CEO Summary 2014 Summer Patient Newsletter Update: Bridgette McGivern, CEO After the winter we had this past year, I hope everyone is able to enjoy the beautiful summer days as before we know itit will be fall! CHDC continues to turn the heat up by adding new sites, services, providers and programs to provide our community with quality health and dental care for all ages! Please enjoy this issue of our quarterly patient newsletter as our staff is excited to provide some interesting, knowledgeable and fun information for you! Current and Upcoming projects to improve your patient visit at CHDC: If you haven t been to our new location at 700 Heritage Drive in Pottstown, you have an opportunity as a patient if your Provider is scheduled to work there and as a registered patient you can access our WALK-IN services from 8am-5pm, Monday-Friday. We developed this service to help provide more sick visits for patients who may not be able to get an appointment as quickly as you would like when you need them the most. Please grab a Walk-In flyer at the front desk and review what services are offered for your convenience. (CEO Summary continued from page 1) 700 Heritage Drive provides medical services for children and adults and specialized services for children s dental needs. CHDC is working hard to provide more intense women s health and prenatal care by working with a local OBGYN group and offering these specialized services at 700 Heritage Drive. We hope to have this program in place by the fall of 2014 and expanding prenatal services by the beginning of 2015. Do you need transportation? We have been offering FREE transportation for patients who need this service to assist with getting to their appointments. We are currently developing a schedule to begin pickups/drop-offs in the Boyertown/Douglassville area to help patients who need transportation so please stay tuned for more information. Customer Service Training program is being developed for staff and will be an in-depth initiative over the next few months to provide training for all staff to enhance your patient experience! I am developing a Community Committee to help me understand and develop new programs that this community needs. I asked the staff to refer some patients that may be interested in joining this committee and helping CHDC make informative decisions. My goal of this committee is to also work with people who can understand what CHDC is providing to the community and then have this information relayed back to people s communities. If you are interested in becoming part of this committee please feel free to email info@ch-dc.org. Do you have an email account? If so, please register it with the front desk staff as we are trying very hard to increase the email accounts from our patients so we can improve services for you. Over the next couple of months, we will be adding an electronic portal where you can create an account and access certain information, send emails to your health care team, cancel and/or schedule an appointment and many other exciting features- all at your fingertips! We can t offer you this service if you don t provide an email account. If you need help in setting up an email account, please refer to the cheat sheet in this newsletter to help you. CHDC is creating a team of staff to help each of you with all your health and dental care needs. In a few months, you will be receiving a Total Care Program magnet and a detailed description explaining who is on your team and what their role is to take care of you! If you sign up for the patient portal, you will have access to email certain members of your team to help answer any questions you may have. Members of your team would include: Medical/Dental/Behavioral Health Provider Health/Life Coach Medical Assistant Nutritionist Patient Case Coordinator Care Nurse Manager Pharmacy Technician This exciting team of caring people will be your direct access to receive quality care and provide additional support in all different areas. This team will help you navigate the healthcare world which can be difficult in this complicated world - not to mention the daily tasks of life. Please continue to watch for the announcement of the Total Care Program! As you can imagine, we are very busy with these projects and day to day operations. We have great employees and I am proud to work with each of them. Grilled Corn on the Cob Healthy Bites on a Budget Ingredients: - 6 ears of corn - Salt and pepper to taste - 6 tablespoons butter softened Directions: 1) Pre-heat an outdoor grill for high heat and lightly oil grate. 2) Peel back corn husks and remove silk. Place one tablespoon butter, salt and pepper on each piece of corn. Close husks. 3) Wrap each ear of corn tightly in aluminum foil. Place on the prepared grill and cook approx. 30 minutes, turning occasionally, until corn is tender. Enjoy! Providing a Medical Home for Our Patients By: Dr. Gary Davis, Chief Medical Officer Community Health and Dental Care has an ongoing process for quality improvement. This includes our seeking recognition as a Patient Centered Medical Home (PCMH). Over the next year, you will be seeing and hearing this term. For you, the patient, this means that all of your care starts here. We treat and help with all that we can as it pertains to primary care whether it is acute care, chronic disease management or well/preventative care. Beyond medical care we strive to integrate dental, mental health and nutritional care. For those issues that are beyond our scope, we will help you get access to the providers you need. As you see providers outside of CHDC, we will keep track of those visits and make sure that