Background This research was undertaken in order to draw a picture of the role that Community Transport services play in the transport of patients to public health facilities in the South East Sydney area. In order to do this participating Community Transport groups were asked to provide information about transport provided to public health facilities during two weeks in November. The research shows which facilities Community Transport groups served, which clinics patients attended, where the patients lived, the times of their appointments and the days they travelled. Figure. Northern part of SESIAHS showing major public health facilities The research covered the northern part of the Area Health Service which includes the Local Government Areas of City of Sydney, Woollahra, Waverley, Randwick, Botany Bay, Rockdale, Kogarah, Hurstville and Sutherland Shire. The area contains major hospitals including Prince of Wales, St Vincent s and St George Hospitals and a large number of other health services. Methodology The five Community Transport groups in the region that provide transport to health facilities were involved in the survey. These were South Sydney Community Transport, Woollahra Community Transport, Randwick/Waverley Community Transport Group, St George Community Transport and Sutherland Shire Community Transport. Each printed a report from their computerised booking system covering two weeks during November ( days). These reports were then de-identified with the patient s names being replaced by a code and their street addressed being obliterated. Suburb of origin was retained. The Community Transport groups also coded each passenger according to their need for personal assistance. Information about travel to public health facilities was entered onto a database for analysis. The meaning of trips Normally when talking about the provision of Community Transport services a trip is taken to mean a one way trip (usually called a passenger trip ). In this report a trip means a one way trip to a health facility only. The return trips have not been considered because they are not always recorded on driver run sheets and it was not possible to collect this information. Service administrators have indicated to us that most trips are return trips (except for services such as hospital admissions etc.). The total number of passenger trips provided can therefore be roughly estimated by doubling the number of trips noted in this report. Provision of Health Related Transport Report - Page

Salient points from the research During the survey period Community Transport groups provided,7 return journeys to health facilities. Of these were to public health facilities, 9 to private health facilities and to GPs or medical specialists. The following comments relate only to transport to public health facilities: The volume of trips provided ranged from 7 by St George Community Transport to by Woollahra Community Transport. Fifty percent of all trips went to just two destinations, St George Hospital and Community Health (%) and Prince of Wales Community Health (%). Eighty four percent of all trips were to just seven destinations. Two health facilities that are very close together, St George Hospital and Calvary Health Care, accounted for % of all trips and % of all trips provided by the local Community Transport group. One third of all trips were provided to locations beyond the transport providers local area. These trips have significant resource implications for the transport operators as not only are the distances greater then for services provided within the local area, the drivers can be away for a great part of the day to transport just one passenger. A significant number (%) of trips were to public health facilities beyond the boundaries of the SES/IAHS. 7% of these trips were to the Royal Prince Alfred Hospital/QEII complex at Camperdown. One resident of Waterloo is being taken three times a week to and from dialysis treatment at Concord. Nearly thirty percent of trips (where a particular clinic was identified) were to either physiotherapy or hydrotherapy services. Over half of the trips were taken by passengers who made use of the service more than once during the survey period. The clinics where multi visits happened most often were Renal Units (% of visits), Hydrotherapy (9%) and Physiotherapy (%). Of the four main hospitals in the SESIAHS region, most trips to the two southern hospitals, Sutherland and St George were mainly provided by the local Community Transport operator whereas trips to the Prince of Wales and St Vincent s were mainly served by three operators. Possible corridors along which minibus shuttles could be operated can be identified to the West and North of the St George Hospital/Calvary Health Care site and to the South and to the South West of the Prince of Wales Hospital. All of the trips were provided on weekdays. Overall, a similar number of trips were provided on each day of the week. However, among individual operators there were significant differences in the number of trips provided each day. Sixty one percent of trips were provided to passengers with medium level care needs, thirty one percent to those with low care needs and eight percent to those with high care needs. Public and Private Health This study concentrates on the transport of Community Transport passengers who reside in the South East Sydney/Illawarra Area Health Service region to public health facilities. As can be seen in Table, transport to public health facilities is only part of the overall health related transport task undertaken by Community Transport groups. Trips provided to public health facilities only constitute about a third of all trips to health facilities and doctors rooms/surgeries. Table. In Sutherland and the Inner City about % of all health related transport trips were to public facilities while in the Eastern Suburbs public facilities accounted for 7% of all health related transport trips. Private health facilities that attracted significant numbers of Community Transport passengers include St George Private Hospital at Kogarah, the President Private Hospital at Gymea and St Luke s Hospital at Potts Point. Destinations The most striking thing about the data is that it shows that half of all trips were to just two destinations, St George Hospital (9% of all trips) and Prince of Wales Hospital (%). Eighty nine percent of trips were to just seven destinations. St George Hospital and Calvary Health Care are very close together and could be considered one destination for transport purposes. These two venues account for % of all trips. Perhaps a surprising result was the low demand for transport to St Vincent s Hospital. Provision of Health Related Transport Report - Page

