How To Use This Directory 1. Entries are alphabetically organized according to neighborhoods. It will be much easier if you do service close to home. Remember, it gets dark earlier in the winter. 2. Information in this directory may change, so be sure to inquire about any telephone number or fax number changes for the site you choose for service. When calling a site, ask for the Volunteer Supervisor or Director of Volunteers. 3. Note that hospitals require more than our 56 required hours. If you choose a hospital, you must follow that hospital's time requirements. 4. Ask the supervisor for a description of the work and the time when your services will be required before you make a final decision on your service site. 5. Hospitals and a few other sites require working papers. Applications are in the Nurse's Office. 6. Please notify your Religion teacher if you know there needs to be changes in the information in this directory. Student Guidelines 1. Summer Service requires at least 15 days for a total of 56 hours. This requirement must be met in the summer. 2. Sophomore Year Service requires two hours of weekly service from September to the end of May for a total of 56 hours. - If a site cannot offer you two hours weekly, then you must have two service sites. The only exception to this is CCD. - If a program does not end until June, a student must continue to do service even if she has more than 56 hours. - Since the weekly service from September to May is required, no student may stop earlier than May even if she has 56 hours before this time. 3. Service must be volunteer (unpaid), verifiable, and supervised. 4. Service time excludes travel to and from the activity. 5. If a day of service falls on a school holiday, the student is still responsible for performing the service activity unless otherwise arranged by the supervisor and student. 6. Students should not be absent from service without prior notification to the supervisor. 7. The service requirement must be completed during the summer before Sophomore year or during the Sophomore year. 8. All supervisors must be adults (age 18 years or older). 9. It is the student's responsibility to sign in/out in an attendance book kept by the supervisor in his/her office. Students are advised to keep their own record of dates and times in their agenda book. 10. Supervisors must fill out the evaluation forms mailed to them. In no case may a student fill out any part of her evaluation. SOPHOMORE SERVICE IS A REQUIREMENT FOR ALL STUDENTS AT ST. AGNES ACADEMIC HIGH SCHOOL. SERVICE IS 20% OF THE SOPHOMORE RELIGION MARK. STUDENTS MAY CONTINUE TO DO SERVICE BEYOND SOPHOMORE YEAR. A DOCUMENTATION FORM WILL BE PROVIDED TO INTERESTED STUDENTS DURING THEIR JUNIOR AND SENIOR YEARS RESPECTIVELY. 2
