Senior services Senior Navigator Senior Navigator is a free telephone information, referral and assistance center that provides advice about a range of elder care issues. Nurses and social workers can refer you to appropriate medical and community resources. They have information about options and eligibility for financial assistance for long-term care and community agencies that offer counseling, transportation, housing and legal services. Circle of Care You may choose to contract for the services of a geriatric care manager (sometimes called aging life specialists ) from the Circle of Care Program. They are experts in helping families manage the daily challenges faced by older adults, especially with physical or memory issues. Adult children often find that a geriatric care manager can relieve the burden of arranging services and monitoring their loved one s safety and well-being. Expert guidance leads families to decisions that ensure quality care and an optimal life for those they love. Your geriatric care manager will conduct a thorough in-person evaluation, lay out a clear plan and coordinate the services that you and your family need. Geriatric care managers know how to get things done and will take care of all the details. They will coordinate with family members, doctors and other professionals, service providers from home care, and community agencies. Hospice services Hospice services Hospice care provides an array of services to people with advanced illnesses, as well as their families, so that they may remain in the comfort of their own home or nursing home. If a patient needs more advanced symptom management, inpatient care is available in a hospice facility, where staff members are available 24/7 to respond to questions or concerns. We accept most insurance plans, including Medicare and Medicaid.