Passengers, of course, are not required to divulge this information. Nearly a third of all trips (where the clinic was recorded) were to two types of clinic - physiotherapy and hydrotherapy. A significant number of people also attended podiatry clinics. Table. The trips to Prince of Wales Hospital were more evenly divided among transport operators. Half of these trips were provided by the local group, Randwick/Waverley Community Transport, and a third by South Sydney Community Transport. All of the trips to Sutherland Hospital were provided by the local Community Transport group and all of the trips to the Royal Prince Alfred Hospital and the QEII center were provided by South Sydney Community Transport. Fifteen of these trips were to distant facilities (e.g. Sutherland to Westmead or Randwick to Concord). These trips have significant resource implications for the transport operator. Not only are the distances greater than services provided within the local area, the drivers can be away for a great part of the day on one job. trips per day, this is dramatically reduced when an out of area trip is included on their schedule. One of the groups, Randwick/Waverley Community Transport Group, is specifically funded to take passengers to out of area locations. Table. Frequency of use of services by individual passengers Over half of the trips provided catered to passengers who made multiple use of the services over the two-week period. Multi service use varied from % of passengers at South Sydney to % at Sutherland. The high multiple use of services is not surprising given the high number of trips provided to therapy centres. They are also funded to provide out of area transport for residents of Randwick, Waverley the City of Sydney, Botany Bay and Woollahra Funding: HACC and Health Related Transport Program Resources available for health related transport: Three full-time drivers and three cars for local transport. This equates to journeys per day. The most common public health destination for RWCTG was the Prince of Wales Hospital which accounted for % of all journeys followed by War Memorial Hospital (% of trips). RWCTG also provided a number of out of area trips including six to the Renal Dialysis Clinic at Dame Edith Walker Hospital at Concord. This is the highest proportion of trips to one destination for any of the five groups surveyed. In addition, % of trips were to Calvary Healthcare which is close to the St George Hospital. This means that % of all trips to public health facilities provided by St George Community Transport went to one suburb (Kogarah). On average the group provided nearly trips a day to this area during the survey period. Of the 7 trips to St George Hospital and Calvary, (or %) were by passengers who made more than one journey in the survey period. Given the volume of travel and the regular nature of many of the trips, there may be scope to develop more group transport or share ride services to these venues. However, to do this efficiently may require the cooperation of the hospital appointments system in order to group patients by time and location in order to make group travel feasible. Health transport corridors From the data available over the two weeks of the study it is possible to identify some possible health transport corridors along which group transport services could be operated, particularly if the demand for health related transport were to rise, as has been predicted. The provision of group or share ride services would increase the number of patients that could be carried and would free individual transport resources up for use with patients who require one-to-one assistance. We have examined possible corridors to the two most common destinations, Prince of Wales Hospital and the St George Hospital/Calvary area. In the case of the Prince of Wales Hospital there would appear to be two possible corridors. Most trips during the survey period originated in a southern corridor which roughly runs parallel to the coast from Little Bay to Randwick. During the survey period trips originated from this area. Another possible corridor runs south-west from the Prince of Wales Hospital to Botany via Kingsford. Thirty three trips originated from this area during the survey period. Such share-ride services have been considered by the groups in question but it they agree that they would require the close cooperation of the Area Health appointments system - something that has not been achieved to date. An example of this type of cooperation can be found at Canterbury Hospital where the Hydrotherapy Clinic staff arrange appointments in a way that enables the local Community Transport group to bring in multiple patients at a time. The other major destination was the St George Hospital/Calvary area. There appear to be three possible candidates for corridors to this destination, west, north east and from the south via Captain Cook Bridge. The most promising corridor appears to be the western where 7 trips originated during the survey period. Table. The development of such corridor services would be most effective if passengers could be picked up all along the route, irrespective of which Community Transport group is providing the service. This will require that new ways of cooperating be developed between Community Transport operators. One possibility is the establishment of a Spare Seat Sharing Scheme, as is currently in operation in two regions of Victoria. capacity is highlighted. Other Community Transport operators or other agencies can then purchase those seats on behalf of their clients thus negating then need for them to provide a service. Passenger classification Participating Community Transport groups were asked to classify all of their passengers according to their need for assistance. The passengers were classified using the following classification tool: Statement Yes With help No Passenger is alert and orientated to time, place and person Passenger can walk from home to vehicle Passenger can manage to climb two steps Passenger can get in and out of vehicle Passenger can manage alone during appointment Passenger s carer will accompany them and provide all necessary help Passenger does not need observation during travel Passenger does not need supervision or management of challenging behaviour If boxes are only ticked on the Yes column the passenger is categorised as Low Level. If any boxes are ticked in the With Help column they are categorised as Medium Level, unless any boxes are ticked in the No column, in which case they are categorised as High Level. Low Medium High It should be noted that the classifications must be considered to be somewhat clumsy as there was a certain amount of subjective judgement involved. The figures do, however, provide a rough picture of the care needs of the passengers carried. Table.
