Position Description Position Details: Title: Nurse Specialist Thromboembolism Unit Reports Professionally to: Nurse Unit Manager Haematology Date: June 2017 Position Purpose: The Nurse Specialist is an advanced nursing practice role and is responsible for providing specialist nursing care and/or coordination of care to a specific patient / client population across health care settings. As a senior nurse, this role also has a responsibility for improving the quality of professional nursing practice and safe guarding high standards of care in ADHB by supporting an environment in which excellence in clinical care can flourish. Description: Needs to include: The Clinical Nurse Specialist (CNS) in the Thromboembolism Unit will co-ordinate and manage the provision of care to patients with thromboembolic disease, including those participating in Clinical Trials. The Nurse Specialist facilitates the established clinical pathway used by medical teams in the acute setting to streamline the investigation and management of patients admitted with a suspected DVT/ PE to reduce length of stay and optimise care. The role manages after-hours discharge of patients from the Emergency Department (ED) and Assessment and Planning Unit (APU) in a timely manner to ensure patient flow within the acute care setting. This role is pivotal in enhancing the relationship between primary, secondary and tertiary healthcare services in the diagnosis and treatment of Venous Thromboembolis (VTE) in greater Auckland. This allows patient treatment and contact to be optimised across the Cancer and Blood HSG. Specifically: Administration of the outpatient care programme Commencement and control of anticoagulation therapy Liaison with community agencies and General Practitioners Provision of patient information and education This role will be actively involved in ongoing research into the management and treatment of VTE through international clinical trials involving extensive specialist nursing input to monitor the safety and efficacy parameters, Our vision for the future is to provide a quality healthcare service that responds to and meets the needs of our community. Patient care management / care coordination Provides direct care and uses advanced nursing knowledge and skills to assess, plan, implement and evaluate patients / clients / family / whanau health needs. Facilitates effective discharge planning and/or referral processes to health care providers and support agencies to meet identified health needs. Monitors clinical outcomes / indicators and manages variance to clinical pathway, protocols or best practice guidelines. Educates and provides information to patients / clients / family / whanau to improve knowledge of disease / illness, self management, prevention of complications and promotion of recovery. Identifies barriers and solutions to access for patient / client, family or whanau. Accurately documents assessments of patients / clients health status, diagnosis and decisions made regarding interventions and referrals or follow-up. Participates in case review and debriefing activities as required.
