Santiago Canyon College High School Scholarship Program APPLICATION PACKAGE DUE FRIDAY, APRIL 14, 2006* Waltmar Honors Program Scholarship Deadline: October 2, 2006 1 This 2006 application package contains all the information that you will need to apply for the four scholarships as listed below. All required information MUST be included for your application to be considered complete. Please read all instructions carefully. Santiago Canyon College (as part of the Rancho Santiago Community College District) sponsors a scholarship program which recognizes both academic and specialized achievement among seniors graduating from high schools within the SCC service area who will be attending SCC as a full-time student in the Fall 2006 and Spring 2007 semesters. A ceremony to award scholarships will be held on Thursday, May 11, 2006 at Santiago Canyon College. SCC LEADERSHIP & ACHIEVEMENT SCHOLARSHIP HORACE SCOTT LEADERSHIP & ACHIEVEMENT SCHOLARSHIP Community Involvement: Volunteer public or private. Employment: Work outside school as well as any jobs held on campus. Number & Amount of Award: Two $500 awards will be available to high schools in the Santiago Canyon College service area, see list below. ORANGE ROTARY HIGH SCHOOL TO THE FUTURE SCHOLARSHIP Please Note: This scholarship is limited to students graduating from Canyon, El Modena, Orange, Richland and Villa Park High Schools ONLY. No other applicants will be considered. Community Involvement: Documented volunteer work in service to others. Employment: Work outside school as well as any jobs held on campus. Applicants must complete an Essay of Interest, a brief (60 words max) biographical statement. Number & Amount of Award: One award given for 2-year college program, not to exceed $1,100/yearly. Santiago Canyon College High School Scholarship Program APPLICATION PROCEDURES 3 Please Read ALL Directions Carefully Before Completing the Attached Application Form. 1. Complete the attached application form, all attachments and biographical statement and return to: SCC Foundation/Scholarship Office Santiago Canyon College 8045 E. Chapman Avenue, A-206-A Orange, CA 92869 PLEASE NOTE: Located at the top of the following Application Form are check boxes for each scholarship. Be sure to check each of the scholarship(s) for which you meet the minimum requirements. You may check more than one, however, if you do NOT check the box you may not be considered for that scholarship. 2. The following items MUST be attached to your application: a. An official high school transcript which includes the Fall 2005 semester of your senior year and your Spring 2006 work in progress. The Waltmar Honors Program Scholarship requires Spring 2006 final grades. b. One to three letters of recommendation from people who know first-hand of your achievements and your qualifications for the scholarship, with the following exception, the Waltmar Honors Program Scholarship requires two letters of recommendation; c. Your Biographical Statement (may be used as the Essay of Interest for the Orange Rotary Scholarship); d. A copy of: Fee Application for Federal Student Assistance (Financial Aid application)*; Or The first two pages of your parent s 1040 or 1040 EZ tax form.* *Not required if only applying for the Leadership & Achievement and/or Honors Program Scholarships. 3. Completed applications are due April 14, 2006 to the Scholarship Office (A-206-A) located at Santiago Canyon College, 8045 E. Chapman Avenue, Orange. R E M E M B E R : Applications are NOT considered complete unless all necessary items are attached and the application is SIGNED. Incomplete applications will NOT be considered. All award monies will be disbursed in the following manner: 1. Half to be awarded to the student for full-time (12 or more units) enrollment in the Fall 2006 semester at Santiago Canyon College and the balance awarded for the Spring 2007 semester, except for the Waltmar Honors Program Scholarships. 2. The second increment is contingent upon successful completion of your Fall 06 semester and registration as a full-time student in the Spring 07 semester at Santiago Canyon College, except for the Waltmar Honors Program Scholarships. 