Transforming Health Through Innovative Ideas and Partnerships RFP Release Date: September 4, 2018 bi3, Bethesda Inc. s grantmaking initiative to transform health in Greater Cincinnati, is teaming up with TriHealth s Innovation Center to seek funding requests for planning, implementing and/or scaling new ideas and approaches to improve health and health care. We are particularly interested in ideas from TriHealth team members that have the potential to lead to break-through change in population health, behavioral health, integrated care models and programs that address barriers and disparities for underserved people in the Greater Cincinnati. The submission deadline is November 9 by 5 pm EST. This is a multi-stage, competitive process. An invitation to submit a proposal does not guarantee a grant award. Funding Focus bi3 invests in promising projects and initiatives to spark systems and culture change, drive improved outcomes for patients and the community and create fundamental shifts in health and healthcare. Successful proposals will be those that offer innovative approaches to achieving marked improvement for defined populations. bi3 is particularly interested in projects in the following two areas: Behavioral health We seek to promote mental health and well-being for all including building resilience. We are particularly interested in efforts that seek to raise awareness and mitigate the impact of adverse childhood experiences (ACEs) and trauma and build the capacity of the behavioral health work force to address community needs now and into the future. Health innovations We seek to enable new technologies to improve patient access to care, accelerate innovative approaches and the integration of care to advance the healthcare system s movement towards a population health approach, particularly for patients receiving Medicaid or who are uninsured. 1

We seek projects that will improve the health and social outcomes for patients with complex care needs and address health disparities for underserved populations. bi3 will also consider proposals in its other two priority areas: Maternal and infant health We seek projects that can lead to positive health and social outcomes for mothers and young children including building a strong foundation for children s healthy development and addressing the social determinants of health. Palliative and hospice care We seek to address the needs of the seriously ill and those with chronic, life limiting illness. We aim to educate and empower patients and their families to make informed treatment decisions. Priority will be given to requests for support that: prevent adverse health effects and/or promote healthy behavioral change; create new, sustainable models of care; focus on the Triple Aim with an emphasis on population health; integrate medical and community resources to address the social determinants of health and health equity; involve strong leaders, champions and community partners; drive cross-sector collaboration; engage community members and patients in the design and refinement of the project; measure and evaluate improved health outcomes; leverage other resources and dollars; and emphasize learning and continuous improvement and share lessons learned. Types of Grants and Use of Funds bi3 awards three types of grants: 1. Planning Grants may include support for formal research, needs assessment and the identification of best practices to develop an implementation plan and/or a written business plan to guide future efforts. Planning grants are typically one year and range from $150,000-$300,000. 2. Implementation grants are for the initiation of a program that has the potential to transform health; an idea that is fully developed and ready to be launched but requires additional resources to pilot, execute and build toward sustainability. Implementation grants are typically three years and range from $1.5-$3 million. 3. Expansion grants are designed to bring tested projects to scale. Proposed expansion projects should have established evidence of success and be ready to expand promising practices to new geographies or 2

populations. Projects should have an articulated sustainability plan to ensure adequate resources are in place after the grant funding ends. Expansion grant awards are typically two years and range from $1- $2 million. Criteria bi3 partners with grant recipients and provides resources throughout the life of the grant. bi3 will invest in grantees that demonstrate the capacity to deliver successful programs and services, engage in evaluation, have strong management teams, and are fiscally sound and financially diverse. Funded partners will meet all grant monitoring and reporting requirements. In turn, bi3 will provide support and technical assistance to evaluate and communicate project results and lessons learned. As an invested partner, bi3 will work collaboratively with grantees to develop sustainability plans, address unexpected risks, and refresh performance plans based on interim results. bi3 does not fund capital campaigns or general operating expenses although reasonable administrative costs may be included as a percentage of the project budget. bi3 will not fund health care levies, lobbying or political campaigns, emergency requests, scholarships, endowments or grants to individuals. Three-Step Application Process bi3 will follow a three-step application process: Big Idea Submission, presentation at our Ideas Expo and final submission of a Detailed Proposal. Respond to this Request for Proposals (RFP) by submitting a Big Idea via the Innovation Center s platform http://trihealth.ideascale.com/ by November 9, 2018 by 5 pm. Big Idea Submission The content of the Big Idea submission should address the following questions: a) Create a title for your idea b) Describe your idea c) What is the opportunity or problem, including the targeted population? d) What is the requested grant type (e.g. planning, implementation, expansion)? e) What is the requested time period of the grant (e.g. one, two or three years)? f) What is the requested grant amount? g) Which of the following bi3 funding priorities does your project fall within? (Maternal and Infant Health, Behavioral Health, Palliative and Hospice Care, Health Innovation) h) How does the project align with the selected bi3 funding priority? i) How does the project advance TriHealth s Strategic Goals? j) What makes this project innovative? 3

k) How will this project lead to breakthrough change in health or healthcare versus incremental change? l) A brief Case Statement (2-3 sentences) for why bi3 should invest in the project m) The name, title and contact information for the lead applicant n) What business unit does your project impact? TriHealth team members will be instructed to submit their Big Ideas on the TriHealth Innovation Center platform for peer review and voting through a focused ideation process called crowdsourcing. There will be an initial screening of submissions prior to a Big Idea moving to crowdsourcing. It may be determined that some submissions do not meet the criteria for Big Ideas and may be referred to Bright Ideas. Crowdsourcing results will be shared with the bi3 Review Team as part of the review process. Applicants will receive a response to their Big Idea Submission on or before January 11, 2018. Ideas Expo Selected Big Idea applicants will be invited to give a 5-minute pitch at the bi3 Ideas Expo to be held on the week of February 4, 2019, date to be scheduled. Applicants will receive coaching from bi3 s communications council in preparation for the Ideas Expo. Selected applicants will receive an invitation to submit a detailed proposal by February 15, 2019. Detailed Proposal Applicants invited to submit a Detailed Proposal are expected to attend a RFP workshop on Wednesday February 20, 2019. The workshop will provide more information about the application and give each applicant the opportunity to receive coaching and feedback from bi3 and Innovation Center staff. Full Proposals will be submitted via bi3 s online grants management system by April 26, 2019 at 5p.m. Final grant awards will be announced in July 2019.
