As soon as it is expected that there is a likelihood the patient will require a longer than typical hospital stay time to start thinking... Harbor LTACH???

We need to be good stewards of our patients Medicare days, so the faster we can move the patient through the Healthcare Continuum, the better! The average length of stay for our MEDICARE patients is approximately 25 days The length of stay for patients with HMOs or private insurance is determined by their insurance company case manager and medical director, depending on patient s condition

With early referral, we can perform a preadmission screen and coordinate with you to develop a discharge plan as ordered by the attending physician. Our task is to assist in obtaining a receiving physician, if needed, complete the admission criteria, review the patient s medical record, refer to Harbor Case Manager and expedite approval. Thus allowing for a seamless transition from facility to facility.

We can help decrease your hospital s length of stay We can free up beds in your ICU so you can admit more patients We can help keep your hospital from going on diversion!
