All other roles can view patient clinical records for patients at facilities within their scope. The patient must be admitted to the facility and have a treatment record in the selected modality (HD or PD) during the selected clinical month (including Transient patients). Let s get started!

If no lab data is available, you can satisfy the submission guidelines by using the Not Available (NA) checkbox. CROWNWeb does not have a report that summarizes the data that has been submitted for all patients that month. You will need to view the clinical screens for each patient individually to validate this information.

The transient facility can find vascular access data for a transient patient using the following steps: 1. From the patient s View Patient Attributes screen, click on the Clinical Data Summary link in the gray sub-menu. 2. Click on a previous Clinical Month s link to view that month s clinical lab data for the patient. 3. Under Vascular Access, you can view and note the Date Access Type Changed, AV Fistula Usable Date and/or Fistula Creation Date fields, and enter that data in the transient patient s Vascular Access fields. 4. Complete any of the other required fields in Vascular Access.
