HRM CERTIFICATE PROGRAM MODULE 1: ESSENTIALS IN HEALTH CARE RISK MANAGEMENT ACTIVE FACULTY JOYCE H. BENTON RN, MSA, ARM, CPHRM, DFASHRM, DSA, INDEPENDENT RISK CONTROL CONSULTANT Joyce Benton is an award-winning leader and independent risk control consultant who has provided customized risk management consultation services both nationally and internationally. Prior to becoming an independent consultant, she was the AVP, Healthcare Risk Control for CNA where she led a team of healthcare risk control professionals, providing enterprise risk control services for a wide-range of healthcare entities both nationally and internationally. In her career, she also served as the Director of Quality Management and Safety at Henry General Hospital, an acute care hospital in the suburbs of Atlanta. In that role, she developed the risk management and infection control programs and assisted in restructuring the safety, workers compensation and quality improvement programs at the facility. Ms. Benton began her career as a registered nurse in surgical pediatrics at Grady Memorial Hospital in Atlanta and newborn nursery and childbirth education at Henry General Hospital. Ms. Benton earned a Diploma Degree in Nursing from Georgia Baptist Hospital School of Nursing, a Bachelor of Science in Health Arts from the College of St. Francis and a Master of Science in Administration with a concentration in Health Services from Central Michigan University. In 1991, she earned her Associate in Risk Management (ARM) from the Insurance Institute of America. Ms. Benton is an active member of the American Society for Healthcare Risk Management (ASHRM), served on ASHRM s 2003-2004 Board of Directors and currently serves as the chairman of the 2018 Nominating Committee. She was awarded the Fellow designation (FASHRM) from ASHRM in 1992 and the Distinguished Fellow designation (DFASHRM) in 1998. Ms. Benton completed ASHRM s Healthcare Risk Management Certificate Program in 1996. She also received the Certified Professional in Healthcare Risk Management (CPHRM) designation from ASHRM in 2000. Ms. Benton was also the recipient of ASHRM s Excellence in Writing Award in 1999, Distinguished Service Award (DSA) in 2005 and Presidential Citation Award in 2014. In addition, Ms. Benton is a member of the Georgia Society for Healthcare Risk Management (GSHRM) and received the 1995-1996 GSHRM s Risk Manager of the Year Award, the 2002 Distinguished Service Award for outstanding contributions in the advancement of the profession of risk management and service to GSHRM, and 2004 GSHRM Lifetime Achievement Award. In 2010, she received the Distinguished Alumni Award for Excellence in Clinical Development by Georgia Baptist College of Nursing at Mercer University. Ms. Benton is a frequent speaker on healthcare risk management topics at the state, national and international level and has authored articles and chapters on healthcare risk management. She was a contributing author for the 4 th, 5 th and 6 th Editions of ASHRM s Risk Management Handbook for Healthcare Organization; the 2003, 2006 and 2014 CPHRM Examination Preparation Guide; and the 2017 Health Care Risk Financing Playbook: Risk Identification, Monitoring and Insurance. She has served as faculty for ASHRM s Healthcare Risk Management Certificate Program, CPHRM Exam Preparation Course and Insurance 101. Ms. Benton is a master trainer for TeamSTEPPS.

