Position: Location: FTE Reports To: Responsible For: Adult Mental Health Clinician Dunedin 0.5 FTE Clinical Nurse Manager, Family Mental Health Services Nil Overview WellSouth Primary Health Network has responsibility for the provision of effective primary health services to approximately 290,000 enrolled patients in Otago and Southland, and contracts with Southern DHB and a number of other health funders. WellSouth is constituted as a charitable trust with a Board of 10 Trustees and an advisory groups comprised of Maori, Community, and Clinical representatives. WellSouth works with health care clinicians, health care providers, Iwi and local communities to: Coordinate the delivery of regionally consistent high value primary care health services care across Otago and Southland that deliver real benefits to patients and the community through a range of evidence based and clinically proven health programmes. Improve access to primary health care for the community. Encourage people to take responsibility for their own health. WellSouth has offices in Alexandra, Invercargill, Wanaka and Dunedin. Position Purpose The primary objectives of this position are: To work as a member of Family Mental Health Service multidisciplinary team and complete a comprehensive mental health assessment and brief therapeutic interventions up to a 6-8 month period. The Family Mental Health Service provides comprehensive screening assessments for people presenting with moderate to severe mental health difficulties.

Main Objectives The key objectives of the position are: To carry a clinical caseload for adults over 19 years of age. Develop inter agency relationships and service co-ordination to ensure clients and their families receive timely and appropriate support and interventions. Clinical practice framework for working with adults is evidence based and encompasses the strength based model. Contribute to the development and evaluation of the Family Mental Health Service. Participate in ongoing development of specific treatment programmes within the service. Maintain ethical standards of practice and comply with the professional ethics of your Professional Association. Key Tasks and Accountabilities The table below provides an overview of the key responsibilities for this position in the form of deliverables and expected outcomes. KEY TASKS/ACCOUNTABILITIES Client Undertake a case manager role for a limited caseload of clients, either within the allocated base or in the community when required. PERFORMANCE INDICATORS Respond to requests for brief assessment and screen referrals to determine need for further assessment and/or intervention, considering referral to and liaison with other community services where appropriate. Provision of comprehensive mental health assessment and co-ordination of treatment and discharge for clients. Provide appropriate treatment and/or management of adults through specific and evidence based therapeutic modalities. Complete a comprehensive assessment and maintain accurate and up to date clinical records. Participation at Multidisciplinary Team meeting to present and discuss clinical information of clients. When appropriate, provide consultation to other services where adults and their families are involved. Demonstrates provision of interventions, which occur within a specific evidence-based therapeutic model; are relevant and appropriate for the individual, cultural and gender identity needs of the consumer; includes the monitoring of outcomes. -Effective co-ordination of services, particularly Primary Health Services and Secondary Mental Health services. -Appropriate documentation of assessment, treatment and discharge plans and clinical reviews in accordance with Family Mental Health policies. -Input and case review at Clinical Review, presenting and discussing clinical information -Effective inter-sectorial communication. Participate in education/in-service including compulsory modules. Knowledge of professionally relevant Code of Ethics. Evidence of attendance at Education/Inservice, including emergency and OH&S procedures. Working knowledge of relevant legislation, policies and procedures. Able to locate and access relevant Legislation. Evidence of having read and work in accordance with Family Mental Health Service and WellSouth policies.
