Innovation Award Submissions Submissions Must be received by COB on: Friday 17 May 2019 Judging Will be completed by: Friday 31 May 2019 Notification of Acceptance/ Decline Will be forwarded by email no later than: Friday 7 June 2019 Final Judging Innovation Session at the Conference on: Monday 19 August 2019 Awards Presentation Winners will be announced and presented with their Award and Certificate at the Conference Dinner on: Wednesday 21 August 2019 Sponsor 2019 Innovation Award winner Prize is sponsored by uvex safety Australia Aim The Queensland Mining Industry Health and Safety Conference Innovation Awards aim to recognise creative and practical solutions to safety and occupational health problems and to promote their application across the Australian Resources Industry. Entry Entry is open to all Australian resources employees, companies, sites, contractors (minerals, coal, quarries, exploration, oil and energy), associated with the Queensland resources industry. There is no limit on the number of entries that may be submitted. Scope For the purpose of these Awards Innovation is broadly defined as either a new initiative or an original solution to a specific safety and occupational health problem and must not include products for commercial sale or those seeking opportunities for production and commercial sale. Innovations eligible for the Awards include: Solutions to specific safety problems (e.g. design of safer, more efficient access to plant for maintenance purposes, risk assessment, etc.); Solutions to an identified occupational health hazard that will minimise workers exposure by managing the hazard at its source or interrupting its pathway and that will provide a measurable long term benefit (e.g. noise; ergonomic solutions, etc); Selection, design and/or safe introduction of new equipment or processes; Development of a safer/healthier work procedure (e.g. new specific maintenance changed-out method); Introduction of innovative occupational health and safety initiatives and/or training programs (e.g. safety culture initiatives, fitness for duty programs, accident investigation methods, first-aid training, education programs); This list is indicative only. Submissions on health and or safety Innovations which do not fit the above scope are also encouraged. Submissions will be included on the Conference Handbook and Website, at the discretion of the Conference Committee. Judging Process There are two rounds of judging: Preliminary Judging Round All submissions are assessed by a Judging Panel comprised of selected representatives across industry sectors. From these submissions the Panel will select Ten Finalists to go forward to be presented at the Conference. Final Judging Round This judging will be carried out at the Conference on Monday where each of the finalists will present their Innovation to Conference Delegates and in front of the Judging Panel. Following the presentation the Panel may ask questions about your Innovation. Judging Criteria Your submission must include text under the following bold headings in the order they are listed to attract the maximum potential score value. The Problem: Score value - 10% Provide a brief description of the identified problem, or where a process has been improved, describe how the initiative or improvement opportunity was identified. Submissions will be judged on: How the problem has been identified and the safety and health consequences that have been addressed. Where no problem exists, how the initiative identified improvement opportunities. The Solution: Score value - 30% Describe the strategies and initiatives developed to identify and address the problem or improve the current system, including: The internal/external resources used in the process; Methods used in trialing and testing; Implementation process where/how on site; Demonstrate how the hierarchy of control has been applied. Benefits/Effects: Score value - 20% Explain the safety and/or occupational health benefits. You should provide any supporting data or other evidence that demonstrates the benefits/effects that were achieved and describe the extent of deployment across the site. Transferability: Score value - 10% Describe the potential for the Innovation to be used, modified or transferred across the resources industry and how this could be achieved. Innovation: Score value - 30% Describe the originality of the Innovation. Approximate Cost A statement of approximate cost, if known, should be included. While there are no strict requirements to demonstrate cost effectiveness, where it is otherwise difficult to separate nominations, the judging panel may use this to help decide the ultimate winner. 3 www.qldminingsafety.org.au

Awards Categories Winner - as judged by the Industry Judging Panel at the Conference. People s Choice - as judged by the Conference Delegates. Highly Commended - combined vote count of the Judging Panel and Conference Delegates. Winning Prize - proudly sponsored by uvex safety Australia The Winner of the Award will receive the Queensland Mining Industry Innovation Award Trophy and a Framed Certificate. The winning organisation will also have the opportunity to nominate one person associated with the development of the Innovation to attend the A + A Trade Exhibition in Dusseldorf in November as a guest of our sponsor uvex safety Australia. A+A is the number one international platform for safety, security and health at work. The People s Choice and Highly Commended Submissions will receive a Queensland Mining Industry Innovation Award Trophy and a framed Certificate. How to Enter 1. Complete the official entry form (Page 7). 2. Prepare a description of no more than 1500 words. If this number is exceeded you will be asked to resubmit. The description should address the submission criteria and use the same sub-headings as outlined in the judging criteria section. Submissions may include an appropriate selection of photographs, up to a maximum of 5, diagrams and video etc. These are to be provided in electronic format (tiff or jpg), each with a caption; video should be in mpeg or wmv. Please select one photo of the Innovation that can be used for publication in Conference material. 