Whitley branch surgery closure complete, no major capacity issues. In principle support given to premises scheme which would rehouse South Reading Surgery as well as allowing capacity to absorb population growth. Annex 1: Extract from CCG s Risk Management Strategy 2015-16 Risk Assessment Tool (Risk Matrix) The CCG has adopted a risk assessment tool, which is based on a 5 x 5 matrix (Used by Risk Management AS/NZS 4360:1999, revised 2004).
