Guide to Fulton County Public Health Programs and Services for,, & OUR VISION: January 2019 All People Are Healthy

The Fulton County Board of Health (FCBOH) offers many programs to serve children, adolescents and women. The following is a brief guide to understanding the various programs and services offered by the FCBOH. This guide was developed for use by health and social service providers. PROGRAM NAME POPULATION Adolescent Health and Youth Development (AHYD) Youth 10 19 years AHYD provides a network of communitybased support through youth development educators focused on enhancing the skills and improving the health status of Fulton County adolescents. This program uses evidence-based curricula aimed at reinforcing positive attitudes, healthy behaviors and activities to decrease violence, substance abuse, poor school performance and early sexual activity. Ages 10 19 years Fulton County resident Youth Development 404-613-3791 Babies Can t Wait (BCW) Birth 3 years This program provides early intervention services for infants and toddlers with developmental delays or disabilities and their families. Services include evaluation, service coordination, and other services identified through the development of the Individualized Family Service Plan (IFSP). BCW provides screening and assessment for speech/language, physical and occupational therapy sessions, referrals to other MCH programs, community services, and transition to formal school settings (pre-k, special day care). Diagnosed medical condition resulting in a delay or a significant delay in 1 or more developmental areas or moderate delays in 2 or more developmental areas; no cost for evaluation and service coordination; fees for other services are based on family cost participation BCW Program Manager 404-612-4017 Breast and Cervical Cancer Program (BCCP) 40 64 years BCCP provides breast exams, cytology screenings, HPV testing, referrals for screening and/or diagnostic mammograms, additional diagnostic procedures, biopsies, and case management. At least one year since the last screening mammogram; 40 64 years, under 40 years with positive symptoms; low-income, uninsured, and underinsured women (based on Federal Poverty Guidelines) District BCCP Cervical Cancer Screening Program (CCSP) 21 years and older CCSP provides breast exams, cytology screenings, HPV testing, referrals for screening and/or diagnostic mammograms, additional diagnostic procedures, biopsies, and case management. 21 years and older who qualify to have a pap test or referral for diagnostic procedures District BCCP 2

Child Passenger Safety Seat Program (CPSS), adolescents, and their parents CPSS reinforces the importance of the proper use of car seats, booster seats, and seat belts, through education, advocacy, training and hands-on demonstrations; provides child passenger safety seats to eligible families. Contact Safe Kids for eligibility Safe Kids 404-612-4236 1st / First Care Birth 5 years 1st is the entry point into all child public health services. Its purpose is to identify, screen, and link children birth to 5 years of age who are at risk of poor health and/or developmental outcomes to appropriate public health or community services. This program ensures infants and children are linked to a medical home. First Care provides health and developmental monitoring for infants born weighing less than 1500 grams, which is considered very low birth weight (VLBW). These services are provided by nurses. No cost for children identified at risk of poor health outcome and /or developmental delays as determined by medical diagnosis Serves VLBW infants up to age 1. 1st / First Care 404-612-8770 s Medical Services (CMS) Birth 21 years CMS provides care coordination to assure necessary comprehensive communitybased specialty health care for children and youth, and transition services to adult health care. Provides access to specialized medical visits, medication, durable medical equipment, personal care items, glasses, dental services, in-patient and out-patient surgeries, genetic services, diagnostic studies, physical/occupational/speech therapies, and transportation. No cost for children identified at risk of poor health outcome and /or developmental delays as determined by medical diagnosis and family cost participation CMS Program Manager 404-612-1496 s Oral Health Infants, school-age children, and adolescents The s Oral Health program promotes prevention, education, screening, early detection, dental sealants, comprehensive oral health treatment and control of oral diseases in children. Enrolled in Medicaid and PeachCare for Kids programs, or Reduced/ Free Lunch programs, and/or low-income patients on a sliding fee scale (based on ability to pay) Oral Health Administrator 404-612-2730 Early Hearing Detection and Intervention (EHDI) Birth 6 months EHDI promotes and provides hearing screening and appropriate follow-up for all newborns and