WELCOME TO SANZIE HEALTHCARE SERVICES CNA CERTIFIED SCHOOL Nursing Assistant/CNA training in Georgia begins here. We aim to provide everything you need at SANZIE HEALTHCARE SERVICES, INC. Here we are focused on providing high-quality Certified Nursing training services to all of metro and East Atlanta. SANZIE HEALTHCARE SERVICES, INC. CNA Training is a state of Georgia approved comprehensive nurse aide training program that prepares students for the health care industry by providing education in the areas of basic nurse assistant training. In Georgia, all individuals who want to become certified nurse assistants must meet course requirements prior to sitting for the state certification exam which is necessary to obtain employment. Prospective students need to understand these requirements before entering into a program. In Georgia, all approved certified nurse assistant training programs must provide students with comprehensive instruction on the requirements to work in all types of licensed health care facilities including nursing homes, home health agencies, hospices, hospitals, and intermediate care facilities. Programs require a minimum of 85 hours of training which must include twenty-four (24) hours of clinical rotation in a nursing home supervised by an approved Registered Nurse or Licensed Practical Nurse. The concepts and skills must include the following topics: Communication, interpersonal skills, infection control, safety & emergency procedures, promoting residents' independence and respecting residents' rights. After the completion of the State approved training program, the candidate must take and pass the competency evaluation examination. The examination included a written/oral and skills competency examination that evaluated by an approved Registered Nurse with the approved testing agency. Candidates who successfully pass the written/oral and skills competency examination are included on the Georgia Nurse Aide Registry.

For those transferring from another state, they must not provide care in a facility until being placed in the registry. A nurse aide's employment eligibility status will be determined by the training provided by that state's instructional program. These individuals must complete the nurse aide registry out-of-state application to determine eligibility for reciprocity.

REGISTRATION: Sanzie Healthcare Services, Inc. is excited that you chose to start your career as a Health Care professional with us. We are dedicated to giving up to date training with excellent customer service being our focal point. Enrollment in Sanzie Healthcare Services, Inc. CNA Training begins with completing the contract as well as the assessment page. After completing the assessment page you will receive a call from one of our enrollment specialist who will be assigned to you through your registration process. Registration involves a phone and/or in person interview, submitting the required documentation, receiving an acceptance or denial letter and payment. Below you will find all of the required documentation that is required to begin the process. Again we recognize the fact that there are many nursing assistant training programs to choose from and to every student that we are privileged to serve, WE THANK YOU!

ADMISSION REQUIREMENTS TB skin test from local health department within 1 year Must be at least 18 year of age High school graduate and diploma or have satisfied the requirements through the General Education Development (GED) test 2 forms of Identification, one (1) must be a valid Social Security Card and one (1) must be government issued picture identification (ID) card A criminal background check, (this can be done at the police station). This is required by law; some convictions may prevent a student from completing the course, or from becoming certified after completing the course

ATTENDANCE Absences - More than two unexcused absences result in immediate failure in the program. Due to the critical nature of the Nurse Aide training program, none of the material presented should be missed. - Absentee must make arrangement with instructor and get approval to make up their work. Administration or instructor will not be able to let student back in class otherwise - Material missed in class must be made up by the second week of class. - The Georgia State Law requires that a student must have at least 85 hours class/theory/lab and 24 of those hours must be clinical experience. One missed clinical would result in immediate failure from clinical rotation. - Emergency Absences would be handled on an individual bases. - Tardy Two tardiness will equal one absence - Arrival to class 10 minutes after class starts is considered late