KAREN S. GARVEY BSN, MPA/HCA, DFASHRM, CPHRM, CPPS, VICE PRESIDENT, SAFETY & CLINICAL RISK MANAGEMENT, PARKLAND HEALTH & HOSPITAL SYSTEM Karen Garvey has been with the Parkland Health & Hospital System for close to five years where her responsibilities have included enterprise culture change focused on Patient Safety as well as integration and oversight of services of Infection Prevention, Regulatory & Accreditation, Patient Safety and Clinical Risk Management. Much of this work has been completed while under the watchful eye of the Office Inspector General (OIG) focus due to a five-year Corporate Integrity Agreement (CIA) focused on Quality & Patient Safety. Parkland successfully completed the CIA in May 2018. She actively leads several committees and projects focused on risk mitigation and is an active member of the Parkland Enterprise Risk Management (ERM) Steering Committee. Prior to joining Parkland in 2014, Ms. Garvey served as the Senior Director of Patient Safety & Clinical Risk Management for all Cleveland Clinic domestic and foreign facilities in making improvements in patient safety, culture change, transparency and medical error disclosure. Ms. Garvey s clinical background includes high risk obstetrics, neonatal intensive care and adolescent pediatrics. She found her way into the healthcare risk management due to a significant obstetrics event she and her team experienced in Labor & Delivery. She drew on the experience of this event which later moved her into the field of healthcare risk management and patient safety where she continues to mentor others in this line of work and is a frequent speaker at the local, national and international levels. Ms. Garvey has led many educational programs and staff support initiatives including the development of Clinician Support Groups (2 nd Victims), Medical Error Disclosure Simulation Program, RCA Education for residents and providers, TeamSTEPPS Implementation, Demystifying the Root Cause Process, RCA Lessons Learned through Patient Safety Alerts, Patient Safety in Graduate Medical Education curriculum, Just Culture Principles and Anatomy of Errors. Ms. Garvey is an active member of the American Society for Healthcare Risk Management (ASHRM), served as Chairperson or a member on several Committees and Task Forces and is a current member of the ASHRM Board of Directors. She has served as the ASHRM liaison for the organization, Medically Induced Trauma Support Services (MITSS) where her passion is staff support program development. Ms. Garvey also serves as active teaching faculty for the Healthcare Risk Management Modules (Essentials) and for ASHRM s program for the Certified Professional in Healthcare Risk Management (CPHRM) where she has taught since 2008. Ms. Garvey has also been a contributing author to several of the ASHRM Pearls and the 2006 and 2014 CPHRM Examination Preparation Guide. Ms. Garvey holds a Bachelor s degree in Nursing and a Master s degree in Public Administration with a focus on Healthcare Administration. She also holds a IHI Fellow designation as a graduate of the IHI/NPSF Patient Safety Leadership Program. She was selected as a DFW Great 100 Nurse in 2017 and was also awarded the Distinguished Fellow (DFASHRM) designation from ASHRM that same year. She also holds the Certified Professional in Healthcare Risk Management (CPHRM) designation from ASHRM and attained the Certified Professional in Patient Safety (CPPS) designation from the National Patient Safety Foundation/ Institute for Healthcare Improvement (NPSF/IHI).

JEAN HOOD RN, MSN, CPHRM, CPPS, FASHRM, DIRECTOR, CLINICAL RISK MANAGEMENT, ADVENTHEALTH Jean Hood is the Director of Clinical Risk Management for AdventHealth which operates and provides healthcare services for many hospitals, clinics, physician practices, skilled nursing facilities, home health agencies and hospice care centers across nine states. Jean has over 35 years of healthcare experience, including time spent as a bedside nurse in critical care, surgical services and the outpatient setting and then as a nurse in leader in Quality and Risk Management. She was a hospital-based Risk Manager prior to being recruited by AdventHealth in 2006 to help develop a robust, system-wide Clinical Risk Management / Loss Control program. Currently, Jean directs clinical risk management efforts and guides unified risk, patient safety, and employee safety efforts across the large healthcare system. This work has led to the creation of several risk and safety assessment tools, claim investigation guides, regulatory response strategy guides, dashboards and presentation templates. This work has promoted a Just Culture of clinical risk management within AdventHealth. In 2018, Jean s Clinical Risk team was the recipient of ASHRM s Patient Safety Innovation Award for their Human Factor Engineer Approach in Conducting a Root Cause Analysis. Jean is an active member of the American Society for Healthcare Risk Management (ASHRM). She currently serves on the Chapter Leader Task Force, the Education Development Task Force and as faculty for ASHRM s Healthcare Certification Program Modules (Essentials). Jean is also very active in the Florida Society for Healthcare Risk Management and Patient Safety (FSHRMPS). She is an officer on its Board of Directors and currently participates in the Advocacy, Education, and Membership committees. Jean was awarded FSHRMPS Professional Recognition Award in 2015. Jean shares her knowledge and passion for Risk Management education in her role as adjunct faculty for University of Central Florida s healthcare risk management continuing education program and is also a frequent guest lecturer on Risk Management s role in Healthcare at AdventHealth University. Jean holds a bachelor s and a master s degree in nursing and holds certificates as a certified healthcare risk manager (CPHRM) and certification in patient safety (CPPS). Away from work, Jean enjoys traveling adventures with her husband of 35 years.

