Quality / Cost Initiatives Government Physician Quality Reporting System (PQRS) Meaningful Use E- Prescribing Value Based Payment Modifier Primary Care Incentive Payment Commercial Shared Savings Bridges to Excellence Generic Prescribing New payment systems reward physicians for reducing total cost of care, improving quality, and eliminating hospitalizations and ED visits (population management) Physician incentive payments are becoming a larger portion of physicians annual income. 7

Bridges to Excellence Diabetes Care Recognition Program 18

BTE Diabetes Recognition at HTPN # HTPN Physicians: IM, FP, Endo Quarterly EHR data submission through approved BTE data aggregator for all eligible clinicians.

BTE Diabetes Care: Incentive Payments Upon recommendation from the Best Care Committee, HTPN submitted Diabetes data for all Internal Medicine, Family Medicine, and Endocrinology physicians to become recognized as Bridges to Excellence in Diabetes providers. Quarterly email notification of results sent to practices and physicians. Certificates given at the practice level. Disease Management Physician Champions work with physicians who were not recognized to improve scores. Current Incentives: BCBSTX offers $100/BCBSTX Diabetic patient and $500 one time bonus for recognition prior to 12/31/12. Aetna TRS offers $100/TRS diabetic member (as of July 2013)

Let s Not Forget To remember the importance of our relationship with the patient (That trust is key to improving care). To improve quality and service while protecting the care team. To create a Checklist culture but not one that causes a checklist to block your view of the patient. 22
