Reducing Disparities in Breastfeeding through Peer and Professional Lactation Support Funding provided by the Centers for Disease Control and Prevention (CDC) Funding for a 6-month community-level, peer and professional support project, provided federal funding availability Purpose Increase implementation of evidence-based breastfeeding programs, practices, and services at the community level. Focus on providing peer and professional lactation support to breastfeeding mothers in predominantly African American and underserved communities Develop and maintain public health partnerships critical to building community support for breastfeeding. Goal Increase breastfeeding initiation, duration, and exclusivity among African American and underserved populations in the U.S.
Breastfeeding Support Network Activities Identify LHDs that can implement breastfeeding support activities in partnership with local EMPower hospital(s) Provide financial, training, and technical assistance resources to support LHD and CBO efforts to develop or enhance peer and professional lactation services, expertise, and capacity Engage state and local breastfeeding coalitions as a part of the local or community-level partnerships Collaborate with national partners to support project activities Evaluate process and reach of community-level peer and professional lactation support Disseminate information about peer and professional lactation support activities

Eligibility Invited LHDs who have an EMPower Hospital within their service jurisdiction and willing to partner with the EMPower Hospital to provide community support breastfeeding services. The LHD may defer this application to a designee, including: Other, non-lhd, governmental entities Community-based organizations or non-profits (e.g., coalitions or task forces) Faith-based organizations (e.g., churches and other places of worship) Tribal governments and organizations Community Health Centers, hospitals, and health clinics Serving predominantly African American or Underserved Populations At least 50% of population is African American or Underserved Underserved populations may include low income (as defined by Medicaid Eligibility) or other Racial/Ethnic minority groups (e.g. Native American, etc.)
Contract Terms No modifications to the terms or contract language will be made. LHD applicants that cannot agree to NACCHO s standard contract language should NOT apply for this initiative. LHDs can identify a designee. NACCHO s standard contract language is provided in the RFA Announcement. NACCHO will reimburse grantees, bi-monthly (i.e., every other month), upon receipt of deliverables and invoices outlining approved project expenses.
Program Expectations Attend two regional grantee meetings (January 2016 and April 2016) Develop and refine a 6-month work plan to provide lactation support for African American and underserved women. Participate in assessments to determine training and technical assistance needs of the organization and staff. Participate in monthly conference calls, project-related webinars, and other activities to support Breastfeeding Project activities and capacity-building in the local organization.
Program Expectations Implement peer or professional lactation support activities, as defined in the project work plan. Collect individual participant data to facilitate understanding of program activity reach. Report aggregate data related to services provided, including but not limited to: number of women served, by race and age; number of women reached, by race and age; and number of referrals or connections to local resources. Participate in project-related evaluation activities, including pre-post assessments, focus groups, and other identified evaluation activities.
NACCHO Support Provide background information related to the project, including access to NACCHO reports, data, and other resources necessary to complete the tasks above. Support an online community of practice portal to disseminate data and information to the project sites. Provide direct technical assistance for completion of tasks, including periodic webinars and phone or e-mail consultations. Provide tools, guidance, and assistance to support organizational capacity to collect and report project data and evaluate project activities. Assist in the development of sustainability plans for project activities.

Development and Distribution of Community Resource Guide Development of a Referral Network for Community Lactation Support Breastfeeding Clinics (e.g., Baby Cafes) Pediatric/Family Provider outreach/partnership to support breastfeeding Inpatient or Outpatient lactation support services provided in a hospital or clinic setting Staff training to improve or enhance skills of current staff in lactation care, counseling, and case management in a community setting Local Breastfeeding Coalition or Advisory Board development and planning

Organizational Capacity and Experience Describe your organization mission and structure, and explain how your organization s efforts align with the aims of this project Describe your organizational/agency and staff qualifications and experience engaging in breastfeeding promotion efforts that involve: Implementing evidence-based and innovative breastfeeding programs, practices, and services Developing and maintaining public health partnerships for breastfeeding. Identify key staff responsible for completing your proposed work and provide sufficient detail to demonstrate knowledge, skills, and abilities to perform the functions outlined in the RFA Describe your organization s capacity to collect or obtain data regarding services provided and women served
E. Project Work Plan and Timeline Applicants must prepare a detailed 6-month work plan for the entire project period Work plans should not exceed 4 pages Work plans should be submitted using the template provided in the RFA Work plan objectives/activities should clearly indicate a partnership between the LHD and/or Designee and address how the partnership benefits the organizations and the community served.
Examples of Breastfeeding Support Activities The community-level breastfeeding supports previously identified for Step 3 and Step 10 provide the full range of examples and are inclusive of all activities that can be implemented as part of this funding opportunity. While applicants can combine examples of activities (e.g., do a gap analysis in addition to another activity), applicants must include the aforementioned activities which speak to Steps 3 and 10 of the Ten Steps to Successful Breastfeeding. Applicants should identify those appropriate activities and strategies to help them meet the breastfeeding support needs of their community, as identified in their problem statement.

This agency is authorized to commence project activities upon notification of award.

Attachments (3 of 3) Letters of Support (REQUIRED) Provide one letter of support, on official letterhead, from an EMPower hospital(s) to demonstrate that your agency has the necessary partnership to fully engage in the Breastfeeding Project. If the LHD identifies a designee to submit the application, please provide one letter of support, on official letterhead, from your local health officer indicating why the designee is most appropriately positioned to engage the Breastfeeding Project and outlining the partnership between the LHD and the designee organization.
