Presented by: Kristy Thomson, COO HomeTown Health, LLC Project: Care Management: Welcome and Introductions HomeTown Health Biography Kristy Thomson joined the HomeTown Health team in 2006 and served for 9 years as the Director of Education where she has directed the education services offered to HomeTown s hospital members and their affiliated clinics and physician practices. In this role, Kristy was responsible for course curriculum, content, technical set up and instructors, as well as directing live and online education services offered to HomeTown s network of 60+ rural hospitals and their affiliated clinics and physician practices along with 60 supporting business partners. She was also responsible for compliance with the IACET/ANSI continuing education and training standards along with other accreditation standards and assisted in preparation, review and administration of state and federal grants. In 2015, Kristy moved into the Chief Operating Officer position for HomeTown Health where she is responsible for both the HomeTown Health operations and the education programs of HomeTown Health University. 1

Who is HomeTown Health? HomeTown Health, LLC (HTH) is an organization that was started 18 years ago in GA to pursue ways to help rural and small hospitals survive in this environment of tremendous budget cuts from the state and federal level HTH works hospitals through grant work and education programs nationwide In 2016, the Iowa Department of Public Health issued a Request for Proposal to contract out the management oversight and coordination of the SHIP program activities and services HTH was selected for years 2016-2019 As Project Manager, HomeTown Health's responsibility is to provide comprehensive management and oversight of the State of Iowa s SHIP Program, such as reporting, budgeting and managing other grant requirements with HRSA HTH also charged with the coordination of group purchases of goods and services that align with the participating hospitals designated activities, as submitted/requested on the SHIP applications Welcome & Introductions Learning Outcomes: By the end of this session, attendees should be able to: Identify the purpose of this workshop Describe resources available for the Project participants A Workshop is A room or building where things are created or repaired A discussion group that emphasizes exchange of ideas and the demonstration and application of techniques and skills Using intensive group discussion & improvisation in order to explore before implementation Intensive educational program that focuses especially on techniques and skills in a particular field A period of discussion or practical work which a group of people share their knowledge or experience Training in which the participants work individually or in groups to solve actual work related tasks to gain hands-on experience 2

Her passion to make a difference flourished from that point forward. She started 23 years ago as an RN, and has had a vast career in bedside nursing; Skilled Care settings; Managerial & Administrative positions; and regulatory/compliance oversight. She understands the importance of coordinated patient care, and strives to assist healthcare settings achieve this through her current position with ICAHN and IRCCO as the Care Coordination Specialist. 5

Project: Care Management: Closing & Evaluations Presented by: Jennie Price, Director of Business Development HomeTown Health, LLC Speaker Biography Closing & Evaluations Jennie Price joined the HomeTown Health team in May 2014 with experience in healthcare risk management and marketing, and having worked with a number of member hospitals. Jennie s specialties, outside of marketing, include professional liability such as medical malpractice, and property/casualty insurance. She holds a Bachelor s degree from Berry College in Rome, GA, in communication, as well as Bachelor of Arts degree in Religious Studies. She has earned a variety of designations in the area of insurance and risk management, including the Accredited Advisor of Insurance (AAI), Registered Professional Liability Underwriter (RPLU), and Associate in Risk Management (ARM).