infants through education, technical assistance and training. Hearing aids are provided when indicated, as well as referrals to other maternal and child health programs where appropriate. Family Planning* All genders of childbearing age (10 49 years) Men/Boys 11 45 years The Family Planning program assists with the spacing or prevention of pregnancies by providing comprehensive physical examinations, sexually transmitted infection screenings and contraceptives to adolescents, women and men. Services are provided to both men and women and include physical exams; birth control counseling and supplies; abstinence skills training; immunization; and screening for cancer, high blood pressure, diabetes, HIV and other sexually transmitted infections (STIs). Also provides screening, counseling and referral for risk factors affecting health such as substance abuse, poor nutrition, cigarette smoking and exposure to violence. Sliding scale fee available for Fulton County residents District Family Planning Health Check Program Medicaideligible and non-medicaidcovered children from birth to 21 years; PeachCare for Kids -eligible children from birth to 19 years Health Check is a preventive health care program that provides physical assessments for children to ensure that they are healthy and are reaching their expected physical and developmental milestones. Medicaid, PeachCare for Kids, fee for service, or Care Management Organizations (CMOs); and some private insurance accepted; sliding scale based on proof of income. Flat $56 fee for uninsured individuals not eligible for sliding scale. Call centers for availability and additional details. Hearing and Vision Screening Program in Preschool, Pre-K, or any student entering a Georgia school for the first time The Hearing and Vision Screening Program provides early detection and intervention of vision and/or hearing defects through screening and referral to vision and hearing specialists. Medicaid and some private insurance accepted; fee for service Immunizations* Adults The Immunizations program is intended to ensure adults are adequately immunized. Allows providers to immunize eligible adults for a nominal fee. Medicaid, Medicare, and some private insurance accepted Immunization 404-612-1677 Newborn Screening for Sickle Cell Disorders Program Newborns Newborn Screening for Sickle Cell Disorders Program ensures that all infants receive appropriate screening, follow-up, and medical services for genetic disorders, and endocrine conditions. Oak Hill Teen Clinic All genders, 12 21 years The Teen Clinic provides comprehensive physical and school exams, sports physicals, sexually transmitted infection screenings, HIV screenings, abstinence counseling, a variety of contraceptives, and case management. All teens placed on sliding scale fee. Medicaid accepted. No adolescent is turned away because of inability to pay. Family Planning 404-612-0626 Teen Clinic Parents as Teachers (PAT) Home Visiting Services in their first trimester of pregnancy; mothers of African-American children birth 3 years Parents as Teachers is an evidence-based home visitation program that promotes optimal health and development of children by supporting and engaging their parents and caregivers. The program honors the concept that a parent is a child s first and most important teacher. in their first trimester of pregnancy and/or who have children birth to 3 years of age, residing in specific Fulton County zip codes, and at risk for poor birth outcomes (women) or poor developmental and/or health outcomes (child) Parents as Teachers Program Manager 404-612-4111 Perinatal Case Management (PCM) Pregnant Medicaidinsured women with identifiable medical, nutritional and/ or psychosocial problems A Medicaid program that pays for a case manager (via referral program to CMO s and other public health services) to assist a pregnant woman with identifying her special needs and helping her find ways to meet those needs in order to improve maternal and infant health outcomes. Must be a pregnant Medicaid-insured woman with one or more of the following a medical, nutritional and/or psychosocial problem and not assigned to a CMO (Care Management Organization) Planning for Healthy Babies (P4HB) who have given birth to a low birth weight (LBW) baby or a very low weight baby (VLBW) Planning for Healthy Babies aims to reduce low birth weight (LBW less than 5lbs. 3oz.) and very low birth weight (VLBW less than 3lbs. 5 oz.) rates in Fulton County. The program provides family planning, inter-pregnancy care (IPC) health services (which include Family Planning, Case Management, and a Resource Mother), and Resource Mother Outreach services. Must be enrolled in a Care Management Organization (CMO); be a U.S. citizen between18 and 44 years of age; be a Georgia resident; not be eligible for any other Medicaid or CMO program; have annual income < 200% of Federal Poverty Level District Family Planning For more information, visit fultoncountyboh.org. 5

