Request for Proposal 2019 Overview Beacon Health System Community Health is offering technical assistance for Program Development in addition to funding for Program Implementation. Community Health will select partners working to improve health in St. Joseph, Elkhart, and Marshall Counties. The objective of the RFP is to collaborate with partners to address the four priority health needs of the community. Applicants can apply for either/both Program Development and Program Implementation. However, applicants applying solely for Program Development will not receive any form of funding. Program Development Community Health will review and assess programmatic needs and, from that, help create a project plan that includes the needs assessment, overview of services/deliverables to be provided, and a customized work plan. Program Implementation Community Health will award funds to selected participants for up to three years, contingent on, addressing community health needs,having the capacity/ resources to perform, meeting program objectives, and demonstrating impact. Background Creating community health is at the core of Beacon Health System s mission. We believe promoting responsible, preventive health in our community is the right thing to do, and is key to long-term cost-effectiveness. Improving the health of our region s residents is as much social, economic, and environmental as it is a medical issue. When we look at changing the health of our community members, we look at it in two ways: through the priorities identified in our last Community Health Needs Assessment (CHNA), and through our Principles of Wellness. Principles of Wellness The best health results come from focusing efforts on the whole person: mind, body, and spirit. That s why we ve asked our community partners to connect with these three Principles of Wellness to their programming. Mind refers to feeling financial, emotional, and physical safety, as well as strong relationships and ties to the community. Physical activity, good nutrition, and mental stimulation help keep your body in good health. Finally, a healthy spirit is one content with daily activities and achievable goals, as well as having resilience and a driving purpose in life. General Criteria Creating community health is at the core of Beacon Health System s mission. To apply for program funding from Beacon Health System, please complete and submit the following request for proposal. Incomplete proposals will not be considered. 2. Requests are accepted and reviewed upon receipt. Allow up to six weeks for a decision. 3. Due to the large number of requests received, we can only approve those that meet our guidelines and priorities. Depending on available funds, requests may be denied even if they meet with the general criteria. 2

Your final submission should be submitted online or in a PDF file to AEstrada@beaconhealthsystem.org by no later than January 11, 2019 at 4:00pm EST. For fairness and consistency, applications will be assessed on only what is presented below in the Proposal Narrative. Applicants applying for both Program Development and Program Implementation do not need to file two separate applications. There are four components to the program funding request application: Proposal Cover Page Please complete cover page template attached in this document. Proposal Narrative a detailed narrative explaining your project or program. Organizational Chart a diagram that shows the structure of an organization and the relationships and relative ranks of its parts and positions/jobs in relation to the program/ project. Logic Chain* Please complete the attached one-page overview and illustration of your proposed plan to submit with your application. Budget* a detailed spreadsheet showing application of program funding. *Community partners requesting only Program Development assistance need not include the logic chain or budget. The proposal should include evidence of your alignment with Beacon Health System s mission, vision, and values which may be found at www.beaconhealthsystem.org. The proposed project or program should address one of the four priority health needs identified in the most recent CHNA, as well as one or more of the Principles of Wellness outlined above. A clear and specific request for Beacon Community Health to fulfill Narrative The proposal narrative should describe how the project or program will address the CHNA priority. Choose a priority that will encompass your project goal(s). Applicants must address each of the five sections below. For more information see the Criteria section included in this document. a. Target Audience b. Program Description c. Organizational Capacity d. Implementation Plan e. Sustainability f. Program Assessment* *Program Assessment need only be included in proposals that include Program Development Narrative Criteria The proposal narrative will be judged on: Target Audience o The project or program clearly addresses a vulnerable or underserved population within the St Joseph, Elkhart, and Marshall County communities; may include details such as age, gender, ethnic or racial groups, disease, or other pertinent determinant. Program Description o The proposal describes the project or program intervention in detail, including how it will address the designated priority health need, principle(s) of wellness, and provide community benefit. o The planned work is evidence-based or tied to existing research citing references. If this is a new effort or adaptation, the planned work must show plans to collect supportive evidence in a way that meets current scientific standards. o The proposed project or program is identifiably innovative in its approach toward problem solving. Organizational Capacity o The proposal describes the applicant organization s capacity to accomplish this project, including any necessary training and/or support. Implementation Plan o The proposed intervention is actionable: Includes a description of benchmarks and projected outcomes expected to be achieved within one year of implementation; Proposed dates for fulfilling this plan are outlined using a brief timetable of activities with dates and expected achievements; 5
Identifies at least one achievable behavioral change on the part of its participants that will positively influence the future wellness of the individual, family, or community. o The proposed intervention is measurable: Describes what data will be collected, how it will be collected, and how the data will be measured to determine if the program has achieved its projected outcomes. Data should include number served and demographics of those served by the intervention. o The proposed intervention is collaborative: Shows at least two partners that have substantial roles in the success of the project. Partner roles are clearly defined. o The proposed intervention identifies any potential concerns and outlines a brief strategy for addressing them. Sustainability o The proposal outlines the potential the project/program has to continue after Beacon Community Health s initial funding commitment is completed. o The project has characteristics that can lead to being replicated in other settings. Program Assessment o The proposal describes its current strengths and weaknesses. o The proposed intervention identifies current barriers and possible strategies for overcoming them. o The proposal describes the project s technical assistance needs for which the applicant is requesting Program Development support. Additional Documents The following should be included in an Appendix: References Include all in-text citations from the narrative (if applicable). Letters of Intent These are not letters of support they are letters written from proposed collaborators detailing the terms of the partnership with regards to this project or program (if applicable). Organizational Chart The organization chart is a diagram that shows the structure of an organization and the relationships and relative ranks of its parts and positions/jobs in relation to the program/project. Proposal Budget The proposal budget list in detail all expected expenses, and include any existing or potential sources and amounts of additional revenue streams as well as inkind contributors. The Intervention/Activity is what your program is doing to create a change. Output is the immediate effect of the intervention/activity; outputs are short-term and can be considered the first stage in moving toward community improvement. This leads to the Outcome, which is what participants do differently that shows your project affected change.
