PI 11 Reference Number Lothian Community Pharmacy ST diabetes Services Task Group - Local Waist Measurement Campaign Pan Lothian Project - Patient Involvement input from Edinburgh CHP. This initiative was coinciding with the National diabetes week. Complete Patients sat on a working group to develop and progress the project. Patients reviewed and helped develop promotional material for the campaign. Two reusable and 20 disposable tape measures were provided to each pharmacy in lothian along with background information and instructions on how to measure the waist. Programme was evaluated and results used to inform further projects with community pharmacies. Supporting document No. 43 Archive at the next update PI 12 Pain Management LT The aim of the project is for community pharmacists to provide additional support and education on pain management for people prescribed drugs to treat chronic pain. Survey developed to assess the impact of this support on those with chronic pain. Six pharmacists and GP have agreed to be involved. Each pharmacist have identified 12 to 16 to be included in the project totalling to approximately 80 patients. The survey still has to be disseminated. Results will be used to feedback on the impact of pharmacist support from the patients perspective and used to put together a case for continuing funding this service and extend to other Edinburgh pharmacies. Ref to HEAT target QA08.25 PI 15 Care of the Elderly Occupational Therapy Rehab Team - Liberton Hospital LT Older people who are on rehab wards in hospitals who are actively involved in OT as part of their treatment plan. Potentially open to all 100 beds / pts. Gard. Group 5 per fortnight. Rem Group 8 per week. Met with PIW and discussed methods of involvement with patients. Have evaluated the OT gardening and reminiscing groups in terms of effectiveness and impact on patients overall function. Focus group approach using open ended questions to lead and initiate discussion and thoughts. Comments captured. Ongoing to establish if this treatment approach is an effective use of clinical time and cost effective. Positive feedback gained so far with only a few suggested improvements. PI 16 Physician Assistants LT To review the established Physician Assistant Service in diabetes and COPD from a patient perspective. TBA Outside company currently undertaking service evaluation. Edinburgh CHP PIW met with company and has developed a user survey to be used in conjunction with the evaluation. Company to progress survey used and dissemination.