3. For clarification of disbursement of monies for the Waltmar Honors Program Scholarship, please read the Waltmar Honors Program Scholarship Information sheet. IMPORTANT NOTE: All high school scholarships are awarded to graduating high school seniors who will be attending SANTIAGO CANYON COLLEGE as a FULLTIME (12 or more units) student for the FALL 2006 semester. SCC High School Scholarship Program Application WALTMAR HONORS PROGRAM SCHOLARSHIP Application for 2006-2007 Scholarship Program Office (714) 628-4793 APPLICATION DEADLINE: OCTOBER 2, 2006 Please review the Waltmar Honors Program Scholarship description and qualifications carefully. All requirements must be met BEFORE semester funds will be disbursed. Recipients will be announced at the Fall 2006 SCC Honors Program Retreat. (Check SCC Honors Program website for the date.) DESCRIPTION The SCC Honors Program Scholarships are designed to recruit entering freshman from local high schools to participate in the SCC Honors Program. The purpose of the scholarship is to encourage students who might otherwise have transferred to the university as freshman to consider an avenue which may ultimately give them more choices and opportunities. Thanks to a donation from the Waltmar Foundation, SCC will offer five $500 scholarships to be used toward enrollment fees, books, lab fees, and other classroom expenses. Once all required criteria have been verified by the SCC Scholarship Office & the SCC Honors Program, the first $250 will be awarded for the spring semester. The second $250 will be awarded the following fall semester contingent upon maintaining the minimum requirements for recipients (see below) the first semester. One $500 scholarship will go to an academically outstanding student at each of the following local high schools: Canyon, El Modena, Orange, Villa Park and to one of the following high schools: Esperanza, Valencia, Foothill, Lutheran or Tustin High School MINIMUM CRITERIA FOR APPLICANTS Entering freshman from one of the above high schools A minimum high school overall GPA of 3.00 Must become a member of the SCC Honors Program and be enrolled in at least one honors class in fall of 2006 Two (2) letters of recommendation on letterhead (at least one from a school official) addressing any of the following: academic performance, character, extracurricular/leadership activities, academic goals; Interview may be required Financial need may be considered. ADDITIONAL CRITERIA TO BE CONSIDERED IN EVALUATING APPLICANTS: GPA above 3.00; Depth and breadth of courses taken in high school; Beyond classroom activities: campus, civic, family activities and service; Degree goal. REQUIREMENTS FOR RECIPIENTS Must complete a minimum of 3 Honors courses in the first year of enrollment with at least one the first semester and English 101H the first year; Must be a full-time SCC student (12 units minimum); Must maintain an overall SCC GPA of 3.0 both semesters; Must attend the Spring 2007 Scholarship Awards Ceremony. REMEMBER: Semester funds will not be disbursed until all requirements have been met. 4 PROCEDURE TO APPLY Complete both sides of the attached application form. Be sure to check if the scholarship your are applying for requires this information. If you have completed the Free Application for Federal Student Assistance (financial aid application) please attach a copy. If you have not, attach a copy of the first two pages of your parent s 1040 or 1040 EZ tax form OR your 1040 tax form if you are an independent student. EXTRACURRICULAR ACTIVITIES AND WORK EXPERIENCE: Specify what you were involved in, any special roles or functions you performed, and number of years involved. Attach additional sheet if necessary. Include your name in the upper right hand corner and limit the length to no more than 2 double-spaced, typed pages. Include any or all of the following topics: 1) Your interest in your intended major, how your interest developed and how it related to your educational and career goals; 2) Information about your family and work experience that contributed to your personal growth and/or education and career goal; 3) Information about your school activities and involvement in community service that contributed to your personal growth and/or education and career goals: 4) Why it is important for you to receive this scholarship. IF YOU ARE APPLYING FOR THE WALTMAR HONORS PROGRAM SCHOLAR- SHIP ALSO INCLUDE THE FOLLOWING: 1) The importance of academic excellence in your life; and 2) What you hope to receive from and give to the SCC Honors Program. RECOMMENDATION LETTERS: One to three letters of recommendation must be included with your application. With the following exception: The Waltmar Honors Program Scholarship requires 2 letters. These letters should be from people who know first hand or your achievements and your qualifications for the scholarship. For example: an academic recommendation from a teacher; a leadership recommendation from a club advisor or community leader; and/or a general recommendation from a supervisor at work or a mentor. RELEASE: I intend to enroll at Santiago Canyon College as a full time (12 or more units) student beginning in the Fall 2006 semester.