3. Prepare a Short Form Description of the submission, limited to one A4 page, including up to 2 photos, with captions, using the guidelines below including headings 1 and 2. It is not necessary to incorporate all the sub headings in this description. Guidelines for Submission: Format: Adobe PDF Edit Enabled. Font: Arial. Page: A4 with no company logo or designs. Heading 1 Main title of the Innovation: 14pt Bold (a maximum of 50 characters with spaces) justified to the left of the page in upper and lower case NOT UPPER CASE. Heading 2 Company/Organisation name immediately under the main title: 14pt justified to the left of the page in upper and lower case NOT UPPER CASE. Heading 3 Sub-headings (e.g. The Problem): 12pt justified to the left of the page in upper and lower case NOT UPPER CASE. Text The rest of the text must be in 12pt, sentence case, single line spacing, with double spacing between paragraphs. Forwarding Information: Please forward your submission by email OR post/courier, if files are too large, with the following items attached as separate files: Innovation Entry Form. Submission this should be made as an Adobe PDF, which should be saved as Edit Enabled to allow copy and paste. Photos and captions are to be shown in the preferred position in the submission. All photos included in the submission are also to be attached separately as individual tiff or jpg files. Videos are to be sent as mpeg or wmv file. Short Form Description of the submission (in same format as main submission - see paragraph 3 above). One photo is to be selected and sent as tiff or jpg file and identified as the one for publicity purposes. Company letter incorporating the information listed in the Conditions of Entry on page 6. 4. Preliminary Judging Round will be completed by Friday 31 May 2019 and will involve a Judging Panel reviewing all submissions. A list of up to 10 finalists will be selected from this process. All applicants will be notified by email no later than Friday 7 June 2019. 5. Final Judging Round will be held on Monday 19 August 2019 as part of the Conference program. Each finalist will have the opportunity to present their Innovation to the Judging Panel and Conference Delegates. 6. Presentation time of 10 minutes will be allocated to each finalist, with additional time for questions from the Judging Panel. 7. Your presentation will be video recorded, synchronised to your PowerPoint presentation and placed on the Conference website. Provision has been made for you to liaise with the recording contractor to amend any of your PowerPoint slides prior to publication. Awards Presentation The presentation of Awards will be held at the Conference Dinner on Wednesday 21 August 2019. All participants are encouraged to attend the Awards presentation to promote the application of the Innovations across the industry. Innovation Profiles To promote information sharing, the finalists Short Form Descriptions will be included in the Handbook supplied to all Delegates attending the Conference. The complete Innovation Submission will be published on the Conference website. Poster Display and Presentation Session Everyone who provides a submission is encouraged to prepare a Poster for displaying at Conference. Dedicated times will be set aside for Authors to engage more closely with Delegates. Guidelines for preparing posters will be provided. Static Demonstration Space will be set aside in the Event Centre Gallery for those Finalists whose innovation lends itself to a small static demonstration. Please indicate your intention on the Entry Form. More details will be provided to those taking up this option. Permission to Publish An authorised person must provide written permission for the Conference Committee to publish the Innovation Submission in the Conference Handbook and on the Conference website. The Winning Prize proudly sponsored by uvex safety Australia The winner of the Queensland Mining Industry Health and Safety Innovation Award for 2019 will have the opportunity to nominate one person associated with the development of the innovation to travel with Team Members of UVEX SAFETY AUSTRALIA LIMITED PARTNERSHIP (ABN:56 417 194 966) during the period 4-8 November 2019 to participate in activities that may include: A+A International Trade Show attendance in Dusseldorf, Germany for 2 days between 5-8 November 2019 which will include: Being special guest of honour at the uvex exhibition stand (approx. 850 sqm with up to 120 uvex representatives) Meet and greet by CEO of the uvex safety group of Companies Introduction to all Directors of all Strategic Business Units Personalised introduction and training by the head of each Strategic Business Unit (SBU) to all new uvex innovations Products and service concepts which will be released and presented at the A+A Visit the largest workplace safety, health at work and security related trade event in the World (approx. 1,500 exhibitors and 60,000 + visitors) Meetings, information and experience exchange with safety experts from around the world and many industries uvex s Global Head Office and Training Centre visit in Fuerth, Bavaria, Germany including: Guided factory tour, the uvex academy and Training Centre, Research & Development facilities and Laboratories Innovation workshops with key uvex Management Team and other selected uvex visitors Experience uvex hospitality and typically German Gemuetlichkeit in Bavaria and Rhineland. This prize includes: Economy class return airfare for one person to Germany. Airlines and carriers will be chosen and arranged by uvex safety Australia Domestic transport in Germany by plane, train and or self-driven bus/people mover arranged by uvex safety Australia Accommodation, food and beverages arranged by uvex safety Australia according to uvex company policy Access to the A+A International Trade Show and certain other venues selected by uvex safety Australia The winner takes part in the competition at their own risk and must complete the relevant documentation specified below. Conditions of entry Acceptance and confirmation of the following requirements must be set out on a company letterhead and signed by an authorised person and must accompany the submission: The name and position within the company and details of their association with the entry of the person nominated to accept the prize. This prize is not transferable without the permission of UVEX SAFETY AUSTRALIA LIMITED PARTNERSHIP. Approval for the person to be released from their duties to participate in the prize during the specified dates. The company must arrange all health and travel insurances and to provide a copy to uvex safety Australia prior to departure. Confirmation that the person has a valid passport and Visa (if required) with a remaining validity of at least 6 months as from 4 November 2019 or will undertake to obtain such a passport by no later than 1 September 2019. Certified photocopies of the passport of the nominee to be attached to the acceptance letter or where the nominee does not have a current passport, copies must be provided by no later than 1 September 2019. The nominated person will not be able to collect the prize unless they and the Company has executed the waiver, release and discharge provided by UVEX SAFETY AUSTRALIA LIMITED PARTNERSHIP. uvex reserves the right to withhold the prize from the nominated recipient at any time for any reason deemed necessary by the uvex Management Board. Failure to provide an authorised letter setting out the above will void the innovation entry from being eligible to accept the prize.