information flows easily between CHDC and your specialty providers. This may mean a friendly reminder from our staff to follow up with the specialists that are involved in your care. For those of you who have chronic diseases (Diabetes, Hypertension, Asthma) or have a need for preventative screenings (Pap smear, Colonoscopy, Mammogram, Immunizations) we will be actively trying to keep you up-to-date. Again, you may expect friendly reminders from us. How do we know who needs what? Well, one of the pillars of being a Patient Centered Medical Home is information management. With our electronic medical record we are able to identify patients by age and/or medical condition. This information cross referenced with the recommended tests or procedures gives us a group or list of people to whom we will reach out. With this active approach, we hope to improve the health of each of our patients and community as a whole. Our access to information is your access to information. Our electronic record will soon have an internet feature (portal) that will allow you to communicate with us electronically request refills, correspond with staff. The portal feature requires email access, so we will be asking you for your email address at upcoming visits. As the center of the process, you can be an active participant- use the portal, keep us up-to-date with your care that is not at CHDC, follow up for screenings, visits, and immunizations. When you see or hear of Patient Centered Medical Home, know that it means easy access to personalized care supported by cutting edge information technology. Most importantly, remember that PCMH means improvement in quality of care that is going on right here at CHDC. Page 4

Enjoy the Sun Safely this Summer! By: Deborah Cellers, CRNP Choosing to be outside and enjoying the sun is a good idea, but taking precautions to be safe is a wise decision. Applying sunscreen of at least SPF 15 to the body before heading outside will not only protect from burning but also decrease the risk of skin cancer. Sunscreen is useful even if you are not on a beach, but also good to wear if you are walking your dog, or simply running errands. If you and your family are at a pool or the beach during the summer do not forget to reapply the sunscreen at least every two hours. No sunscreen is waterproof, even though that term used to appear on product labels. Based on performance in lab tests, new Food and Drug Administration (FDA) guidelines now allow products to claim water (or sweat) resistance of either 40 or 80 minutes. Women who moisturize their faces daily have the added benefit of sun protection. Most products these days have at least an SPF of 15. It is still important to add another layer of protection for the face such as sunglasses and a hat. If you are spending more than 2 hours outside SPF of 30 or higher is recommended, but again remember to reapply as you can sweat if off. Limit time in the sun, especially between the hours of 10 a.m. and 2 p.m., when the sun s rays are most intense. Spending time in the sun increases the risk of skin cancer and early skin aging. However, it also provides Vitamin D. Look at expiration dates. Throw out your used bottles or sprays from last year since sunscreen does lose it s effectiveness. Enjoy the summer, soak up the sun and shake off the cold snowy winter, just remember to apply the sunscreen Page 5

Leaves of Three... By: Irene Shepherd, CPNP "Leaves of three, let them be." Great advice from my mother on avoiding poison ivy. Poison ivy is a vine whose leaves are in clusters of three. The leaves, stems, and even roots of this plant contain an oil which can cause a serious skin reaction when touched. It usually causes a red, itchy, and sometimes weepy and blistery rash on the skin. The best way to prevent poison ivy is to avoid the plant. If the oil has touched your skin, it can easily be removed with soap and water within one hour. If left on longer than this, the oil can bind to the skin and is much more difficult to remove. There are also products in the pharmacy which can remove it. Make sure you wash anything that may have touched the plant; shoes, hats, clothing, pets, etc. The oil can then get onto your skin and again cause a rash. Poison ivy is not contagious from the rash on another person. It is spread by the oil of the plant. Most mild cases of poison ivy are easy to treat at home. Cool compresses, anti-itch creams and antihistamines like Benadryl can help. In some cases you may need further care. If you experience any trouble breathing or swallowing, go directly to the Emergency Room. If you have swelling if your face and eyes or rash on large part of your body, you may need to see your nurse practitioner or doctor. Summer Pet Health and Safety By: Christina Gallardo Community Health and Dental Executive Assistant The summer months can be uncomfortable, even dangerous and deadly for pets and people. Please ensure that you and your pets are cool during the dog days of summer! To practice basic summer safety 1. NEVER leave your pets in a parked car, not even for a MINUTE! Not even with the car running and air conditioner on! On a warm day, temperatures inside a vehicle can rise rapidly to dangerous levels. For example, on an 85-degree day, the temperature inside a car with the windows opened slightly can reach 102-degrees within 10 minutes. After 30 minutes, the temperature will reach 120 degrees. Your pet can suffer irreversible organ damage or die. If you see a pet left in a hot car, please do the following: Take down the car s make, model, and license-plate number. If there is a business nearby, notify their managers or security guards, and ask them to make an announcement to find the car s owner. If the owner cannot be found, call the non-emergency number of the local police or animal control, and wait by the car for them to arrive. 