GEORGENE SALIBA RN, BSN, MBA, CPHRM, FASHRM, SENIOR DIRECTOR, CORPORATE RISK MANAGEMENT ACUTE CARE, UHS OF DELAWARE, INC. Georgene is the Senior Director, Corporate Risk Management, Acute Care for UHS of Delaware, Inc. She is responsible for analysis and oversight of the risk management and patient safety functions for all UHS acute care facilities, including participation in facility surveys to evaluate functionality of the facility risk programs and performance of risk staff. She directs organizational efforts in planning, implementation evaluation and management of the risk program, including consultation, direction and assistance to corporate risk staff for the implementation monitoring and enforcement of all aspects of the risk program. Prior to coming to UHS she was the Administrator Risk Management & Patient Safety for Lehigh Valley Health Network in Allentown PA. She is a diploma graduate of the Hospital of the University of Pennsylvania, obtained her BSN from Cedar Crest College and her MBA from DeSales University. She holds the CPHRM and FASHRM designations. She is a Past President of ASHRM and a current faculty member. She has been a speaker both nationally and regionally on risk management and patient safety topics. DENISE SHOPE RN, MHSA, ARM, CPHRM, DFASHRM, HEALTH CARE RISK MANAGEMENT CONSULTANT, RCM&D Denise joined RCM&D in 2008 as a Risk Management Consultant. She brings 33 years of extensive healthcare experience, 25+ years of which she spent working in healthcare risk management handling risk management issues for tertiary care systems, physician groups, post-acute care and enterprise-wide health care delivery systems. She is a nationally recognized healthcare enterprise risk management professional. Prior to joining RCM&D, Denise began her career as a registered staff nurse at Geisinger Medical Center, and then progressed through several advancements, including to the position of Geisinger Health System Director of Performance Improvement and Risk Management, and Patient Safety Officer. She held leadership positions with the Pennsylvania Association of Health Care Risk Management and served on the Board of Directors for the American Society for Healthcare Risk Management (ASHRM) from 2011 through 2014. She is currently ASHRM s 2019 President. Denise has chaired ASHRM s Enterprise Risk Management Advisory Committee and is faculty for ASHRM s ERM Certificate program and ASHRM s Healthcare Risk Management Certificate program. She speaks nationally on healthcare enterprise risk management topics. She holds the Designated Fellow of the American Society for Healthcare Risk Management and is a Certified Professional Healthcare Risk Management (CPHRM) designee. In addition to her advanced degree and certifications she is a licensed producer for Property and Casualty insurances.

CYNTHIA (CYNDI) S. SIDERS RN, MSN, DFASHRM, CPHRM, CPPS, CEO AND EXECUTIVE CONSULTANT, SIDERS HEALTHCARE CONSULTING Cyndi has more than 30 years of healthcare, administrative and insurance experience, 25 of those focused on risk management and patient safety. Cyndi s responsibilities as CEO and Executive Consultant include providing customized risk management and patient safety coaching, training and professional consultation for healthcare organizations and health systems, patient safety organizations, insurance companies and risk management companies. Cyndi has served as vice president of consulting services for Coverys providing leadership for a team of consultants delivering risk management and patient safety consultative services, education and strategic support for clients nationally. These services include educational programs and training; comprehensive risk management, patient safety and quality improvement evaluations; coaching and mentoring programs and consultative services for healthcare systems, hospitals, physician office practices, community health centers and long-term care facilities. She was also responsible for the development of proactive risk management and patient safety products and services. Cyndi has served as corporate risk manager for an integrated health system. In this role, she was responsible for the overall risk management and patient safety program, loss control and risk financing, claims administration, contract review, patient relations, and served as a liaison with insurance and regulatory representatives. She has previously served as senior consultant and regional director for two national professional liability insurance companies. Cyndi has Bachelor and Master of Science degrees in nursing from the University of North Dakota. She is a graduate of the Healthcare Risk Management Certificate Program co-sponsored by MMI Companies Inc. and the University of Health Sciences/The Chicago Medical School. Cyndi is a distinguished fellow with the American Society for Healthcare Risk Management, a certified professional in healthcare risk management from the American Hospital Association, a certified professional in patient safety from the certification board for professionals in patient safety and an advanced master trainer in TEAMSTEPPS. Cyndi is past president of the North Dakota Society for Healthcare Risk Management and serves as faculty for the ASHRM Health Care Risk Management Certificate Program. Cyndi is a frequent state and national speaker on a variety of risk management and patient safety topics and has authored several national publications, including a chapter in the ASHRM Risk Management Handbook, Sixth Edition, ASHRM Health Care Risk Management Fundamentals and Health Care Risk Management workbook. Cyndi is actively involved in various committees and Task Forces for ASHRM including the ERM Taskforce. She is a past recipient of the ASHRM Research Incentive Award.