Pregnancy Testing All women/girls requesting a pregnancy test Seeks to increase the number of uninsured or underinsured pregnant women receiving early prenatal care and other services such as WIC. with a history of a missed menstrual period are administered a pregnancy test and offered Presumptive Medicaid services if the test is positive. They are encouraged to begin prenatal care services within 15 days. with a negative pregnancy test result are offered family planning services. These women receive education and counseling on pregnancy planning or spacing, preventing unplanned pregnancies and contraceptive options. Missed at least one menstrual cycle; service fee required; all positive tests are billed to Medicaid when they apply for Presumptive Medicaid through the health department at their visit. Presumptive Eligibility Medicaid Pregnant residents of Georgia, any age Allows immediate, but temporary, limited Medicaid coverage for prenatal care (excludes inpatient hospital and delivery services). Coverage is effective until Medicaid application is fully processed. State Office of Medicaid will confirm eligibility. Must be a U. S. Citizen or legal Georgia resident with income < or = to 200% of Federal Poverty Level; proof of pregnancy is provided by a Public Health Registered Nurse. District Family Planning Ryan White* Adults, 18 years and older, diagnosed with HIV/AIDS The Ryan White Program provides primary medical care and essential support services for people living with HIV and/or AIDS who are uninsured or underinsured. Georgia resident residing within 20 county EMA (Eligible Metropolitan Area), with income < or = to 400% of the Federal Poverty Level Services by appointment only Monday Friday, 8:30 a.m. 3:30 p.m. Phone: 404-613-1430 Respiratory Health (Tuberculosis) Program* All ages, all genders at risk for Tuberculosis (TB) infection or TB disease The Respiratory Health (Tuberculosis) Program provides screening, testing, diagnosis, and treatment for individuals at risk for TB infection or TB disease. Medicare/Medicaid, and some private insurance accepted; also fee-forservice with a sliding scale based on income. Residents or potential residents of transitional housing facilities can provide a letter from the shelter for free screening. Respiratory Health Manager 404-613-1551 Sexual Health Clinic* All genders 13 years and older The Sexual Health Clinic provides screening, diagnosis, and treatment for sexually transmitted infections (including HIV testing), as well as pre-exposure HIV prophylaxis (PrEP). Medicare/Medicaid and some private insurance accepted; also, fee-forservice with a sliding scale based on income Records Supervisor 404-613-1442 6 *Services available for men also

Sexual Health Program* Persons at high risk for HIV; persons diagnosed with sexually transmitted infections (STI/ STD) SHP provides HIV testing and linkage to treatment and PrEP through funding community-based agencies focused on providing testing among populations at high risk for HIV infection. The program further provides HIV testing and linkage to treatment or PrEP for those who come to the Fulton County Board of Health Sexual Health Clinic. Finally, SHP provides partner services for persons diagnosed with syphilis or HIV to alert their current and former sexual partners of their potential infection without the partners knowing who reported that they had sexual contact with them. and men of all ages at risk for HIV and/ or STI/STDs, regardless of insurance status Sexual Health Clinic 404-613-1538 Travel Clinic* International travelers; 2 months and older Clinic provides destination-specific vaccines, education and travel health advisories to anyone traveling outside of the U.S. continent. Fee for service. Clients may contact their private insurance for reimbursement. Travel Clinic Administration Vaccines for (VFC) Birth 18 years The VFC program is intended to help raise childhood immunization levels for infants and young children. Allows providers to immunize eligible children for a nominal administrative fee. Encourages participation of private healthcare providers to reach children who might not otherwise receive vaccines because of financial barriers, or who might receive vaccines late because they would be referred to another setting for free vaccines. Medicaid/PeachCare for Kids enrolled; uninsured (child has no health insurance coverage); underinsured (child has health insurance but immunizations are not a covered benefit); or American Indian or Alaska Native Immunization 404-612-1677, Infants, and (WIC) Pregnant, postpartum, and breastfeeding mothers, and children up to 5 years The Special Supplemental Nutrition Program for, Infants, and (WIC) provides nutrition assessment and education, breastfeeding support, monthly food vouchers and refers applicants to available health and social services.