2. Watch the humidity. It is important to remember that it s not just the ambient temperature, but also the humidity that can affect your pet. Animals pant to evaporate moisture from their lungs, which takes heat away from their body. If the humidity is too high, they are unable to cool themselves, which will make their body temperatures reach dangerous levels very quickly! 3. Limit exercise on hot days. Adjust intensity and duration of exercise in accordance with the temperature. On very hot days, limit exercise to early morning or evening hours, and be especially careful with pets with white-colored ears, which are more susceptible to skin cancer, and short-nosed pets, which typically have difficulty breathing. Asphalt also gets very hot, and can burn your pet s paws, so walk your dog on the grass if possible. Be sure to always carry water with you to keep your dog from dehydrating. 4. Provide ample shade and water. Any time your pet is outside, make sure he or she has protection from heat and the sun and plenty of fresh, cold water. In heat waves, add ice to the water when possible. Tree shades and tarps are ideal because they don t obstruct air flow. A doghouse does NOT provide relief from the heat, in fact, it makes it worse! 5. Watch for signs of heatstroke. Extreme temperatures can cause heatstroke in your pet. Some signs of heatstroke are heavy panting, glazed eyes, a rapid heartbeat, difficulty breathing, excessive thirst, lethargy, fever, dizziness, lack of coordination, profuse salivation, vomiting, a deep red or purple tongue, seizure, and unconsciousness. If you think your pet may be suffering a heat stroke, move your pet into the shade or an air-conditioned area. Apply ice packs or cold towels on their head, neck, and chest or run cool (not cold) water over the pet. Let them drink small amounts of cool water or lick ice cubes. Take your pet to the veterinarian right away! source humanesociety.org 6. Cool your pet inside and out. Whip up a batch of quick and easy DIY Frozen Banana/Peanut Butter Popsicles for Dogs with this easy recipe you will need the following: - source food.com - 4 cups plain yogurt - 2 tablespoons peanut butter - 3 bananas, ripe, peeled & mashed - Directions: o o o o o Blend all the ingredients to a puree in a blender/food processor Pour into 4 ounce plastic cups Freeze until firm Pop the treat out of the cup and watch your pet enjoy Store the treats in the freezer for up to two weeks (References: humanesociety.org, food.com) Page 8

C O M M U N I T Y A Thoughtful Gesture By: Paula Kamp, Community Outreach Director Lisa A. and her daughters Madelyn and Amelia (shown in picture on right) from the Phoenixville area were so kind in donating a large box of gently used books that they hand selected from their bookcases. Lisa heard about Community Health and Dental Care s Reach Out and Read program through another organization within her community and really hopes that the kids enjoy the books. When they arrived, I offered them a tour of our new site at 700 Heritage Drive where Madelyn and Amelia met Dr. Shibanova and were able to pick a prize out of the dental Treasure Chest, admire the murals in the pediatric exam rooms and meet Irene Shepherd, our Certified Pediatric Nurse Practitioner and Reach Out and Read coordinator. After the tour of our new site, the girls were ready to select the books that were going to be placed in the waiting areas at both of our locations. Lisa, Madelyn and Amelia were a great help in tagging the books and had much interest in volunteering some hours at Community Health and Dental Care in the future. It was so nice meeting with them today and I hope to see them all again soon! Community Health & Dental Care Expands Services in Pottstown Community Health & Dental Care, Pottstown, is the proud recipient of the $500,000 Community-Based Health Care Program Grant provided by the Pennsylvania Department of Health effective January 1, 2014 through June 30, 2015. Due to the overwhelming need for affordable health care, Community Health & Dental Care will be able to expand their services and open an additional site in 2014 with this funding. The new site will be located at 700 Heritage Drive, Pottstown, and will include affordable medical and dental services (with a primary focus on pediatrics), a discount pharmacy for patients, courtesy transportation program and a multi-purpose community room for educational and teen inspired programs. Since opening in 2008 with funds obtained by the Pottstown Area Health & Wellness Foundation and various contributions from local businesses, Community Health & Dental Care s mission is to identify gaps in health services and ensure access to appropriate levels of care for all people in the service area regardless of their ability to pay. Currently, over 15,000 patients are being cared for by CHDC s highly qualified Providers and support staff with more new patients registering each month. Community Health & Dental Care is committed to improving the lives of our patients and making this community a healthier place to live and work. Bridgette Madden, CEO of CHDC, states It is exciting to partner with the Department of Health and the Pottstown Health & Wellness Foundation to develop a new community-based health care center for Pottstown area and work together to improve access and services for people needing comprehensive medical care, dental services, transportation, translation, referral and case