As we begin preparing name badges for the event, we will need an accurate list of those who will be working your booth in order to alleviate last minute changes. Please fill out the form included in this packet and return to our office along with the other requested materials. Remember only two name badges per paid booth space are included with registration. Companies requiring more than two badges will be charged $50 for each additional badge.
