Click on Application forms for General Nursing and Midwifery Examinations Pdf Format (Legal Size) on the Home Page 3. Download the prescribed New application form and take a print out. Guide the students in filling-up prescribed printed forms. Receive the filled in applications along with the prescribed enclosures from students. 4. Any application without the following particulars are liable for rejection. Further, they should follow the following instructions scrupulously. 1) All the applications should have the enclosures as noted in the application form.