management, education, assistance in insurance enrollment in the new Federal Marketplace (www.healthcare.gov) and many more services you will only find in a community-based health center, regardless of your insurance status. Patient Reminder: In order to maintain our high quality of patient care, we are asking all patients to verify their Primary Care Physician (PCP), Insurance information and any changes that have occurred since your last visit to CHDC. In addition, email addresses are being collected for our new patient portal that will allow patients access to certain information, connect with your health care team, schedule and cancel appointments and many additional features. By: Beth Desch, RDH, PHDHP, BS Bad breath is embarrassing and may be a sign of other dental or medical problems. Halitosis, another name for bad breath, may be caused by the foods you eat. Foods with strong odors such as garlic or onions when digested enter the blood stream and are eventually carried to the lungs. The odor then may be given off when you breathe out. Patients might think that mouth wash, brushing or flossing will eliminate the odor but those remedies merely cover over the odor temporarily. The odor will only dissipate when the foods have passed through the body. Poor dental care can also cause halitosis. If you don t brush and floss daily, food particles will collect in between the teeth. The bacteria present in your mouth utilize the food remnants as nourishment, giving off toxins which may cause decay, gingivitis or gum disease. That process may result in bad breath and or a bad taste in your mouth. To alleviate this type of bad breath better home care is the answer. Also regular dental visits are necessary to maintain healthy gums and teeth. Dry Mouth or Xerostomia can also cause bad breath. Saliva is important to moisten the mouth and neutralize acids produced by plaque and wash away dead cells that accumulate on the tongue, gums, or cheeks. If not rinsed away the dried out cells decompose and can cause bad breath. Dry mouth may be caused by various medications, or other medical problems. Drinking water often to help hydrate the tissue is one solution and your CHDC dental staff may have suggestions of products you can try. In some cases bad breath may be caused by other medical conditions such as acid reflux or diabetes, which you can address with your physician. No one wants to experience halitosis but there is good news.in most cases there are simple solutions. Regular dental hygiene appointments are extremely important to main healthy teeth and gums and keep the bad breath away. Meet Jessica Garnett, Registered Dietician I started out my schooling in early childhood education and decided, after 5 years of work at a daycare, that I did not pick the right major! I chose to switch into nutrition because I LOVE food, and I believe being healthy is such an important part of life that can be achieved through diet and exercise. I transferred from MCCC to West Chester University where I completed my undergrad in nutrition and dietetics. From there I moved to Indiana where I participated in a dietetic internship/master s program through Ball State University. I decided to move back to PA because there is no place like home, and my sister, and best friend, had just had her first babies - twin boys. Someone had to teach them to jump on the couch, slide down the front steps in their sleeping bags, and eat cookies and chocolate, along with their veggies that is! I became a registered dietitian and took my first job at Einstein in Philly where I taught nutrition education in the Philadelphia School District, spent time in the teen and peds clinic, and worked in the Norristown Health Center. I decided I needed a job closer to home, and a wonderful opportunity at Creative Health and CHDC presented itself. I currently work on the Healthy Weights, Healthy Families program with CHS and I spend about ten hours a week here at CHDC. Free meals offered to all children by Pottstown Parks and Rec The Pottstown Parks and Recreation Department is participating in the Summer Food Service Program. Meals will be provided to all children ages 18 months to 18 years without charge. Acceptance and participation requirements for the program and all activities are the same for all participants regardless of race, gender or national origin. Meals will be provided beginning June 16, 2014 and ending August 29, 2014. How to Set-up an Email Account on Personal Computer Setting up a free email account is pretty quick and easy. You can view the instructions below for setting up email accounts through two of the most common providers (Google and Yahoo!)*. Go to http://mail.google.com to open your new Gmail account. Click the "Create an Account" link 2. On the Create an Account page, start by filling in the following information: First name Last name Desired login name (be sure to click on the check availability box to ensure you get the login name you want) 3. Select an 8 character minimum password. 4. Enter a security question from the drop down menu. Try to choose one that would not be easy for someone else to answer. 5. Type in the answer to your security question. 6. Select your Country location from the drop-down menu. 7. Type in the characters that you see for the word verification. 8. Review the Google Terms of Service. If you accept the terms, click on the "I accept. Create my account" button. 1. Go to the Yahoo Mail home page. 2. Click on the "Sign Up" link that appears near the top of the page. 3. Enter your name and other requested information in the fields provided. 4. Type the username you choose in the field provided.