CODE OF CONDUCT: The Code of Conduct is the professional standards for conduct, attitude, performance and ethics while enrolled in a nurse aide training program including the testing for the State competency examination. The Code of Conduct for Sanzie Healthcare Services, Inc. CNA School would include: a. Appropriate Dress Code including name tag identification and nonskid footwear; b. Appropriate interaction with instructors, coworkers, residents, etc. c. No personal cell phone usage during class, lab or clinical rotation; d. Disruptive behavior or inappropriate language such as profanity or inappropriate discussion of personal problems during any phase of training and testing is not permitted; e. All Students would display professionalism and self-control at all times; f. All Students would perform no task for which the trainee or student has not received training; e.g. Practice all safety precautions taught for each lab skill; h. All students will report appropriate information such as changes or health status in resident to the instructor and/or supervisor; i.e. students must report all concerns immediately to the clinical instructor first, or a nursing home supervisor per nursing home policy. I. All students would follow HIPAA Guidelines at all times. HIPAA violations may result in dismissal from program and may prevent placement on the Georgia Nurse Aide Registry; j. All Students would respond to emergencies as directed by a Licensed Professional and within the Training Guidelines. k. Cheating is prohibited and will result in the immediate dismissal from the nurse aide course;

DRESS CODE FOR SANZIE HEALTHCARE SCHOOL Clean scrubs of any color chosen by the school, or clean white uniform are required for all classes. Uniform or any color chosen by the class (to be purchased by the student) - Uniform consists of: Uniform pants and top White shoes with enclosed heel and toes All clothing s must fit properly. Overly tight scrubs, or scrubs that are too loose, are not permitted. No visible body piercing/tattoos Hair should be pulled back, or pinned up. Nails should be short, smooth, and clean with no polish. No longer than 1/8 inches. No Artificial nails. Jewelry is limited to your wedding band, a clinical watch with a second hand, and earrings are limited to one pair of stud (post) earrings no longer than 3mm.

DRESS CODE FOR SANZIE HEALTHCARE SERVICES CNA SCHOOL CLINICALS 1. White uniforms (scrubs), white tennis or nursing shoes and white socks or hose. Skirt or dress hems should be just below the knees. No sleeveless attire, mini or low-cut tops. No shorts. 2. Nails, hairs and Jewelry guidelines are the same as for the school. 3. Bathe daily and use deodorant. No perfumes or cologne will be allowed. 4. No personal telephone calls are to be made or taken without permission from the instructor. 5. Student may not leave their assigned unity without permission from instructor. 6. If granted break time, this is to be taken in the employee break area. There are snack and drink machines for your use. 7. NO ONE is to take snacks from the residents snack area. 8. You may bring a bag lunch to clinical. It may be left in the refrigerator in the employee break room. 9. All Students must wear name badge in a visible location to identify that you are a Nursing Assistant student and not an employee. 10. You may not be in the building unless your instructor is present and aware you are there. You may not leave the facility without the permission of your instructor. 11. All students are expected to conduct themselves in a professional, dignified manner at all times.

12. Lunch is 30 minutes and must be taken at the facility. Lunch is to be eaten in the employee break room. 13. Smoking is not permitted anywhere in the facility. There is one designated area for smoking for employees/students. Smoking is allowed only during lunch or granted break. 14. Facility phones may not be used for personal calls without the express permission of the instructor. 15. You may not have personal visitors while at the facility. 16. No gambling, alcoholic beverages, drugs or weapons (including firearms and knives) are allowed. 17. Report any unusual or threatening behavior by anyone to your instructor and/or the Director of Nurses (DON) or Facility Administrator. 18. Check all at risk residents and their areas at least every 2 hours for ant activity. There should be no open food containers in the residents rooms. All left over food from meals or snacks is to be removed from the room and disposed of properly. All residents who are fed are to have their clothing/bed linen checked and cleaned if necessary after meals.

TUITION AND FEES: Program cost Breakdown $100 registration and application fee is required and nonrefundable Tuition fee: $600 Books: $50 (Purchased here at Sanzie Healthcare Services, Inc. CNA School) Uniform: $40 (covered by students) Total Tuition fee: $750 except for uniforms, which are covered by students. * Any remaining balance after registration and application fee is paid will be due in 3 equal payments 1. First Day of Class 2. Second Monday 3. Third Monday (TUITION MUST BE PAID IN FULL BY THIRD WEEK OF CLASS BEFORE CLINICALS)

Payment methods accepted: Debit/Credit Card Money Orders/Checks Cash State Certification Exam: The State Certification Exam fee ($107) shall be paid to NACES separately by money order due 3 rd week of Class The State Certification Exam fee has to be submitted by student.

