the corporation to practice as a General Contractor. 5. Include a completed Public Entity Crime Statement for the applicant. As required by Florida Statutes, a Construction Management firm may not submit a proposal for this project if it is on the convicted vendor list for a public entity crime committed within the past 36 months. (See Exhibit A Form). 6. Attach a letter of intent from a surety company indicating the applicant s bondability for this project. The surety shall acknowledge that the firm may be bonded for each phase of the project, with a potential maximum construction cost of $23,500,000. The surety company must be licensed to do business in the State of Florida, and must have a Best Rating of A. 7. Only individual firms or legally formed joint ventures may apply for this project. Two or more firms may not apply jointly unless they have formed a joint venture. Joint ventures shall include a copy of the joint venture agreement. This does not preclude an applicant from having consultants. Any associations will be disqualified. Page 2 of 11

List the total number of firm personnel by skill group; clerical, administrative, architects, engineers, foremen, superintendents, skilled employees, technicians, schedulers, estimators, unskilled employees, and other (specify). b. List the names of all key personnel which will be part of the Construction Management team for this project and provide their cities of residence. Describe in detail the experience and expertise of each team member and include a resume. Key personnel must be committed to this project for its duration. c. For each project listed in response to question 15, list the members of the proposed team who worked on the project and describe their roles. d. If the team, as a whole, provided Construction Management services for any of the projects listed in response to question 15, so indicate. 14. List any consultants which are included as part of the proposed Construction Management team. Describe each consultants proposed role in the project and its related experience. List projects on which your firm has worked with the consultant in the past. V. CM EXPERIENCE & REFERENCES: 15. a. List five (5) projects for which your firm has provided, or is providing, Construction Management services which are most related to this project. In determining which projects are most related, consider: related size and complexity; how many members of the proposed team worked on the listed projects; and, how recently the project was completed. List the projects in priority order, with the most related project first. (See Exhibit B format) b. For each of the listed projects, provide the following information: location, construction cost (original GMP and final contract amount), current phase of development, original and actual (or projected) completion date, type of Construction Management services provided, Owner s contact person and telephone number, and the name and telephone number of the Project Architect. How did you develop cost estimates and how often were they updated? Provide examples of how these techniques were used and what degree of accuracy was achieved. Include examples of successful value engineering to maintain project budgets without sacrificing quality. Submit a work product sample of a cost estimate. 17. For two (2) of the projects listed in response to question 15, describe the way in which your firm developed and maintained project schedules. How did you develop schedules and how often were they updated? Provide examples of how these techniques were used. Include specific examples of scheduling challenges, and how your firm helped solve them. Submit a work product sample of a project schedule. 18. For two (2) of the projects listed in response to question 15, describe the types of records, reports, monitoring systems, and information management systems which your firm used in the management of those projects. Submit a work product sample of each report used. 19. For two (2) of the projects listed in response to question 15, describe any conflicts or potential conflicts with the Owner or with trade contractors, and describe the methods used to prevent and/or resolve those conflicts. 20. For two (2) of the projects listed in response to question 15, describe the way your firm maintained quality control during the pre-construction and construction phases. Provide specific examples of how these techniques were used. 21. Give full information about all your present uncompleted contracts.
