Undergraduate Scholarships / Awards Worksheet This worksheet is provided for you to collect and organize your information in preparation for submission on the official on-line application form for School of Information Technology scholarships and awards to be awarded to current students in April 2018. To be considered for a scholarship or award you must complete and submit the official on-line application form. All of the information requested on this form is required in order to assure your full consideration for all eligible scholarships and awards. Applications must be submitted on-line by Friday, February 9, 2018. You may apply for an award, a scholarship or both. Eligibility: The School of Information Technology scholarships and awards are limited. Only undergraduate students currently enrolled in an IT major (e.g., Computer Science, Cybersecurity, Information Systems, Network & Telecommunications Management) are eligible to apply. Requirements for Scholarships: You must be enrolled as an undergraduate in an IT major to apply for a scholarship. You must remain an IT major to continue receive the scholarship. Some scholarships will be awarded based on academic merit; others will be awarded based on a combination of academic merit and need. Requirements for Awards: You must be enrolled as an undergraduate in an IT major to apply for or receive an award. Awards are based on academic merit. Scholarships/Awards Reception All School Awards will be presented at the Celebrate IT event, on Friday, April 27, 2018. All students enrolled in IT majors are encouraged to attend this event. Award recipients are required to attend this event. Award recipients will be notified in advance of the event. There will be a separate reception for scholarship recipients in the fall. Scholarship donors will be on hand for the scholarship presentations and students selected to receive scholarships are required to attend. Students receiving scholarships will be required to send a formal thank you note to the scholarship donor and to attend any special events hosted by the donor. The school will help facilitate the thank you.