REFUNDS No refunds will be given for the application and registration fee ($100.00) A full (100%) refund will be given if the student is not accepted by the school. All tuition paid through AE/MC/VISA/DISCOVER will be refunded through the applicable charge account. A full (100%) refund of tuition and fees excepts the application and registration fee will be allowed of the applicant withdraws within three days after signing the contract or making an initial payment, provided that the applicant has not started training. All refunds will be made within thirty (30) days of termination. A full (100%) refund of tuition and fees paid will be paid if the school discontinues a course of program during a period of time within which student could reasonably have completed the same. After student has begun training: Cost for books, application fee and registration cost are not refundable.

TUITION: 1. For an applicant requesting cancellation more than three business days after signing the contract or for a student completing no more than 5% of the instructional time allotted for the program, a 95% refund of tuition fees, less cancellation fee (assessed by the school) will be given. 2. 10% or less of the instructional time allotted for the program, a 90% refund of tuition fees, less cancellation fee (assessed by the school) will be given. 3. Greater than 10% but less than 25% of instructional time completed, a 75% refund of tuition fees, less the cancellation fee (assessed by the school) will be given. 4. Greater than 25% but less than 50% of instructional time completed, a 50% of tuition fees, less the cancellation fee (assessed by the school) will be given. 5. Greater than 50% of instructional time completed, no refund of tuition fees. Instructional time completed for the purpose of calculating refunds is defined as the time between the start date and the date an official student withdrawal request form is received by the school.

The official date of termination or withdrawal of a student shall be determined in the following manner: 1. The date on which the school received notice of the student s intention to discontinue the training program; or 2. The date on which the student violates published school rules and/or policy which provides suspension 3. All refunds must be calculated from the last date of recorded attendance. 4. Missed clinical rotation would result in failure to complete class, and refund would not be awarded.

PHYSICALS: (Second week of class prior to Clinical Rotation) All physicals MUST be up to date (within 1 year) If no physical is on file by the second week of class. The student would not attend clinical rotation. This would result in failure to complete course requirement. - Up to date TB screening or chest x-ray (within 1 year) from local Health Department or private physician - Hepatitis B Vaccine or signed waiver (vaccine may be obtained at local Health Dept. or private physician), waiver can be signed at the school. CLASS SCHEDULE: Day and Evening classes are available Monday Thursday (Days): 9:00-4:30 Monday-Thursday (Evenings): 6:00pm-10:15pm Clinical At the Clinical site/healthcare facility -Fridays, Saturdays, Sundays 7am-3pm and/or 2pm-10pm In case, time schedule is changeable. SUPPLIES: Please bring your own supplies (pen/pencil and notebooks).

CURRICULUM: The first 16 hours of courses and class time will consist of: Communication and interpersonal skills; Infection Control; Safety/emergency procedures/heimlich maneuver Promoting resident s independence; Respecting resident s Rights Additionally students will get training as follows; Basic nursing related skills Personal care skills, including Activities of Daily Living (ADL s) (i.e. bathing, grooming, oral care, shaving, bed making, vital signs, height & weights, feeding, physical disabilities). Mental health and social service needs Care of cognitively impaired residents Basic restorative services Resident s Rights 24 hours practical training at the clinical site; the trainee, the student will demonstrate knowledge and proper techniques under supervised training. Student shall follow healthcare facility shift schedule and keep the facility s rules and regulations with respectful. To pass, twenty four (24) hours of clinical practice at the healthcare facility is required to graduate from the school. A Returned Demonstration on Skills Checklist must be 100% in order to pass the course.

Ed GRADUATION REQUIREMENTS: Students must complete all required subjects or course in its entirety, have a cumulative GPA of 2.0 or higher, and meet all required standards for the program in order to be eligible for graduation. A Certificate of Completion would be awarded to those students who meet all the eligible requirement mentioned above. GRADING SCALE Grade % A 90-100 B 80-89 C 71-79 F Below 70 After the completion of the state approved training program, the student must take and pass the competency evaluation examination. The examination includes a written /oral and skills competency examination that is evaluated by an approved Registered Nurse with the approved testing agency. Students who successfully pass the written/ oral and skills competency examination are included on the Georgia Nurse Aide Registry.
