EOSC ACADEMIC SCHOLARSHIP APPLICATION FOR DEPENDENT CHILD CATEGORY 1: GRADUATING HIGH SCHOOL SENIOR Eligibility Requirements: Dependent Child must have a cumulative, unweighted high school GPA of 3.0 or higher. Applicants must be planning to attend an accredited college or university, within the U.S. or abroad, full-time during the Fall 2019- Spring 2020 academic year in pursuit of an undergraduate degree. Applicants must be the dependents of active duty, retired, or deceased enlisted and officers of the United States Air Force, United States Air Force Reserve, and Air National Guard, assigned to or residing near Eielson AFB. Dependent status will be determined in accordance with AFI 36-3001 or Federal Internal Revenue Service regulations, whichever is more favorable to the applicant. The following are NOT ELIGIBLE for an award: 1. Any Scholarship Rating Judge on the panel or his/her child 2. Scholarship Chairperson or his/her child Criteria for Award: The scholarship awards shall be based upon the following criteria: scholastic ability, extracurricular activities/work experience, extraordinary circumstances, service activities/citizenship, personal essay, and letters of recommendation. The Scholarship Rating Committee, an independent panel, shall evaluate and score the applications in a blinded process. This panel is made up of military and community members. No member of the EOSC is on the Scholarship Rating Committee. Number and Amount of Awards: The EOSC Scholarship Committee will determine the number and total amount to be awarded. Application Procedure: The completed application, personal essay, letters of recommendation, and unofficial transcript must be postmarked by February 22, 2019. Any application that is postmarked AFTER the above date will NOT be accepted. Late or incomplete applications will not be considered. The completed application packet must include all of the following: Completed Scholarship Application. Please type or write legibly in blue or black ink. 1. Incomplete applications will not be reviewed. Proof of eligibility must accompany the application. Acceptable proofs of eligibility are: 1. Copy of PCS orders (AF form 899) to Eielson AFB listing the applicant s name as a dependent. If retiree or other category in which PCS orders is not applicable please provide proof of local residency through a local utility bill. 2. Notary statement from the Base Legal Office verifying the applicant s dependent status. To obtain statement, military member must present his/her military ID along with dependent s ID or other proof of dependency. DO NOT SUBMIT COPIES OF ANY MILITARY ID CARDS. It is illegal to photocopy IDs for non-government agency use. Official high school Transcript mailed directly from the records custodian at the high school to our mailbox or submitted in a sealed school envelope. Copy of official ACT or SAT report/scores reported on official high school transcript. Two signed letters of recommendation from adults not related to the applicant. The person writing the letter must be a teacher, faculty member, coach, supervisor or mentor. These letters can either be included with the application packet or mailed separately by the author of the letter. Personal Essay: Write a personal essay (2 page, double spaced, 1 inch margins, Times New Roman, font size 12), answering ONE of the following: a. Frequent moves, an inherent part of the military lifestyle, have allowed you to live in diverse places and form relationships with a wide variety of people. How have these experiences and relationships influenced your educational opportunities and subsequent choices? b. Discuss your previous service to others and the community. Explain how your sponsor s service in the U.S. military has influenced your actions or inspired you. What have you learned through your own service and sacrifice? c. How will your field of study contribute to your immediate or long range career plans? Explain the importance of your major in today s society. What do you think your industry will be like in the next 10 years? What are the most important issues your field is facing today? Please type the chosen essay question at the top of the page. Recipient s Use of Award: The scholarship must be used by the recipient at an accredited 2-4 year college, university, vocational or technical school during the Fall 2019- Spring 2020 academic year. In addition, scholarships may be used at a school pending accreditation by an organization approved by the US Dept. of Education. The scholarship must be used for payment of tuition, fees, room/board, or books. Recipients must submit the Scholarship Award Information Sheet to the EOSC Welfare Chairperson by April 29, 2019. This information sheet will be given to each recipient following the announcement of winners. Verification of enrollment letter or email from the educational institution s registrar office must be received by the Welfare Chairperson by April 29, 2019. Electronic verification through National Student Clearinghouse will be accepted. Any fee associated with this service is the responsibility of the recipient. The EOSC Welfare Treasurer shall issue a check in the full amount of the scholarship payable to the educational institution that has verified the recipient s enrollment. This check shall be sent on May 1, 2019 after receiving all necessary paperwork form the recipient and the educational institution. Any scholarships not claimed by April 29, 2019 will be forfeited.
