JOB DESCRIPTION CLINICAL NURSE MANAGER Reports To: FLSA Status: Director of Private Duty Non-Exempt General Summary: Responsible for the management, supervision, direction, coordination and provision of high quality patient care; for maintaining the highest standards of care at the lowest possible cost; and for ensuring compliance with Company policies, procedures, and federal/state regulatory requirements. Essential Functions: 1. Client Intake, Admission and Ongoing Provision of Care a. Completes referral intake b. Completes admission of new clients to Private Duty services c. Determines payer sources (private pay, auto, workers comp, long term care); verifies what payer source is primary and if client has coordinated benefit with health insurance d. Provides client evaluation to determine clients needs for services and potential risks e. Secures order for services and verifies insurance coverage as applicable f. Consults with Director to set prices for billing and staff wage differential if applicable g. Obtains signature of client/poa on required documents (contract, privacy rights, etc.) h. Determines deposit amount for private pay clients and obtains payment on admission i. Completes non-skilled (nurse aide) or skilled (nurse) plan of care (POC) as appropriate; updates POC throughout duration of care as needed and informs field staff of changes j. Reviews clinical documentation for compliance with the POC k. Conducts supervisory visits (private pay clients every three months and as needed; Insurance clients every thirty days and as often as clients condition requires) to assess client s status, needs and necessary changes to the POC l. Re-assesses client after return from hospital or rehab and updates POC. m. Coordinates care with case managers, families, physicians, other health care providers n. Communicates changes in client s condition, plan of care revisions, medical supply and other health care needs to case manager, POA/Guardian o. Ensures client certification is current and obtains order for recertification as appropriate p. Assists Schedule Coordinator with choosing appropriate staff for cases; informs Coordinator of schedule changes; assists with staffing when warranted; notifies field staff if taken off case by client q. Provides skilled nursing services to clients as needed r. Resolves concerns/problems with client care, documents any incidents and report to Director as appropriate s. 2. Supervision of staff a. Conducts supervisory visits to assess staff compliance with the client s POC; observe staff for required skills and upkeep of client s living area as well as interaction with client/family b. Ensures staff ability to safely perform client s care c. Provides feedback to staff regarding performance; recognizes exemplary performance and develops plan of performance improvement with staff as needed d. Coach field staff regarding policy/procedure compliance e. Completes investigation into employee policy/procedure violations, completes documentation, and discuss with Director for appropriate action f. Conducts the Disciplinary Action session and based on the type of Disciplinary action or progression of Disciplinary Action, arrange for the Director of Private Duty or Human Resources to be present either in person or via a teleconference call. g. Immediately informs Director of any occurrence that may pose risk to the client, employee or the Company 3. Interview, orientation and follow up of new direct care staff a. Conducts drug screening b. Interviews applicants for open field positions c. Consults with Director of Private Duty regarding rate of pay for job offer d. Submits new hire file to HR for follow up with required documentation and to schedule orientation e. Responsible for clinical orientation of new field employees; ensures new hire proficiency in navigating Generations for schedule and client care f. Ensures new hire is introduced to Schedule Coordinator and get scheduled for open shifts before leaving orientation g. Coordinates on site orientation to client s care with Schedule Coordinator as needed h. Contacts new hire within a week after orientation to ensure connection with the company, address concerns, and offer assistance as needed i. Ensures new hire knows his/her assigned CNM for the duration of employment with MPI 4. Assistance in the clinical administration and clinical management of Private Duty services a. Assists the Director with writing/revision of policies and procedures b. Assists Director with periodic review and update of employee and client handbooks c. Participates in agency quality assurance activities d. Empowers others by sharing information, resources and responsibility to encourage commitment and company allegiance e. Completes work with accuracy and within agency time frames f. g. Maintains clinical expertise through continuing education opportunities and retraining h. Sets an example for a strong positive professional image i. Sets an example for cooperation and teamwork j. Requires minimal supervision and is self-directed k. Makes every attempt to demonstrate fiscal awareness and assists in ways to reduce costs of care l. Assists Payroll and Billing Specialist by ensuring field staff compliance with clocking in/out and completion of tasks in Generations m. Helps to ensure timely and accurate billing by completing all required information on intake and admission n. Reports all work related injuries to Director of Private Duty and completes incident report 5. Provision/coordination of staff training and education a. Assists Human Resources with new hire and annual mandatory staff training (universal precautions, harassment, workplace safety, HIPPA, and others as assigned). b. Assists Human Resources with follow up on those who have not completed training c. Provides client specific training to staff as necessary d. Completes on-site skills competency with documentation of competency e. Develops/teaches training programs as requested f. Responsible for ongoing in-service training of field staff 6. On call after hours on a weekly rotation or more often as needed 7. Other duties as assigned Qualifications Valid State of Michigan Registered Nurse License required Valid State of Michigan Driver s and current automobile insurance required Two (2) years clinical experience in home care or three (3) years in medical, surgical or critical care within the last five (5) years Experience in the care of clients with spinal cord injury, traumatic brain Injury, complex wounds and on mechanical ventilators One (1) year line management experience Travel within assigned service area, and outside service area as needed Must be able to work on call after hours, as required, for clinical needs and/or problems.
