I certify that I may legally possess firearms and will not use any skill acquired in Central Carolinas Shooting Club activities for any illegal purpose. B. I understand that defensive pistol shooting activities constitute a potentially serious physical hazard and I voluntarily assume these risks. C. I agree to hold harmless Central Carolinas Shooting Club and its officers and directors of any liability for accidents caused by my participating in this activity. D. If under the age of 21 I understand that I must be accompanied by a parent or legal guardian while participating is shooting activities. E. By signing this document I certify that I am not a felon and I am allowed by law to use firearms. One of the following must accompany this application: 1. A copy of your current Conceal Carry Permit; issued by South Carolina, or a state that required a background check before issuing a Conceal Carry Permit. 2. The original copy of a certified national background check, performed by your state or county law enforcement agency. (A SLED Catch is not acceptable) 3. A Sheriff-issued, Handgun Purchase Permit which is current and valid. 4. A Military ID for an applicant currently on active duty with the US Armed Forces. 5. Active Law Enforcement credentials from an accredited local, state, or national law enforcement agency. 6. Items 1-4, above, apply to all adult (per South Carolina firearms statutes) members on a family application. Parents (guardians) are responsible to ensure that all included minor family members are legally eligible to participate in shooting sports. F. CENTRAL CAROLINAS SHOOTING CLUB (CCSC) RELEASE, WAIVER, INDEMNIFICATION, HOLD HARMLESS AND ASSUMPTION OF THE RISK AG REEMENT WHEREAS, in return for instruction in firearms, use of premises, and for other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, the Undersigned agrees to the following: The Undersigned agrees to indemnify, hold harmless and defend the Central Carolinas Shooting Club and its elected officers, from any and all fault, liabilities, costs, expenses, claims, demands or lawsuits arising out of, related to or connected with: the discharge of firearms; the Undersigned's participation in the shooting activities, the range, buildings, land and premises used for the shooting activities (hereinafter the "Premises"); the Undersigned's presence on or use of said Premises; and any and all acts or omissions of the Undersigned. And should any such claim, demand or lawsuit arise or be asserted in any way whatsoever related thereto, whether arising under the laws of the United States or of any State, or under any theory of law or equity, the Undersigned will indemnify, hold harmless and defend the CCSC, from any and all costs, expenses or liability including, but not limited to, the cost of any settlement or judgment made or rendered against the CCSC, whether individually, jointly, or in solido with the Undersigned, together with all costs of court and other costs or expenses incurred in connection with any such claim, demand or lawsuit, including attorney's fees. The Undersigned furthermore waives for himself/herself and his/her executors, administrators, assignees or heirs, any and all rights and claims for damages, losses, demands and any other actions whatsoever, which he/she may have or which may arise against the CCSC, (including, but not limited to any and all injuries, damages or illnesses suffered by the Undersigned or the Undersigned's property), which may, in any way whatsoever, arise out of, be related to or be connected with: the shooting activities; the Premises, including any latent defect in the Premises; the Undersigned's presence on or use of said Premises; the Undersigned's property (whether or not entrusted to the CCSC); and the discharge of firearms. The CCSC shall not be liable for, and the Undersigned, on behalf of himself/herself and his/her executors, administrators, assignees or heirs, hereby expressly releases the CCSC from any and all such claims. The Undersigned hereby expressly assumes the risk of entering the Premises and of taking part in activities on the Premises which include, but are not limited to, instruction in the use of firearms, the discharge of firearms and the firing of live ammunition. The Undersigned furthermore hereby acknowledges and agrees that he/she has read, understands and will at all times abide by all CCSC range rules and procedures. Central Carolinas Shooting Club Membership Policy Membership in the Central Carolina Shooting Club shall be consistent with Article III, section (a) of the club s bylaws. They state that: Any person, who may legally own firearms or use them under the supervision of a parent or guardian, may become a member of this organization on vote of the Board of Governors after subscribing to the following pledge and on payment of the usual application fees and dues: The Pledge: I certify that I am a person of good repute, that I am not a member of any organization or group having as its purpose or one of its purposes the overthrow by force and violence of the Government of the United States or any of its political subdivisions; that I have never been convicted of a crime of violence; and that, if admitted to membership, I will fulfill the obligations of good sportsmanship and good citizenship. Application Process 1. Application Form To initiate the membership process, a current copy of the Central Carolinas Shooting Club Membership Application must be completely filled out, signed and dated. The application form must be accompanied by: a. A check for the first years dues (see section 2) b. A certified background check (see section 3) c. A signed and dated Waiver of Liability Form These items may be submitted to any club officer or appointed representative for consideration and processing. 2. Membership fees Annual membership fees are established by the Board of Governors. Current membership fees will be noted on the Membership Application Form. New member applicant s 1 st year s membership fee will need to be submitted with application via CCSC's Paypal site. If membership is not approved, it will be returned to the applicant. 3. Background Check A current, certified background check for all adult applicants (per South Carolina firearm statutes) must accompany the application. There are four options available to meet this requirement: a. A copy of your current Conceal Carry Permit; issued by South Carolina, or a state that required a background check before issuing a Conceal Carry Permit. b. A notarized, original copy of a certified national criminal history obtained from the Clerk of Court in the county in which you reside, at your expense. (SLED Catch is not acceptable) c. A Sheriff-issued, Handgun Purchase Permit which is current and valid. d. A Military ID for an applicant currently on active duty with the US Armed Forces. e. Current Law Enforcement credentials from an accredited national, state, or local law enforcement agency. Note: For Family memberships, each adult family member listed on the application must submit an individual background check. Parents (guardians) are responsible to ensure that all included minor family members are legally eligible to participate in shooting sports. Privacy Policy:

All personal information provided in the application process will be protected. It will be reviewed only by the board of governors to verify suitability for membership. No part of the personal information provided will be given to anyone outside the board of governors for any reason except legal court order. All background information, such as copy of CWP permit or copy of background check shall be returned to the applicant once the membership determination is made. The ID number of the submitted document will be recorded & retained in secured club records, but no copies of the specific documents will be retained. 4. Review by the Board of Governors a. All applications for membership are contingent on approval of the Central Carolinas Shooting Club Board of Governors. b. The Board will meet to review membership applications at least once per quarter. c. The time between the submission of your application for membership, and approval/rejection by the Board will be a Probationary period 5. Probationary Period During the probationary period (the time between the submission of your membership application and approval/rejection by the Board) the applicant: a. May participate in club activities b. May not vote in club business c. Is expected to participate in at least three (3) club shooting events 6. Application Approval If an Application for Membership is approved: a. The applicant is now a Member with the full benefits of club membership b. The Secretary will issue a membership card to the applicant c. All copies of background information submitted with the application will be returned 7. Application Rejected If an Application for Membership is rejected: a. A member (or members) of the board of Governors will contact the applicant b. The Board Member will verbally inform the applicant that their application has been rejected c. A reason for rejection does not need to be given. d. There is no appeal process. The decision of the Board is final. e. 1st year membership dues will be returned f. All copies of background information submitted with the application will be returned Benefits of Membership As of March, 2013, some of the benefits of membership in the Central Carolinas Shooting Club include: 1. Discounted participation fees at club matches 2. Participation in Tuesday-night practice at Sportsman indoor range (individuals may participate a maximum of three (3) times as a guest of the club) 3. Discounted membership at the Sportsman 4. Voting rights at the annual club meeting and any special meetings
