WaterBreak Ministries Scholarship Application 2018-2019 108 North 26 th Ave Lanett, AL 36863 334 425 0548, http://www.waterbreakministries.com WaterBreak has been generously endowed by WaterBreak members who have, through gifts and memorials, established a variety of scholarship funds that provide financial resources to be awarded to full-time undergraduate students each year. Application Procedure 1. Applications will become available at the beginning of April during the year of application. Applications may be picked up from the Athletic Director in his/her school or may be acquired from the website. 2. Completed applications, current school transcript(s), a letter of recommendation and a photo must be returned to the Scholarship Committee at the office by April 5th. Please check with WaterBreak s office to verify that your application has been received. It s preferred that applications be typed. Additional paper may be used if more room is needed for any responses. It s most important that your responses are detailed and complete to help the Scholarship Committee better know you. Any questions should be directed to the Scholarship Committee chairperson who is currently Mitch Hamilton. 3. After reviewing all applications, the Scholarship Committee will contact all applicants by mail. The scholarship recipients will be invited to the presentation of awards during the winner s school awards day. Recipients are expected to attend this presentation. If unable to attend, the student must contact the chairperson of the Scholarship Committee prior to the awards service. 4. This application is for one academic school year; however a student may reapply in following years if qualifications are met. Students may only receive a total of four scholarships. Restrictions/Guidelines 1. Winner has one year to utilize the funds 2. Awards will be made payable to the accredited college or university of higher education only (not to any third party, including the scholarship recipient), as first designated by the award recipient. Once scholarship funds are sent to the college or university designated by the award recipient, WaterBreak cannot direct those funds to another college or university. Award, when combined with other scholarship funds of recipient from other sources, shall not exceed 100% of the cost of tuition, fees, and required textbooks. 3. Award recipient is solely responsible for providing WaterBreak with appropriate information necessary to process the award and credit the recipient s account at the appropriate financial aid office. Recipients may be required to verify enrollment and other information, as directed. 4. If recipient withdraws from enrollment during the course of a term, recipient may be required to reimburse WaterBreak for unused awards. 5. Awards will only be not applied toward courses the student has not yet attended. Awards will be applied to past due balances. 6. Awards are not assignable or transferable to any other person. 7. Award recipients are full responsible for all applicable federal, state and local taxes and fees. 8. This award may affect the amount of other financial aid for which a recipient may qualify, or the award may be reduced or nullified by other educational assistance and aid the recipient receives Communication A letter of congratulations will be sent to the winning applicants along with any instructions concerning required documentation and/or disbursal of award funds. Publicity WaterBreak Ministries scholarship recipient may be asked for a short interview and photograph to announce their award in the WaterBreak newsletter, website and possibly other public venues.
